Validity of transit time-based blood pressure measurements in patients with and without heart failure or pulmonary arterial hypertension across different breathing maneuvers.

PURPOSE: Pulse transit time (PTT) derived by ECG and plethysmographic signal can be a promising alternative to invasive or oscillometry-based blood pressure (BP) monitoring in sleep laboratories because it does not cause arousals from sleep. Therefore, this study assessed the validity of PTT for BP monitoring under sleep laboratory-like conditions. METHODS: Ten volunteers (55.8 ± 19.6 years), 12 patients with heart failure with reduced ejection fraction (HFrEF; 67.3 ± 8.6 years), and 14 patients with Nizza class I pulmonary arterial hypertension (PAH; 59.5 ± 13.4 years) performed different br

The effects of airway closure in central apneas and obstructed respiratory efforts in mixed apneas in preterm infants.

BACKGROUND: Airway closure during central apnea could allow for better gas exchange. However, the return of inspiratory efforts against a closed airway may cause greater desaturation and prolongation of apnea. OBJECTIVE: To investigate the effect of patency of the airway and obstructed respiratory efforts on the length of apneas and the degree and slope of O(2) desaturation and bradycardia. METHODS: All mixed apneas observed in 15 preterms were matched 1:2 with the longest central apneas in each infant. Central apneas were further classified using the amplified cardiac airflow oscillation in

Beneficial effect of epidural anesthesia on oxygen consumption in a parturient with adult respiratory distress syndrome.

Multifactorial increases in oxygen consumption during labor may exceed available oxygen supply. Cumulative subclinical oxygen debt could be clinically detrimental to both patient and fetus. We have reported the use of continuous mixed venous oxygen saturation monitoring to identify changes in oxygen consumption after painful uterine contractions in a critically ill parturient. We used continuous venous saturation monitoring to document the effects of epidural analgesia on oxygen balance in our patient. The absence of venous desaturation with contractions after abatement of labor pains confirme

Oxygen uptake during weaning from mechanical ventilation.

Total body oxygen uptake (VO2) increases during the transition from machine-assisted ventilation to spontaneous breathing. Since the volume of oxygen consumed by the respiratory muscles must contribute to the increase in VO2 (delta VO2), we explored whether delta VO2 and/or measurements of respiratory power output (Wresp) provide clinically useful information in the evaluation of disease state and weaning decisions in patients with respiratory failure. We determined the metabolic, ventilatory, and hemodynamic responses of ten patients during weaning from controlled mechanical ventilation, and

Disorders of the chest wall and diaphragm.

The clinical presentations of chest wall and diaphragmatic disorders are extremely diverse, and the attention of the emergency physician is often directed to a more obvious problem. The common denominator of these disorders is their effect on respiratory mechanics, a discussion of which precedes the review of specific disorders.

Mechanism of CO(2) retention in patients with neuromuscular disease.

BACKGROUND: In many studies of patients with muscle weakness, chronic hypercapnia has appeared to be out of proportion to the severity of muscle disease, indicating that factors other than muscle weakness are involved in CO(2) retention. In patients with COPD, the unbalanced inspiratory muscle loading-to-strength ratio is thought to trigger the signal for the integrated response that leads to rapid and shallow breathing and eventually to chronic hypercapnia. This mechanism, although postulated, has not yet been assessed in patients with muscular dystrophy. SUBJECTS: Twenty consecutive patient

Particle Swarm Optimization-Based Extreme Learning Machine for COVID-19 Detection.

COVID-19 (coronavirus disease 2019) is an ongoing global pandemic caused by severe acute respiratory syndrome coronavirus 2. Recently, it has been demonstrated that the voice data of the respiratory system (i.e., speech, sneezing, coughing, and breathing) can be processed via machine learning (ML) algorithms to detect respiratory system diseases, including COVID-19. Consequently, many researchers have applied various ML algorithms to detect COVID-19 by using voice data from the respiratory system. However, most of the recent COVID-19 detection systems have worked on a limited dataset. In other

Comparison of Respiratory Support After Delivery in Infants Born Before 28 Weeks' Gestational Age: The CORSAD Randomized Clinical Trial.

IMPORTANCE: Establishing stable breathing is a key event for preterm infants after birth. Delivery of pressure-stable continuous positive airway pressure and avoiding face mask use could be of importance in the delivery room. OBJECTIVE: To determine whether using a new respiratory support system with low imposed work of breathing and short binasal prongs decreases delivery room intubations or death compared with a standard T-piece system with a face mask. DESIGN, SETTING, AND PARTICIPANTS: In this unblinded randomized clinical trial, mothers threatening preterm delivery before week 28 of ges…

Inspiratory muscle training reduces diaphragm activation and dyspnea during exercise in COPD.

Among patients with chronic obstructive pulmonary disease (COPD), those with the lowest maximal inspiratory pressures experience greater breathing discomfort (dyspnea) during exercise. In such individuals, inspiratory muscle training (IMT) may be associated with improvement of dyspnea, but the mechanisms for this are poorly understood. Therefore, we aimed to identify physiological mechanisms of improvement in dyspnea and exercise endurance following inspiratory muscle training (IMT) in patients with COPD and low maximal inspiratory pressure (Pimax). The effects of 8 wk of controlled IMT on res

Chronic cyanide exposure--A biochemical and industrial hygiene study.

A study was undertaken to assess the health status of workers exposed to cyanide fumes and cyanide aerosols in one of the local factories. Cyanide levels in the breathing zone and the working atmosphere were measured. Cyanide and thiocyanate concentrations in blood and urine were determined colorimetrically. The cyanide and thiocyanate concentrations in blood and urine of exposed workers were much higher than those of control workers. Smokers had higher concentrations than nonsmokers. The highest values of cyanide were found to be 0.8 and 0.2 mg/m3 in the breathing zone and general workroom at

[Comprehensive diagnostics of the patients presenting with Eustachian tube dysfunction].

The objective of the present work was comprehensive diagnostics of Eustachian tube dysfunction as exemplified by 82 patients with this disorder. The examination using the standard techniques was supplemented by active anterior rhinomanometry, audiometry, and tympanometry. Three-dimensional computed tomography (3D X-ray CT) was used for detailed diagnostics of the state of the nasal structures and paranasal sinuses. The study has demonstrated that 96.3% of the patients with Eustachian tube dysfunction suffer a concomitant pathology of the paranasal sinuses, nasal cavity, and nasopharynx. Diffic

Chest Wall Diseases: Respiratory Pathophysiology.

The chest wall consists of various structures that function in an integrated fashion to ventilate the lungs. Disorders affecting the bony structures or soft tissues of the chest wall may impose elastic loads by stiffening the chest wall and decreasing respiratory system compliance. These alterations increase the work of breathing and lead to hypoventilation and hypercapnia. Respiratory failure may occur acutely or after a variable period of time. This review focuses on the pathophysiology of respiratory function in specific diseases and disorders of the chest wall, and highlights pathogenic me…

Rheumatoid Arthritis Complicated by Pseudochylothorax Without Pleural Thickening: A Diagnostic Challenge for Clinicians.

A 60-year-old female who was known to have rheumatoid arthritis for the preceding two-and-half years presented with difficulty in breathing associated with chest pain over the right hemithorax of two months duration. She was found to have a right-sided mild to moderate pleural effusion; there was no evidence of pleural thickening. The pleural fluid was pale yellow in appearance and diagnostic work-up confirmed it to be a pseudochylous pleural effusion. The present case highlights the rare occurrence of pseudochylothorax without pleural thickening as a complication of rheumatoid arthritis.

Characterizing the motion management system accuracy on a 1.5T MR-Linac.

INTRODUCTION: Motion management techniques have enabled drastic reductions in treatment volume allowing for dose escalation. The superior contrast of MRI combined with the ability to perform cine imaging during treatment enable surrogate-less tracking for gating. The purpose of this work is to commission motion management system with gating capabilities on the 1.5T MR-Linac and to characterize system performance. METHODS: Linear accelerator stability during gating was measured for a wide range of duty cycles (16 % - 100 %). Motion tracking accuracy and gating latency were characterized for va

Management of Mechanical Ventilation in Decompensated Heart Failure.

Mechanical ventilation (MV) is a life-saving intervention for respiratory failure, including decompensated congestive heart failure. MV can reduce ventricular preload and afterload, decrease extra-vascular lung water, and decrease the work of breathing in heart failure. The advantages of positive pressure ventilation must be balanced with potential harm from MV: volutrauma, hyperoxia-induced injury, and difficulty assessing readiness for liberation. In this review, we will focus on cardiac, pulmonary, and broader effects of MV on patients with decompensated HF, focusing on practical considerat…

Acute symptoms in coffee workers.

Processing of green coffee before export, as carried out in Sri Lanka, is a very dusty process. Thirty-eight workers, who were exposed to coffee dust intermittently, were studied. They developed acute symptoms referrable to the eyes, nostrils and respiratory tract. Cough (84.2%), sputum (76.3%), sneezing (73.7%), difficulty in breathing (63.2%) and running nose (55.3%) were the commonest symptoms. Of the workers 10.5% had a wheeze. These symptoms lasted only during the hours that workers were exposed to the dust, and subsided on returning home after the day's work.
