Less than full circumferential fusion of a tibial nonunion is sufficient to achieve mechanically valid fusion--proof of concept using a finite element modeling approach.

BACKGROUND: Although minimally invasive approaches are widely used in many areas of orthopedic surgery nonunion therapy remains a domain of open surgery. Some attempts have been made to introduce minimally invasive procedures into nonunion therapy. However, these proof of concept studies showed fusion rates comparable to open approaches never gaining wider acceptance in the clinical community. We hypothesize that knowledge of mechanically relevant regions of a nonunion might reduce the complexity of percutaneous procedures, especially in complex fracture patterns, and further reduce the amount

A biomechanical evaluation of a spacer with integrated plate for treating adjacent-level disease in the subaxial cervical spine.

BACKGROUND CONTEXT: Adjacent level degeneration (ALD) has been reported as one of the long-term consequences of anterior discectomy and fusion despite its clinical success in treating cervical pathologies. Traditionally, ALD is treated by replacing the previously implanted plate with a longer plate, which can lead to postoperative complications. The biomechanics of SIP in the adjacent level has not been investigated. PURPOSE: To evaluate the multidirectional stability of a spacer with integrated plate (SIP) in comparison to a traditional spacer and plate (TSP). STUDY DESIGN: To evaluate the

The effect of distal radius locking plates on articular contact pressures.

PURPOSE: Fractures of the distal radius are among the most common injuries treated in hand surgery practice, and distal radius locking plates have become an increasingly popular method of fixation. Despite widespread use of this technology, it is unknown whether the subchondral placement of locking screws affects the loading profile of the distal radius. Our study was designed to determine whether subchondral locking screws change the articular contact pressures in the distal radius. METHODS: Twelve cadaveric forearms underwent a previously described axial loading protocol in a materials test

Anterolateral and medial locking plate stiffness in distal tibial fracture model.

BACKGROUND: The purpose of this study was to compare the axial and torsional stiffness between anterolateral and medial distal tibial locking plates in a pilon fracture model. MATERIALS AND METHODS: The biomechanical stiffness of anterolateral or medial plated pilon fracture models was evaluated. Six Sawbones Composite Tibiae with a simulated pilon fracture representing varus or valgus comminution (OTA 43-A2.2) were plated with a Synthes 3.5-mm contoured LCP anterolateral or medial locking distal tibia plate. Load as a function of axial displacement and torque as a function of angular displac

Percutaneous plating of periarticular tibial fractures: a reliable, reproducible technique for controlling plate passage and positioning.

This article describes percutaneous plating of periarticular tibial fractures, in which a suture, tied to the proximal pole of a precontoured plate, is used to guide the plate through a submuscular tunnel. The technique enables the surgeon to manipulate both poles of the plate during its passage and to accurately position the plate. Percutaneous techniques continue to evolve, providing the advantage of fracture fixation while preserving the integrity of the soft tissue envelope. Various clinicians have described impressive results in percutaneous fixation of fractures of the distal tibia. Thes

Flexor tendon injuries following locked volar plating of distal radius fractures.

We present 2 cases showing that flexor pollicis longus and flexor digitorum profundus index injury can occur after placement of 2 commonly used locked volar plates. In contrast with the literature, the radii healed in an anatomic position without plate lift-off. The patients presented 6 and 8 months after surgery with new onset of radial wrist pain and tenderness at the site of the plate and absence or weakness of the flexor pollicis longus. In both cases, the plate was positioned anterior to the distal radial rim on the lateral radiograph. We suggest close follow-up of all fractures in which

Plate osteosynthesis of the mandibular condyle.

The aim of this study is to evaluate and compare the biomechanical stability of various osteosynthesis materials for mandible condylar-process fractures. On 160 porcine mandibles, four different monocortical plating techniques (40 per group) were investigated. Condyles were fractured at a defined location from the incisure to the posterior border. After correct anatomical reduction the fractures were plated, using four different techniques. Osteosynthesis materials used were the delta plate, the trapezoid plate, the dynamic compression plate and double mini-plates. Each group was subjected to

Periprosthetic femur fractures--a growing epidemic.

The number of periprosthetic femoral fractures is increasing, and this trend will continue. The factors contributing to this epidemic include length of time since implantation of the prosthesis; loose stems; aging of the population, leading to osteoporosis and a tendency to fall; and the large number of hip arthroplasties performed each year. These complex fractures necessitate innovative treatments and consideration of the biomechanical impact of the treatments on the entire femur. We present a small series of cases in which periprosthetic fractures were treated with a variety of techniques,

Single or double plating for nonunion of the clavicle.

Between January 1982 and January 1999, 684 patients presented with a fracture of the clavicle to the accident and emergency departments of the Tamside and Bury District General Hospitals. Twenty patients (3%) subsequently developed symptomatic nonunion of the clavicle. The original injury resulted from a road traffic accident in 13 patients, from a fall on an outstretched hand in five patients, and two patients had sports related injuries. Mean age of the patients was 39 years (range, 17 to 76 years). Mean time from injury to surgery was 2.5 years (range, 6 months to 8 years). Fifteen patients

Extensor tendon rupture after internal fixation of a distal radius fracture using a dorsally placed AO/ASIF titanium pi plate. Arbeitsgemeinschaft für Osteosynthesefragen/Association for the Study of Internal Fixation.

Extensor tenosynovitis has been reported as a complication of the Arbeitsgemeinschaft für Osteosynthesefragen/Association for the Study of Internal Fixation (AO/ASIF) pi plate. The authors present a case of extensor tendon rupture after internal fixation with a pi plate after one of the screws in the distal limb loosened. This mechanism of extensor tendon rupture has not been reported previously. With appropriate prevention, monitoring, and intervention this complication could have been avoided. Schnur DP, Chang B. Transverse surgical correction of the maxilla. A modified procedure.

A modified surgical procedure to expand as well as reduce the width of the hard palate is described. Instead of a single sagittal osteotomy in the midline of the palate, multiple sagittal osteotomies are performed parallel to each other and maintaining the periosteal attachment to each of the fragments. The hard palate is thereby moveable and can be reduced in a transverse direction as well as expanded. The expansion will, however, be limited to the height of the curvature of the palate. Minor complications with the method are described. Long-term postoperative follow-up shows stable results w

Ankle fusion: a new technique of internal fixation using a compression blade plate.

A new technique of tibiotalar arthrodesis has been developed offering wide exposure, excellent correction of deformity, good bony apposition, and blade plate fixation. This technique has even been effective in cases of ischemic necrosis of the talus. A modified pediatric blade plate is used to effect compression of the talus to the tibia and to provide stability against flexion/extension and varus/valgus movement. Fixation is augmented by a lateral fibular struct screwed to the tibia and talus. In this series, 17 compression blade plate tibiotalar arthrodeses were performed. Follow-up averaged

[Acute management of spinal injuries. The thoracic and lumbar spine, surgical therapy].

The aims of internal fixation of thoracic and lumbar spinal fractures are immediate stability, preservation of spine mobility, and restoration of the anatomical form of the vertebra and of the canal diameter at acceptable risks. All these aims together can be achieved only by short posterior pedicle fixation systems, such as the external spine fixator, the various internal spine fixators, special dorsal Roy-Camille plates, Wolter plates, and the combined anterior and posterior unisegmental fusion with the USIS system. Isolated anterior procedures necessitate additional bracing. The indications

Hand injuries--special considerations in children.

Injuries to the hand are very common in children. Because of children's tremendous capability of repair, fingertip injuries can usually be treated with dressings, and complicated closures with flaps or grafts can be avoided. Fractures are common, and the treating physician must be aware of the various ephiphyseal injuries that can occur in the hand. Because children are so active, if the hand must be immobilized, a long arm cast or valpeau dressing will usually be required.
