Influence of intestinal bacterial decontamination using metronidazole and ciprofloxacin or ciprofloxacin alone on the development of acute graft-versus-host disease after marrow transplantation in patients with hematologic malignancies: final results and long-term follow-up of an open-label prospective randomized trial.

In a single-center open-label prospective study, a total of 134 marrow transplant recipients with hematologic malignancies were randomly ass … sibling donors (n = 80), whereas in recipients of transplants from donors other than HLA-identical siblings (n = 54), grades II to IV acute GVHD frequencies between the study arms were not significantly different. The combined decontamination was associated with a significant reduction of culture growth of intestinal anaerobic bacteria during 5 weeks post …

Familial Hypercholesterolemia Presenting With Joint Pain in an Adolescent Girl.

Familial hypercholesterolemia (FH) is a genetic disorder causing high cholesterol levels from birth, significantly increasing the risk of early cardiovascular disease. This abstract summarizes the clinical presentation of FH in an adolescent girl who presented with musculoskeletal manifestations, progressive joint pain and cutaneous xanthomas, and retinal vessel changes. Investigations revealed significantly elevated low-density lipoprotein (LDL) cholesterol levels. Detailed family history revealed premature cardiovascular events and hypercholesterolemia in siblings and father. The patient was…

Two sisters with RSPRY1-related spondyloepimetaphyseal dysplasia.

Biallelic variants in RSPRY1 have been found to result in spondyloepimetaphyseal dysplasia. Two siblings presenting with short stature, facial dysmorphism, progressive vertebral defects, small epiphysis, cupping and fraying of metaphyses, brachydactyly, and short metatarsals harbored a homozygous missense variant c.1652G>A;p.(Cys551Tyr) in the RSPRY1 gene. The phenotype in our patients resembles spondyloepimetaphyseal dysplasia, Faden-Alkuraya type. Thus, our study provides further evidence to support the association of RSPRY1 variants with spondyloepimetaphyseal dysplasia. We observed joint d…

A novel mutation in PTHLH in a family with a variable phenotype with brachydactyly, short stature, oligodontia and developmental delay.

Mutations in PTHLH (PTH-like hormone), cause brachydactyly type E (BDE) characterized by shortening of metacarpals, metatarsals and/or phalanges with short stature. In this report we describe three siblings and their mother with a novel heterozygous mutation c.25 T > C, p.Trp9Arg in exon 2 of the PTHLH gene. Beside the known clinical features of PTHLH mutations all had a delay in speech and language development, unknown if this is related to the mutation. Patients with PTHLH mutation may have a variable phenotypic presentation.

Stüve-Wiedemann syndrome: recurrent neonatal infections caused by impairment of JAK/STAT 3 pathway.

Stüve-Wiedemann syndrome (OMIM #601559) is a rare, autosomal recessive disorder characterized by skeletal dysplasia, consecutive infections, feeding difficulties and autonomic dysregulation. We present an Afro-Caribbean family with two siblings diagnosed with Stüve-Wiedemann syndrome. The underlying loss-of-function mutation in the leukemia inhibitory factor receptor gene is thought to impair proper functioning of the JAK/STAT 3 pathway. As this affects normal functioning of T-helper cells, these patients are prone to infections with uncommon pathogens as illustrated by this case.

Foreign body oesophagus in a neonate: a common occurrence at an uncommon age.

Neonatal ingestion of foreign body is a very uncommon occurrence. We describe a 25-day-old neonate who had a large-sized stone impacted in the cervical oesophagus. The stone was accidently lodged by one of the elder siblings while playing. The stone, after failure to be retrieved endoscopically, was successfully removed by open esophagotomy. The case describes the unusual occurrence at a very uncommon age. It also re-affirms the successful role of open surgery in such situations, which are otherwise commonly treated endoscopically.

Congenital dacryocystocele: is there a familial predisposition?

Congenital impatency of the nasolacrimal drainage system is relatively common. Dacryocystocele formation is an infrequent variant of such congenital nasolacrimal duct obstruction (NLDO). Congenital dacryocystocele typically manifests as a mass in the medial canthal region; alternatively the nasolacrimal expansion may present solely intranasally as a nasolacrimal duct cyst. Despite the relative frequency of congenital NLDO, there have been few reported familial cases. We report here two female siblings with congenital dacryocystocele, and review the etiology and potential inherited predispositi…

Confirmation of microcephaly-facio-cardio-skeletal Hadziselimovic type syndrome.

Three patients are reported, including two dizygotic twins born to consanguineous parents, presenting with a disorder characterized by growth retardation, microcephaly, distinct facial features with hypotelorism, with or without epicanthic folds, prominent lips, low set ears, tetralogy of Fallot in two cases, short first metacarpals and thumbs, and hypoplastic radius and ulna in one patient. These features overlap those previously reported in two male siblings and suggest that this association of microcephaly-facio-cardio-skeletal defects could represent a unique autosomal or X-linked recessiv…

Novel mutation in the PANK2 gene leads to pantothenate kinase-associated neurodegeneration in a Pakistani family.

Pantothenate kinase-associated neurodegeneration is an autosomal-recessive disorder associated with the accumulation of iron in the basal ganglia. The disease presents with dystonia, rigidity, and gait impairment, leading to restriction of activities and loss of ambulation. The disorder is caused by defective iron metabolism associated with mutations in the PANK2 gene, which codes for the pantothenate kinase enzyme. We report on a mutation screen conducted in two siblings to establish a molecular diagnosis of the disease and a genetic test for the family.

Cerebellar hypoperfusion in infantile neuroaxonal dystrophy.

An identical abnormal pattern was detected by means of (99m)Tc-hexamethyl-propyleneamine-oxime single-photon emission computed tomography in two siblings with infantile neuroaxonal dystrophy. The markedly decreased cerebellar perfusion, along with the early motor symptoms, characteristic magnetic resonance imaging and pathologic findings, points to a preferential cerebellar involvement in this disease. A relative increase in the perfusion to the basal ganglia correlated with the magnetic resonance imaging abnormalities, highly resembling that of Hallervorden-Spatz disease in one of the males, …

[Neonatal transient hypothyroidism].

Neonatal screening for congenital hypothyroidism can detect permanent and transient hypothyroidism. The latter condition is called neonatal transient hypothyroidism. This may result from various causes, but, especially, it should be suspected whenever there is a history of maternal autoimmune thyroid disease, including Hashimoto thyroiditis, Graves disease, hypothyroidism on replacement therapy, or recurrent congenital hypothyroidism of a transient nature in subsequent siblings. Transplacental TSH-receptor antibodies may affect fetal or neonatal thyroid function. Although thyroid dysfunction m…

Sporadic cases of dentatorubral-pallidoluysian atrophy associated with maternal transmission.

We report siblings, a 21-year-old woman (proband) and her 26-year-old brother, with dentatorubral-pallidoluysian atrophy (DRPLA). There was no family history of DRPLA and no clinical abnormalities in their parents, who were both above the age of 50. Analysis of the DRPLA gene of leukocytes showed CAG repeat sizes to be 64/17 in the proband, 58/20 in her brother, and 56/8 in their mother. Sporadic cases of DRPLA can occur with maternal as well as paternal transmission.

Indonesian type of metaphyseal dysplasia.

The authors describe two Indonesian siblings affected by a hitherto undocumented form of metaphyseal dysplasia that simulates enchondromatosis. The notable clinical features were short stature and lateral bowing of the lower extremities. These features were readily recognizable at birth. The radiographic examinations showed that the metaphyseal anomalies were localized predominantly in the lower extremities, the hip joints being the most severely affected. The upper extremities showed only minimal abnormalities. The name "Indonesian type of metaphyseal dysplasia" is suggested for this autosoma…

[Evaluation of the clinical typing of patients in psychotherapeutic practice].

The authors describe the clinical typology of patients which develops spontaneously in a psychotherapeutic department. Different categories are investigated in association with other characteristics such as the functioning of the patient's original family, his position among siblings, profession, acquaintances. The typology is also compared with some characteristics of the course and results of treatment. In large groups statistical data are illustrated by brief case--histories. The revealed relationships between variables provide evidence of the "logic" of spontaneously developed typology; ju…

Hereditary gingival fibromatosis in a family with the Zimmermann-Laband syndrome.

Hereditary gingival fibromatosis is frequently an isolated condition of little consequence apart from a cosmetic problem and occasional associations with hypertrichosis and/or epilepsy. There are, however, several uncommon or rare eponymous syndromes described in which gingival fibromatosis can be a feature: these include the Zimmermann-Laband, Murray-Puretic-Drescher, Rutherfurd, Cowden and Cross syndromes. This paper describes two siblings with features of the rare Zimmermann-Laband syndrome and discusses the major aspects of this and other eponymous syndromes that may be associated with her…

A familial syndrome of infantile optic atrophy, movement disorder, and spastic paraplegia.

We describe 19 cases of a familial syndrome consisting of infantile optic atrophy and an early movement disorder in which chorea predominated. About one-half the patients developed spastic paraparesis during the second decade of life. Ataxia and cognitive deficits were common, usually of mild degree. Seventeen of the patients were females. Sixteen had similarly affected siblings, but none had affected parents. All but one belonged to the Iraqi Jewish community in Israel, giving a minimal prevalence rate in this ethnic group of about 1:10,000.

Anomalous inheritance in a kindred with split hand, split foot malformation.

A South African family with autosomal dominant split hand, split foot (SHSF) malformation has been studied. The pedigree was unusual in that three unaffected siblings with normal parents had each produced affected offspring. New mutation, germinal mosaicism and autosomal recessive inheritance are unlikely. Possible explanations of this contentious situation may be an unstable premutation or non-penetrance due to an inhibiting factor associated with the gene responsible for SHSF. Pregnancy monitoring by ultrasound of unaffected close relatives of a person with SHSF is recommended.

[Arthrogryposis multiplex congenita with associated abnormalities. Case report of a fatal course in a premature triplet].

A male premature triplet with healthy siblings was born with characteristic symptoms of a Rossi syndrome: pterygiums at neck, axillae, elbows and in a moderate way at hips and knees with inhibition of extension of the affected joints, dislocation of the right hip, club-feet, cutis laxa et hyperelastica, facial dysmorphism, double pelvis renalis with ureter duplex on the right side, kryptorchidism, hypoplastic genital. The boy died after 38 days because of a haemorrhage of the lungs. An intrauterine immobilisation is discussed as cause of the syndrome.
