Nutritional Supplementation for Obsessive-Compulsive Disorder: Protocol for a Systematic Review of Randomized Controlled Trials.

BACKGROUND: Obsessive-compulsive disorder (OCD) is a chronic mental health condition, and standard treatment often results in incomplete symptom relief. Growing evidence suggests that nutritional supplements, including vitamins, minerals, and amino acids, may serve as potential adjunctive treatments for OCD by influencing neurotransmitter regulation and inflammation. OBJECTIVE: Our systematic review aims to assess the comparative efficacy of nutritional supplements on relevant clinical outcomes in individuals with OCD. METHODS: A protocol was developed and registered for this systematic revi

Direct oral anticoagulants for stroke prevention in patients with atrial fibrillation: A network meta-analysis of randomized trials.

BACKGROUND: Direct oral anticoagulants (DOACs) are increasingly preferred over vitamin K antagonists (VKAs) for stroke prevention in atrial fibrillation (AF), including valvular (VAF) and non-valvular (NVAF). This network meta-analysis aimed to evaluate and compare the efficacy and safety of different DOACs versus VKAs in patients with AF. METHODS: A Bayesian network meta-analysis was conducted to estimate odds ratios (ORs) with 95 % credible intervals (CrIs). RESULTS: 43 RCTs were included (30 VAF, 13 NVAF). Dabigatran (OR 0.77, 95 % CrI 0.68-0.87) demonstrated the strongest reduction in is

Protocol for a Randomized, Open-Label, Multicenter, Parallel-Controlled Study Evaluating Modified Double Filtration Plasmapheresis for the Treatment of Septic Shock.

INTRODUCTION: Septic shock (SS) is defined as sepsis-induced hypotension persisting despite adequate fluid resuscitation. Modified double filtration plasmapheresis (M-DFPP) is a promising therapeutic approach designed to selectively remove small-to-medium molecular weight inflammatory mediators while preserving essential plasma proteins, potentially restoring immune balance and stabilizing hemodynamics in patients with SS. METHODS: Eligible patients diagnosed with SS will be enrolled in this randomized controlled trial. Inclusion criteria include completion of early goal-directed therapy resu

Evaluation of Surgical Approaches to Condylar Fractures: A Systematic Review and Network Meta-Analysis.

BACKGROUND: Condylar fractures are common maxillofacial injuries. Although both open and closed treatments are used, the optimal strategy remains debated due to variable functional outcomes and complication rates. PURPOSE: This network meta-analysis compared the functional outcomes and complications of closed treatment versus various open approaches for mandibular condyle fractures. We evaluated and ranked 6 distinct open approaches: mandibular angle approach (MAA), retromandibular anteroparotid approach (RMAA), endoscopic transoral approach (ETOA), retromandibular through parotid approach (R

Neural network-based multi-task learning to assist planning of posterior spinal fusion surgery for adolescent idiopathic scoliosis.

PURPOSE: Posterior spinal instrumentation and fusion (PSF) is the gold standard for severe adolescent idiopathic scoliosis (AIS), yet instrumentation strategies vary widely, often leading to suboptimal results. Deep learning's potential in AIS planning is underexplored. METHODS: This study trained and validated an artificial neural network multi-task learning model (NNML) using preoperative clinical and radiographic data from 189 AIS patients with Lenke 1A and 2A curves enrolled in the MIMO Clinical Trial (NCT01792609). The model mimics experienced spine surgeons' decision-making for selectin

Effect of TNF alpha inhibitors on atherosclerosis in psoriatic arthritis patients: A Systematic Review Protocol.

INTRODUCTION: Psoriatic arthritis (PsA) is a chronic inflammatory condition associated with joint and skin involvement. Cardiovascular disease, particularly atherosclerosis, is a leading cause of morbidity and premature mortality in PsA patients, due to persistent systemic inflammation. Tumor necrosis factor-alpha (TNF-α) inhibitors have proven effective in reducing inflammation in PsA, and some evidence suggests they may also improve vascular health. However, their specific impact on atherosclerosis in PsA remains unclear. OBJECTIVE: This systematic review aims to evaluate the effect of TNF-

The lowest well tolerated blood pressure: A personalized target for all?

The optimal blood pressure (BP) target for prevention of cardiovascular complications of hypertension remains uncertain. Most Guidelines suggest different targets depending on age, comorbidities and treatment tolerability, but the underlying evidence is not strong. Results of randomized strategy trials comparing lower (i.e., more intensive) versus higher (i.e., less intensive) BP targets should drive the definition. However, these trials tested different BP targets based on systolic BP, diastolic BP or combined systolic and diastolic BP goals. Overall, the more intensive treatment targets redu

Multicentre, open-label, randomised, controlled trial to compare early intervention with calcimimetics and conventional therapy in preventing coronary artery calcification in patients with secondary hyperparathyroidism (UPCOMING): a study protocol.

INTRODUCTION: Coronary artery and heart valve calcification is a risk factor for cardiovascular death in haemodialysis patients, so calcification prevention should be started as early as possible. Treatment with concomitant calcimimetics and low-dose vitamin D receptor activators (VDRAs) is available, but not enough evidence has been obtained on the efficacy of this regimen, particularly in patients with short dialysis duration. Therefore, this study will evaluate the efficacy and safety of early intervention with upacicalcet, a calcimimetic used to prevent coronary artery calcification in thi

Local Adverse Events Associated with Peripheral Vasoactive Infusion in Children: A Systematic Review with Meta-analysis.

Prior meta-analysis suggested a low incidence of local adverse events after infusion of vasoactive agents via a peripheral venous catheter in children. However, the number of included patients was relatively low, and the vasoactive agents used were mostly dopamine. We performed an updated systematic review with meta-analysis using databases of MEDLINE (via PubMed) and Cochrane Central Register of Controlled Trials to explore the safety of infusing vasoactive agents, including epinephrine and norepinephrine, through peripheral venous catheters or intraosseous access in critically ill children.
