Do cobalt and chromium blood metal ion levels normalize after revision of failed metal-on-metal total hip replacements?

INTRODUCTION: Increased cobalt (Co) and chromium (Cr) serum levels are one reason for revision surgery in metal on metal (MoM) total hip arthroplasty (THA) patients. Dual mobility liners are a simple revision option; however, they preserve the metal shell and stem and it is therefore not clear if metal ion levels will fully normalize after revision surgery. MATERIAL AND METHODS: Between April 2013 and December 2017 25 hips (24 patients) underwent revision from a MoM THA to an off-label dual mobility liner. Five patients were lost to follow-up and one patient refused leaving 18 patients (11 me

Potential presence of metals in patients treated with metal-metal coupling prostheses for hip arthroplasty at 7 and 10 years of follow-up.

BACKGROUND: Beginning in 2008, metal-on-metal prostheses have been in the spotlight owing to much higher revision rates than expected. Adverse local tissue reactions have been well described in the literature as potential complications. METHODS: Between 2012 and 2013, 13 patients with metal-on-metal total hip replacements were evaluated clinically and radiologically and with laboratory samples. The same tests were repeated between 2015 and 2016 on eight patients to assess any changes. In the laboratory assessment, we searched for chromium, cobalt, molybdenum, and nickel in blood and urine sam

MRI findings, metal ion levels and clinical outcome of a complete series of large metal on metal THA: what's really going on?

INTRODUCTION:: Metal-on-metal (MoM) large head total hip arthroplasties (THAs) were discontinued early after their introduction because of the high number of failures due to adverse reaction to metal debris (ARMD). Aim of this study is to report the clinical outcome at a mid-term follow-up (FU) of a series of large-head MoM THA. METHODS:: In this prospective study, 25 hips (24 patients, 3 males, 21 females, mean age 62.44 years) who have undergone primary THA with large head (diameter ⩾36 mm) MoM prosthesis were evaluated. Each patient underwent a standard follow-up after surgery, that includ

Adverse Local Tissue Reaction After a Metal-on-Metal Total Hip Prosthesis Without Elevated Serum Metal Ion Levels.

In the 1990s, metal-on-metal (MoM) bearings were reintroduced in total hip arthroplasty (THA), having the proposed advantages of decreased wear and improved stability. However, catastrophic adverse local tissue reaction (ALTR) secondary to metal debris from both the bearing surface and the head-neck junction is a well-recognized complication. The measurement of metal ion levels in the serum is often a first-line test to identify failure of a MoM bearing. The authors describe a 70-year-old man who presented with a multiply revised MoM THA and a large periprosthetic fluid collection incidentally

A coupled-cluster study on the noble gas binding ability of metal cyanides versus metal halides (metal = Cu, Ag, Au).

A coupled-cluster study is carried out to investigate the efficacy of metal(I) cyanide (MCN; M = Cu, Ag, Au) compounds to bind with noble gas (Ng) atoms. The M-Ng bond dissociation energy, enthalpy change, and Gibbs free energy change for the dissociation processes producing Ng and MCN are computed to assess the stability of NgMCN compounds. The Ng binding ability of MCN is then compared with the experimentally detected NgMX (X = F, Cl, Br) compounds. While CuCN and AgCN have larger Ng binding ability than those of MCl and MBr (M = Cu, Ag), AuCN shows larger efficacy toward bond formation with

Serum metal ion concentrations after unilateral vs bilateral large-head metal-on-metal primary total hip arthroplasty.

It is unknown if the presence of bilateral well-functioning large-head metal-on-metal (MOM) total hip arthroplasties (THAs) leads to higher serum metal ion concentrations than unilateral MOM THA. Elevated levels (chromium, 17 μg/L; cobalt, 19 μg/L) have been associated with poorly functioning MOM THA with metallosis. Fourteen patients having undergone bilateral and 25 patients having undergone unilateral large-head primary MOM THA were compared. Harris Hip Scores, University of California Los Angeles activity scores, radiographs, serum creatinine, and serum cobalt and chromium levels were obta

Metal-hydride bond activation and metal-metal interaction in dinuclear iron complexes with linking dinitriles: a synthetic, electrochemical, and theoretical study.

The dinuclear iron(II)-hydride complexes [[FeH(dppe)(2)](2)(mu-LL)][BF(4)](2) (LL = NCCH=CHCN (1a), NCC(6)H(4)CN (1b), NCCH(2)CH(2)CN (1c); dppe = Ph(2)PCH(2)CH(2)PPh(2)) and the corresponding mononuclear ones, trans-[FeH(LL)(dppe)(2)][BF(4)] (2a-c) were prepared by treatment of trans-[FeHCl(dppe)(2)], in tetrahydrofuran (thf) and in the presence of Tl[BF(4)], with the appropriate dinitrile (in molar deficiency or excess, respectively). Metal-metal interaction was detected by cyclic voltammetry for 1a, which, upon single-electron reversible oxidation, forms the mixed valent Fe(II)/Fe(III) 1a(+

Adsorption and catalytic reduction of hexavalent chromium based on nanomaterials: A review on metal, metallic oxide, metallic sulfide and carbon-based catalyst.

Chromium (Cr) is widely recognized as a significant environmental contaminant and a major contributor to global pollution. As a result, there is a strong emphasis on developing effective methods for the removal and reduction of Cr(VI). This review examines various applications of nanomaterial catalysts, including metallic oxides, metals, metallic sulfides, and carbon-based materials. These materials encompass naturally occurring substances, synthetically produced compounds, and artificially modified forms, all of which typically exhibit favorable adsorption properties and catalytic activity. W…

Microporous Metal-Phosphonates with a Novel Orthogonalized Linker and Complementary Guests: Insights for Trivalent Metal Complexes from Divalent Metal Complexes.

The reliable self-assembly of microporous metal-phosphonate materials remains a longstanding challenge. This stems from, generally, more coordination modes for the functional group allowing more dense structures, and stronger bonding driving less crystalline products. Here, a novel orthogonalized aryl-phosphonate linker, 1,3,5-tris(4'-phosphono-2',6'-dimethylphenyl) benzene (H6 L3) has been used to direct formation of open frameworks. The peripheral aryl rings of H6 L3 are orthogonalized relative to the central aromatic ring giving a tri-cleft conformation of the linker in which small aromatic

Synthesis and structure of metal-TCPE (metal = Th, Ce) metal-organic frameworks based on 1,2,4,5-tetrakis(4-carboxyphenyl) ethylene.

Two new metal-organic frameworks (MOFs) (Th/ Ce -TCPE) based on 1,2,4,5-tetrakis(4-carboxyphenyl)ethylene were obtained using a straightforward reaction under moderate conditions. Th and Ce formed the central units of this MOF in the mononuclear and in the unusual trinuclear cluster configurations, respectively. The resulting MOFs were analysed by fluorescence spectroscopy to understand their luminescence. The obtained data revealed that benzene's electron cloud density and torsion angle on the ligand were affected by the acetic acid molecule and Th(IV), which caused Th-TCPE to irradiate stron

Mid-to-long term follow-up of Transcend metal-on-metal versus Interseal metal-on-polyethylene bearings in total hip arthroplasty.

Second-generation metal-on-metal total hip arthroplasty (THA) was introduced in the early 1990s to address osteolysis and aseptic loosening resulting from polyethylene wear. We present a comparison between the Transcend metal-on-metal and Interseal metal-on-polyethylene THAs. Thirty-seven hips with Transcend metal bearings and 36 hips with Interseal polyethylene acetabular liners but identical acetabular shells were reviewed to determine clinical performance, radiographic changes, and survivorship. Patients with higher anticipated activity levels were selected to receive the Transcend bearing.

Understanding metal bonding.

This theoretical discussion covers several effects of metallic bonding based on a simple formula. It comes from the first steps in the Moment Method for calculating the local electronic structure of a solid (such as at a surface or in a random alloy), and depends on the square root of the total coordination numberCof near neighbours. Each atom is covalently bonded to its cluster of near neighboursas a whole. The properties of metals touched on include malleability, crystal structure and phase transitions, vacancy formation energy, surface catalysis, surface reconstruction, graphite stability, …

Transition Metal Homeostasis.

This chapter focuses on transition metals. All transition metal cations are toxic-those that are essential for Escherichia coli and belong to the first transition period of the periodic system of the element and also the "toxic-only" metals with higher atomic numbers. Common themes are visible in the metabolism of these ions. First, there is transport. High-rate but low-affinity uptake systems provide a variety of cations and anions to the cells. Control of the respective systems seems to be mainly through regulation of transport activity (flux control), with control of gene expression playing…

Metal allergy in Singapore.

This is a clinical epidemiologic study to determine the frequency of metal allergy among patch-tested patients in the years 2001-2003. The results are compared with those of previous studies. All patients diagnosed as having allergic contact dermatitis in the National Skin Centre, Singapore, from January 2001 to December 2003 were studied retrospectively. The frequency of positive patch tests to the following metals were nickel 19.9%, chromate 5.6%, cobalt 8.2% and gold 8.3%. The frequency of nickel allergy has been steadily rising over the last 20 years. The most common sources of nickel alle…

Bioremediation of metal contamination.

A study was initiated to evaluate the use of the fungus Aspergillus niger for bioleaching and then to determine the effect of process steps, the tailings concentration and type of substrate. An oxidized mining tailing containing mainly copper (7240 mg kg(-1) tailings) was studied. A sucrose and mineral salts medium was initially used to produce citric and gluconic acids by A. niger at various concentrations of tailings ( 1, 5, 7, 10 and 15% w/v). Maximal removal of up to 60% of the copper was obtained for the 5% tailings when the organic acid supernatant was added to the tailings. In a single

Interactions in metal carcinogenicity.

The carcinogenicity and genotoxicity of cadmium, chromium, cobalt and nickel strongly depend on their chemical ligands (speciation) which modulate their bioavailability and reactivity with biochemical targets. With the exception of hexavalent chromium, carcinogenic metal compounds are only weakly genotoxic. However, the ions of the carcinogenic metals cadmium, cobalt and nickel, and also the noncarcinogenic lead, inhibit the repair of DNA damaged by direct genotoxic agents like UV irradiation and alkylating substances, thereby enhancing the effects of the latter agents. These effects are inter…

Dissimilatory metal reduction.

Microorganisms can enzymatically reduce a variety of metals in metabolic processes that are not related to metal assimilation. Some microorganisms can conserve energy to support growth by coupling the oxidation of simple organic acids and alcohols, H2, or aromatic compounds to the reduction of Fe(III) or Mn(IV). This dissimilatory Fe(III) and Mn(IV) reduction influences the organic as well as the inorganic geochemistry of anaerobic aquatic sediments and ground water. Microorganisms that use U(VI) as a terminal electron acceptor play an important role in uranium geochemistry and may be a useful…

Metal inhibition of ferrochelatase.

Ferrochelatase activity was examined both in growing MEL cells and in in vitro assays of the purified enzyme to determine what effect a variety of divalent cations would have. Data obtained with the purified enzyme demonstrated that Mn2+ strongly inhibits the activity in a competitive fashion with respect to Fe2+ with a calculated Ki of 15 microM. Cadmium ion is also inhibitory (Ki: 50 microM), as is Hg2+ and arsenite, but Pb2+ is a poor inhibitor. All other metals tested had no effect. When these same metal ions were tested on differentiating MEL cells it was found that their ability to inhib…
