Tricuspid Septal Leaflet Detachment for Ventricular Septal Defect Repair in Adults.

We report a 21-year-old patient with a ventricular septal defect (VSD) with an embolic vegetation on the tricuspid valve using a technique of septal leaflet detachment to facilitate the repair of the VSD. Although described in the pediatric patients, this technique has not been reported in adults. The technique of septal leaflet mobilization is described, along with other techniques of tricuspid leaflet mobilization that aid in VSD visualization and repair. Tricuspid leaflet detachment has application for adult surgeons repairing perimembranous, postinfarction, and iatrogenic VSD encountered i

Tetralogy of Fallot with infective endocarditis: an echocardiographic explanation of misleading clinical signs.

Echocardiography has a known key role in the diagnosis of infective endocarditis, the diagnosis of complications, follow-up evaluation after therapy, and prognostic assessment Habib (Eur J Echocardiogr 11:202-219, 3). This report describes a boy with tetralogy of Fallot who presented with infective endocarditis and large vegetation occluding the ventricular septal defect, thus resulting in a hemodynamically restrictive ventriculoseptal defect with misleading clinical signs. This case illustrates the role of echocardiography in both explaining clinical signs and providing hemodynamic data.

A Giant Trichobezoar Causing Rapunzel Syndrome in a 12-year-old Female.

Bezoar is a tightly packed collection of undigested material that is unable to exit the stomach. Most bezoars are of indigestible organic matter such as hair-trichobezoars; or vegetable and fruit-phytobezoars; or a combination of both. Trichobezoars commonly occur in patients with psychiatric disturbances who chew and swallow their own hair. In very rare cases, the Rapunzel syndrome hair extends through the pylorus into the small bowel causing symptom and sign of partial or complete gastric outlet obstruction. A case report of trichobezoar in the stomach causing Rapunzel syndrome in a 12-year-

Splenic amyloidosis: a rare cause of spontaneous splenic rupture.

A 62 year-old woman who presented with an atraumatic acute abdomen was discovered to have haemoperitoneum with splenic rupture on urgent computed tomography and was immediately referred for life-saving emergency splenectomy. Histopathological examination revealed secondary splenic amyloidosis. The patient was later found to be suffering from infective endocarditis secondary to her permanent cardiac pacemaker. This report describes a patient who could have suffered from a long-standing infected vegetation on a permanent cardiac pacemaker, which led to splenic amyloidosis and spontaneous splenic

[Intramural blood flow and neurogenic regulation changes of the bladder wall of rats with infravesical obstruction based on harmonic analysis of bioimpedans].

During high pressure in the bladder, particularly during delay of urination because of infravesical obstruction, raisings of blood pressure were marked with simultaneous increase of renal and spleen sympathetic nerves activity. During urination the activity of sympathetic nervous system decreases while the activity of parasympathetic system which is responsible for detrusor contraction increase. All these data testify to changes of vegetative regulation of hypertrophic bladder during inftavesical obstruction, but on the other hand mechanism of those changes and their functional consequences re

Late bacterial endocarditis of an amplatzer atrial septal defect occluder device.

A 66-year-old man with a large secundum atrial septal defect (ASD) that had been repaired percutaneously 30 months previously with an Amplatzer ASD occluder (ASO) presented with fevers and Staphylococcus aureus bacteremia. Transesophageal echocardiography revealed a 1.5 x 1.5 cm mobile mass on the left atrial side of his ASO consistent with a vegetation. When the ASO was explanted, the left atrial side of the device was poorly endothelialized. In conclusion, the present report is the first description of late infective endocarditis in an adult with an ASO.

Lipoid pneumonia: spectrum of clinical and radiologic manifestations.

OBJECTIVE: Lipoid pneumonia results from accumulation of lipids in the alveoli and can be either exogenous or endogenous in cause based on the source of the lipid. Exogenous lipoid pneumonia is caused by inhalation or aspiration of animal fat or vegetable or mineral oil. Endogenous lipoid pneumonia is usually associated with bronchial obstruction. The purpose of this article is to review the pathogenesis and clinical and radiologic manifestations of exogenous and endogenous lipoid pneumonia. CONCLUSION: The ability to recognize the radiologic manifestations of lipoid pneumonia is important be

[Acute myocardial infarction with left ventricular free wall rupture and papillary muscle rupture caused by infectious endocarditis].

Infectious endocarditis sometimes causes coronary embolism which induces acute myocardial infarction. A 59-year-old man was admitted to our hospital with a diagnosis of acute myocardial infarction accompanied by left ventricular free wall rupture and papillary muscle rupture. We perfomed mitral valve replacement combined with repair of left ventricular free wall rupture. The anterior mitral leaflet had perforation and vegetation, which suggested that acute myocardial infarction was caused by septic embolus originated from infectious endocarditis in this case.

[Clinicofunctional characteristics of primary mitral prolapse in adolescents].

To investigate clinicofunctional features of primary mitral prolapse (PMP) in adolescents, 60 patients were examined (42 adolescent girls and 18 adolescent boys aged 14 to 20 years). Control group consisted of 15 healthy adolescents of the same age. PMP in adolescents was characterized by marked manifestations of connective tissue dysplasia (CTD) and vegetative dysfunction (VD). As shown by echocardiography, 71.7% patients had bilateral prolapse with mitral regurgitation of degree I and II. 18.7% examinees with pronounced CTD and VD had asymptomatic variant of PMP.

Acidic deposition: decline in mobilization of toxic aluminium.

The mobilization of aluminium from acidic forest soils is arguably the most ecologically important consequence of acid deposition in the environment because of its adverse effects on soils, forest vegetation and surface water. Here we show that there has been a significant decline in the concentrations of aluminium species in soil solutions at medium-to-high elevations in a northern hardwood forest in the United States in response to decreasing acidic deposition. Streamwater aluminium concentrations have also fallen and, if this rate of recovery persists, will within 10 years no longer pose a

[Pierre-Marie-Bamberger syndrome - a paraneoplastic syndrome of lung cancer - a case report].

The Pierre-Marie-Bamberger syndrome is a rare paraneoplastic syndrome caused by bronchial carcinoma. Typical signs are symmetric periostoses on the diaphyses of the long tubular bones, clubbed fingers and toes with eye-glass shape of the nails, neuro-vegetative disturbances and dysproteinemia. We report a 37-year-old patient with long-term nicotine abusus, who attracted attention by symptoms of a Pierre-Marie-Bamberger syndrome. Further diagnostics revealed a tumor in the apex of the left lung. After lobectomy of the upper lobe of the left lung the symptoms are completly disappeared.

Massive septic thrombus formation on a superior vena cava indwelling catheter following Torulopsis (Candida) glabrata fungemia.

Fungal endocarditis is an exceedingly rare complication of indwelling central venous catheters in adults. Here we describe what appears to be the first case of a right atrial thrombus superinfected with the yeast Torulopsis (Candida) glabrata and attached to an indwelling superior vena cava catheter that was not used for parenteral nutrition. A large vegetation-like mass adherent to the catheter tip was visualized by transesophageal echocardiography in a patient who presented with signs of septic pulmonary embolism. Following open-heart surgery, the definitive diagnosis was established by hist

[Myocardial infarction caused by septic embolism during mitral endocarditis].

Embolic infarction is a rare complication of mitral valve endocarditis. The authors present the case of a patient with a linear vegetation of the anterior mitral leaflet measuring over 15 mm and entering the aortic valve orifice at each systole. This valvular infarction was the cause of an extensive anterior wall infarct treated by primary angioplasty because of haemodynamic instability. The outcome at 3 months was very favourable with regards to the mitral valve. However, a large mycotic coronary aneurysm developed at the site of angioplasty and there were sequellae of a large anterior wall i

Culture-positive Bartonella quintana endocarditis.

A 50-year-old alcoholic man with a 1-year history of pyrexia of unknown origin was admitted with symptoms of endocarditis. Large vegetations on his mitral and aortic valve were found on echocardiography. Using the Bactec 9240 system. Bartonella quintana could be grown from two culture sets of blood collected before the start of antimicrobial therapy. This is the first case report of culture-proven bartonella infection in the UK. and is noteworthy because Bartonella quintana was detected in conventional blood culture systems after prolonged incubation.

Sudden death in neonate with staphylococcal endocarditis.

A 27-week-old girl, 936 gm, with initial diagnosis of birth asphyxia resulting from prolapse of the umbilical cord, respiratory distress syndrome, and suspected neonatal sepsis received antibiotics for the first 7 days of life. On day 24, evidence developed of acute Staphylococcus aureus endocarditis of the anterior leaflet of the mitral valve, and she died 4 days later. We believe that a rapid enlargement of the endocardial vegetation caused acute occlusion of the small mitral valve and was the cause of sudden death in this infant.

[Detection of the autonomic dystonia syndrome during periodic medical examinations of industrial workers].

Prevalence of the vegetative dystonia syndrome (VDS) among the workers at a chemical plant increased depending on the duration of work, industrial noise being the major factor in the hazardous working conditions. The VDS risk groups were traced in the age-group beyond 40 with the duration of work over 10 years. The following techniques can be adequately used in the VDS detection: baresthesiometry , cardiointervalometry at rest and cardiointervalographic assessment of the Ashmer reflex, which facilitate determination of the specific nature and manifestations of the pathologic developments, as w

[Successful mitral valve replacement for infective endocarditis in pregnancy].

A 27 weeks' pregnant women exhibited infective endocarditis due to alpha-streptococcus. As echocardiography showed vegetations on both mitral leaflets, cesarean section was performed at 35 weeks' gestation and a healthy male infant weighing 2,430 g was delivered. Antibiotic therapy was continued for fever after the cesarean section but macrohematuria and Osler's nodes developed. Emergency mitral valve replacement was performed successfully despite the presence of active infective endocarditis. She was discharged after completion of a 10-week course of antibiotic therapy.

[Comparative evaluation of the status of neuromuscular transmission and evoked skin sympathetic potential in myasthenia syndrome of the Eaton-Lambert type in patients with and without bronchogenic carcinoma].

Neuromuscular transmission and evoked skin sympathetic potential (ESSP) were examined in 14 patients with Lambert-Eaton syndrome. Bronchogenous lung carcinoma was revealed in four of those patients. No correlation was found between the impairment of neuromuscular transmission and the parameters of ESSP. All the four patients suffering from Lambert-Eaton syndrome combined with bronchogenous lung carcinoma did not manifest ESSP. The reversibility of ESSP disorders established after amiridine and guanidine administration attest to the mediatory character of vegetative regulation impairment in pat
