An Unusual Congenital Heart Disease: Giant Left Atrial Appendage.

A 52-year-old female was admitted to our hospital in April 2021 with dyspnea. She was discharged from the hospital 3 weeks ago due to the diagnosis of pneumonia caused by coronavirus disease 2019 (COVID-19). Physical examination revealed an oxygen desaturation of 82%. The patient underwent computed tomography angiography (CTA) that showed a ground-glass pattern and a giant left atrial appendage ( Figure 1A ). Film array respiratory panel was negative, and pulmonary aspergillosis was diagnosed after bronchoscopy. Cardiac magnetic resonance corroborated the huge left atrial appendage ( Figure 1B

Beyond the Maze: Hybrid Ablation and Left Atrial Appendage Occlusion in Cardiac Surgery: Evidence Synthesis and the MESAGE Study Protocol.

Background and Objectives: Atrial fibrillation (AF) is the most common cardiac arrhythmia, present in up to 14-20% of patients undergoing cardiac surgery, with the number of patients expected to double within the next decade. Despite a Class I recommendation for concomitant surgical ablation and a Class I-B recommendation for left atrial appendage (LAA) occlusion in patients with AF undergoing cardiac surgery (Class IIa for endoscopic or hybrid AF ablation), both procedures remain substantially underutilized in clinical practice. The design of the Mapping atrial fibrillation after Epicardial S

The left atrial appendage: our most lethal human attachment! Surgical implications.

OBJECTIVES: To prevent death from atrial fibrillation, a cardiac disease which kills by producing emboli. Atrial fibrillation causes about 25% of strokes and increases stroke rate by five times. Over 90% of these embolic strokes are from clots originating in the left atrial appendage. This study addresses the surgical feasibility of removing the appendage to prevent future deaths in two subcategories of patients. (1) Prophylactic removal during open-heart surgery to study its safety. Theoretically, as these patients age and some develop atrial fibrillation, protection from embolic strokes woul

The clot thickens: an incompletely ligated left atrial appendage.

UNLABELLED: SummaryOur patient presented with known mechanical mitral valve endocarditis documented by 2D transesophageal echocardiogram (TOE) from a recent hospitalization at an outside facility. On admission to our center, there was no prior knowledge of an incompletely ligated left atrial appendage (LAA) according to patient- or family-reported history, review of outside records or the outside facility's 2D TOE report. A 3D TOE performed at our center to assess her pathology, since a month had passed from her prior hospitalization, revealed a LAA ligation with evidence of communication to t

Left Atrial Thrombus in the Setting of Mitral Stenosis.

A 56-year-old man with no significant past medical history presented with exertional shortness of breath. Echocardiogram, cardiac magnetic resonance, and computed tomography showed mitral stenosis and a left atrial thrombus. Left atrial thrombus formation is a well-known complication of severe mitral stenosis that can lead to systemic thromboembolism. The patient underwent mitral valve replacement, left atrial thrombus resection, and left atrial appendage closure that resulted in significant improvement in breathing.

Case Report: WATCHMAN Procedure Complicated By Atrial Perforation and Device Dislodgement.

Atrial perforation and device dislodgment are uncommon encounters after percutaneous left atrial appendage exclusion procedures. The literature describes how various endovascular and surgical approaches can be applied to treat these complications. In this case report, we describe an atrial perforation that occurred after a WATCHMAN™ FLX device (Boston Scientific) deployment. Heart manipulation with lifting was required for the atrial repair. During this maneuver, the device dislodged and was managed by surgical device extraction and left atrial appendage resection and suture closure. Minimal t

New-onset cardiac mass due to left atrial appendage inversion during cardiac surgery.

Cardiac mass assessment is challenging in the intraoperative scenario. We present the case of a 22-year-old woman with a new-onset left atrial mass during cardiopulmonary bypass following closure of a ventricular septal defect. We discuss the role of intraoperative echocardiographic examination, the differential diagnosis, and raise awareness of the left atrial appendage as a rare etiology of an acute left atrial mass.

A 79-year-old woman with atrial fibrillation and new onset of heart failure.

As an alternative to oral anticoagulation, percutaneous left atrial appendage (LAA) closure is an increasingly performed procedure to prevent arterial embolism in patients with non-valvular atrial fibrillation. Besides procedure-related complications, residual leaks, device-related thrombus formation, and dislocation of the LAA occluder have been observed during follow-up. Heart failure as a consequence of interventional LAA closure has not been reported so far. This case report describes a 79-year-old lady with permanent non-valvular atrial fibrillation presenting with New York Heart Associat

Stepwise Percutaneous Management of a Giant Coronary Pseudoaneurysm.

Coronary pseudoaneurysms are rare but potentially life-threatening complications following coronary interventions. Their management presents significant technical challenges, particularly in frail patients with contraindications to surgery. This report describes the sequential percutaneous management of a giant acquired coronary pseudoaneurysm in a 74-year-old male with several comorbidities, including atrial fibrillation, aortic stenosis, and contraindication for open-heart surgery. Exclusion was achieved using covered stents, followed by left atrial appendage closure to allow withdrawal of a…

Left atrial myxoma resection through right mini-thoracotomy in a patient with retrosternal gastric tube.

We report a case with a retrosternal gastric tube after oesophagectomy, who required left atrial myxoma resection, pulmonary vein isolation and left atrial appendage closure. A right mini-thoracotomy approach was adopted to avoid neo-oesophagus injury, and nitric oxide inhalation was useful to facilitate one-lung ventilation while dissecting the pleural adhesion.

Percutaneous Coiling for Late-Acquired Peri-Device Leak Following Left Atrial Appendage Occlusion.

A 77-year-old patient with previous left atrial appendage (LAA) closure suffered from transient ischemic attack 6 years after the initial procedure. Computed tomography (CT) revealed appendage patency related to a late-acquired semicircular peri-device leak. The leak was treated by percutaneous LAA coiling. Subsequent clinical evolution was uneventful. (Level of Difficulty: Advanced.).

Diagnostic and therapeutic pathways for the malignant left atrial appendage: European Heart Rhythm Association physician survey.

AIMS: Patients with atrial fibrillation who despite taking oral anti-coagulant therapy (OAT) suffer a stroke or systemic embolism (SSE) with … lecular-weight heparin, or continued/optimized DOAC dosage, whereas LAA closure is the last option. Left atrial appendage closure is a viable option in patients with embolic stroke despite OAT and no evidence of thrombus at TEE (empty LAA) after comprehensive diagnostic measures to exclude other sources of embolism. CONCLUSION: This EHRA survey provides a snapshot of the cont …

Acute Formation of Thrombus on a Left Atrial Appendage Occlusion Device Immediately After Detachment.

Clots can form on the left atrial surface of left atrial appendage closure devices, with subsequent thromboembolization. However, we are unaware of any reports of clots forming on the devices immediately after deployment. We report a case where an acute thrombus strand formed at the tip of an occlusion device immediately after deployment. (Level of Difficulty: Intermediate.).

Antiplatelet and Anticoagulation Therapy in Structural Heart Disease Interventions Beyond TAVI.

Transcatheter interventions for structural heart disease represent an emerging field in interventional cardiology. Undoubtedly, there is an absolute necessity for antiplatelet and/or anticoagulation treatment prior, during and post such interventions. However, currently administered regimens are mainly based in expert consensus recommendations. In the present review we aim to summarize data regarding anti platelet and/or anticoagulation treatment in the following transcatheter structural heart interventions: left atrial appendage closure, atrial septal defect closure, patent foramen oval closu…
