Deployment of a bioabsorbable plate as the rigid buttress for skull base repair after endoscopic pituitary surgery.

BACKGROUND: Bioresorbable alloplastic implants have become desirable as a rigid buttress for reconstructing skull base defects. This study aimed to describe the use of a biodegradable plate (PolyMax RAPID) in skull base repair of endoscopic endonasal pituitary surgery and to investigate the clinical outcome and safety of this novel method. METHODS: Between January 2019 and January 2020, 22 patients with pituitary adenomas who underwent endoscopic skull base repair with a Polymax RAPID plate were included. After endonasal transsphenoidal surgery, a trimmed bioresorbable plate was placed in the

Minimally invasive plate osteosynthesis of distal fractures of the tibia.

Minimally invasive plate osteosynthesis of distal tibial fractures is technically feasible and may be advantageous in that it minimizes soft tissue compromise and devascularization of the fracture fragments. The technique involves open reduction and internal fixation of the associated fibular fracture when present, followed by temporary external fixation of the tibia until swelling has resolved. Subsequent limited, but open reduction and internal fixation of the articular fragments when displaced followed by minimally invasive plate osteosynthesis of the tibia utilizing precontoured tubular pl

What Is the Revision-free Survival of Resurfaced Allograft-prosthesis Composites for Proximal Humerus Reconstruction in Children With Bone Tumors?

BACKGROUND: Reconstruction of the proximal humerus in children who undergo bone tumor resection is challenging because of patients' small bone size and possible limb length discrepancy at the end of skeletal growth due to loss of the physis. There are several options for proximal humerus reconstruction in children, such as clavicula pro humero, free vascularized fibula grafting, massive bone osteoarticular allografting, endoprostheses, and allograft-prosthesis composites, but no consensus exists on the best method for reconstruction. Resurfaced allograft-prosthesis composites could be an alter

Interbody Bone Graft via Transdiskal Screw Holes as Treatment for High-Grade Spondylolisthesis.

BACKGROUND: Controversy exists regarding surgical treatment of high-grade spondylolisthesis (HGS) in terms of decompression with surgical reduction or in situ fusion. In situ fusion has the advantage of being less technically demanding. However, the residual bone graft area is extremely limited, and posterolateral bone grafting is complex as the transverse process of the slipped vertebrae is located anterior to the sacral ala, which correlates with high rates of pseudoarthrosis. METHODS: Four cases of L5/S1 HGS complaining of low back pain were treated using our new bone graft technique with

Buttress plating of plantar medial tuberosity calcaneus fractures through a medial approach: surgical technique and case series.

This case series introduces the medial approach for surgical management of plantar medial avulsion (PMA) fractures of the calcaneus, which frequently occur in conjunction with tongue-type fracture patterns. The technique allows for direct access to the PMA fragment, enabling precise reduction and stabilization through buttress plating. In a series of 12 patients treated at a single institution, there were no soft tissue related complications including infections, wound issues, or neurologic injuries, and only one case requiring reoperation for symptomatic lateral implants. In conclusion, the m

Mechanical effects of valgus osteotomy angle and re-fixation strategies on re-fixation of the femoral neck fracture: a finite element analysis.

AIMS: This study examined the biomechanical outcomes of osteotomy angles and the internal fixation devices for re-fixation of the femoral neck fracture. METHODS: The finite element model of femoral neck fracture was initially fixed with three cannulated screws, followed by 10° and 20° valgus osteotomy. The models were re-fixed using sliding hip screw (DHS) + anti-rotation screw (AS) or DHS + medial buttress plate (MBP). Physiological loads were applied to all models. RESULT: With 10° and 20° valgus osteotomy, the interfragmentary shear stress, sliding distance, and gap decreased, the compres

Percutaneous anterior to posterior screw fixation via lateral malleolus incision window for Bartoníček-Rammelt type 4 posterior malleolus fractures.

INTRODUCTION: The presence of a posterior malleolar (PM) fragment has a negative prognostic impact in ankle fractures. Percutaneous anterior to posterior screw fixation has been advocated for the treatment of ankle fractures involving the posterior malleolus and satisfactory results were demonstrated in several studies. The aim of this retrospective study was to evaluate the clinical effect of percutaneous anterior to posterior screw fixation via lateral malleolus incision window approach (APSF-LMA) for the treatment of posterior malleolus fracture (PMF) and present outcomes of patients with t

Surgical Techniques to Manage the Unstable Volar Lunate Facet Fragment of Distal Radius Fractures.

INTRODUCTION: The surgical treatment of distal radius fractures is one of the most common procedures performed by orthopaedic surgeons. Intraarticular fracture patterns, specifically those involving the volar lunate facet, can be particularly problematic because this fragment is crucial to radiocarpal and distal radioulnar joint (DRUJ) stability. The problems associated with these patterns are attributed to the anatomy of the volar ulnar corner of the distal radius as well as the limitations of conventional volar distal radius implants and common surgical exposures. METHODS: This article will

Routine Removal of Orthopaedic Implants in Pediatric Trauma Patients With Fracture Union: Who, When, and Why.

PURPOSE: While the indications and options expand for treatment of pediatric fractures with metallic implants, routine removal of implants remains controversial. There is a paucity of randomized prospective trials to determine the risk of retention versus removal. Prior studies provided insight into the preferences of POSNA members versus their nonpediatric counterparts; however, they did not capture specifics regarding timing, anatomic location, implant type, or patient age. This study aims to report POSNA members' practice habits regarding the routine removal of orthopaedic implants after fr

Innovative Technique of Anchoring the Upper End of the Cervical Plate With C1 Anterior Arch Screw for Upper Cervical Tumors.

BACKGROUND AND OBJECTIVES: Upper cervical spine bony tumors pose a challenge to surgeons because of their extensiveness and proximity to vital structures. Reconstruction following resection is necessary to prevent instability. Our objective is to describe a technical note on the C1 anterior arch screw with mesh cage reconstruction for anchoring the upper end of the cervical plate after resection of the upper cervical tumor. METHODS: A retrospective review of patients with primary bony tumors of the upper cervical spine after surgical resection between 2018 and 2022 was included in the study.

Fixing Cho Type IIC Distal Clavicle Fractures with Hook Plates Leads to a High Incidence of Subacromial Osteolysis: A Retrospective Study and Literature Review.

BACKGROUND: This retrospective study investigates the complications, particularly subacromial osteolysis (SAO), associated with hook plate (HP) fixation, in the treatment of unstable distal clavicle fractures characterized by complete coracoclavicular (CC) ligament rupture. The decision-making process for employing HP in fractures of this nature, such as Neer types IIB and V and Cho classification IIC, involves considerations of distal fragment size and displacement. While HP offers advantages in clinical practice, it is not without complications, with SAO being a notable concern. Factors such

Comparison of the outcomes of first metatarsophalangeal joint arthrodesis by locking plate and compression screw in patients with severe hallux valgus or hallux valgus revision.

INTRODUCTION: Arthrodesis of the first metatarsophalangeal joint (MTP1) is indicated for hallux rigidus, septic arthritis, severe hallux valgus (HV) or HV revision. Few studies have compared the functional and radiographic outcomes between indications. The goal of this study was to compare the results of MTP1 arthrodesis in patients with severe HV or HV revision at 6 months postoperative. The hypothesis was that there are no differences in the fusion rate, functional scores and radiological correction between the two groups. MATERIALS AND METHODS: This was a retrospective, observational, sing
