Morbidity and mortality encountered in the medical and surgical management of gastric dilatation-volvulus syndrome are most commonly associated with hypovolemic shock, ventricular arrhythmias, and sepsis related to gastric wall necrosis. Complications requiring spontaneous decisions are frequently encountered preoperatively during emergency stabilization, intraoperatively during exploratory laparotomy and prophylactic gastropexy, and postoperatively during the recovery period.

Increased mortality occurred among sows in a pig-breeding unit during a few months. It was concluded that gastric torsion was one of the causes of death. This disease is becoming increasingly important in large intensive units, also in the Netherlands. Little is so far known of the origin of this syndrome. The feeding system and the quality of the food (fine-grained feed, contamination by toxic agents) could be a factor in the pathogenesis. Closer investigation of this economic problem would appear to be advisable.

Gastric dilatation was diagnosed in 156 dogs during a 5-year period. The recurrence rate was 10% and the overall mortality was 43%. Large-breed dogs of either sex and ranging in age from 2 months to 15 years (mean age, 5.2 +/- 2.9 years) were found to be susceptible to the problem. Cardiac arrhythmias were diagnosed in 48 of 115 (42%) affected dogs. Cardiac arrhythmias occurred most frequently between 12 and 36 hours after the onset of gastric dilatation-volvulus and were generally ventricular in origin.

Displacement of the gastric remnant in patients with left diaphragmatic eventration may lead to gastric remnant volvulus requiring surgical correction. This results from the strong negative intrathoracic pulling force created by the potential subphrenic space an paradoxical movement of th diaphragm. Unless this strong negative force is first eliminated, gastropexy alone is likely to result in recurrence. Obliteration of the subphrenic space by colonic displacement is an effective and simple way of abolishing this negative subdiaphragmatic pulling force. Once this is achieved, any form of gastr

A patient with an unusual type of internal hernia was treated successfully. To our knowledge, this is the first reported case of a hernia emanating through the gastrohepatic ligament that resulted in gastric outlet obstruction. It is even more remarkable because, although the small bowel was the herniated viscus, the symptoms were due to obstruction of the stomach rather than obstruction of the small bowel.

PURPOSE: We explored factors affecting gastric emptying in neurologically impaired (NI) patients using the 13C-acetate breath test.
METHODS: Twenty-four NI patients were classified by the presence of gastroesophageal reflux disease (GERD), which was treated by fundoplication plus gastrostomy, or the absence of GERD, which was treated by gastrostomy alone, along with gastric malposition involving cascade stomach and organoaxial gastric volvulus (OGV). Gastric emptying parameters (GEPs), which were the emptying half time (T 1/2 , minute), the lag phase time (T lag , minute), and the gastric emp

Gastric dilatation-volvulus (at 30 mm of Hg of gastric pressure) was experimentally induced in 8 dogs under sodium pentobarbital anesthesia. Hemodynamic indices including cardiac output, mean aortic pressure, left ventricular end diastolic pressure, heart rate, total peripheral resistance, and dp/dtmax were measured during a 180-minute period of gastric dilatation-volvulus and for 120 minutes after gastric decompression. Experimental gastric dilatation-volvulus resulted in significant (P less than 0.05) decreases in cardiac output (64%), mean aortic pressure (48%), left ventricular end diastol

The objective of this study was to describe the operative technique and outcome of a simplified laparoscopic gastropexy approach in dogs. Twenty-one dogs undergoing prophylactic laparoscopic gastropexy with a simple continuous barbed suture without incising the seromuscular layer of the stomach and transversus abdominis muscle were reviewed. In 20 cases, additional procedures were performed (18 ovariectomies and 2 prescrotal castrations); 1 dog had two prior episodes of gastric dilation without volvulus and underwent gastropexy with a prophylactic intent. The gastropexy procedure had a median

Acute gastric volvulus presents with the classic triad of severe epigastric pain, vomiting, and difficulty or inability to pass a nasogastric tube. It can be complicated by strangulation and perforation which are associated with high mortality rates. Quick diagnosis and attempt at decompression is paramount. Computer-aided tomography is an important diagnostic step, and findings such as gastric wall thickening, extraluminal gas, and mediastinal fluid should prompt urgent surgical intervention. Approaches can include minimally invasive or open operations and in general should include reduction

OBJECTIVE: To compare the acute strength (failure load and work to failure) of standard incisional gastropexy (SIG) and modified incisional gastropexy (MIG).
ANIMALS: 37 pig cadavers.
PROCEDURES: Stomachs and right abdominal walls were harvested from pigs euthanized for reasons unrelated to this study. The tissues were stored in lactated Ringer's solution overnight in a 5 °C cooler. Matching body wall and stomach tissue pairs were randomized and divided into 2 groups, on which either SIG or MIG was performed the following day. The MIG technique was identical to SIG except 2 additional simple

AIM: The aim of this study was to present an overview of the gastric dilatation-volvulus syndrome in guinea pigs.
MATERIAL AND METHODS: Four cases of gastric dilatation-volvulus syndrome in guinea pigs were evaluated. Its clinical presentation, diagnostic options, therapeutical approach and possible alternatives thereof were discussed. Furthermore, risk factors for the disease as well as pathophysiology and etiology were discussed and compared to the situation in other species.
RESULTS: The presented cases indicate that in addition to anamnesis and clinical examination, radiography is the me

BACKGROUND: Tension gastrothorax develops when the stomach, herniated through a congenital diaphragmatic defect into the thorax, is massively distended by trapped air. We report a case of tension gastrothorax and review the literature.
CASE REPORT: A previously healthy 8-month-old female presented with severe respiratory distress, misdiagnosed as tension pneumothorax. Intercostal tube was inserted. The tube was noted to drain food as well as air. The patient was investigated by radio-contrast swallow, which demonstrated the presence of the stomach in the chest. The patient was operated upon,

Gastric volvulus is an extremely rare late complication of pneumonectomy. We report the case of a 61-year-old man who presented with a 1-year history of progressive intolerance of solids and weight loss 33 years after a left pneumonectomy. Preoperative examinations showed a mesenteroaxial gastric volvulus. We performed a laparoscopic Toupet fundoplication and anterior gastropexy, following which his symptoms disappeared.

Gastric volvulus is a potentially lethal condition. Pneumonectomy patients have decreased physiologic reserve, and thus they are more susceptible to morbidity and mortality from postoperative complications. We report successful management of a patient with hiatal hernia that resulted in acute gastric volvulus after left pneumonectomy.
