New therapies for alcohol problems: application to primary care.

Primary care physicians are frequently involved in the longitudinal care of patients with alcohol problems and in helping patients to decrease their alcohol consumption. Recent clinical trials provide evidence in support of new treatment strategies for these patients. Brief interventions have been used successfully to reduce alcohol consumption in patients with hazardous and harmful drinking. Twelve-step facilitation, cognitive behavioral, and motivational enhancement therapies have produced sustained drinking reductions in patients with alcohol dependence. Pharmacologic therapies, such as nal

Virtual Reality for Enhancing Self-Compassion and Reducing Burnout Among Mental Health Care Professionals: Protocol for a Pilot Feasibility Randomized Controlled Trial.

BACKGROUND: Mental health care professionals working in high-demand clinical environments are frequently exposed to patient suffering and trauma, placing them at increased risk for burnout and secondary traumatic stress. Compassion-focused cognitive behavioral therapy (CF-CBT) has shown promise in reducing psychological distress and enhancing self-compassion; however, traditional delivery methods may be limited by time, accessibility, and resource constraints. Virtual reality (VR) may provide an immersive and accessible approach for delivering compassion-focused interventions. OBJECTIVE: The …

One-Session Cognitive Behavioural Therapy for Insomnia in Integrated Primary Care: A Preliminary Study.

Chronic insomnia is commonly reported in primary care settings. One-session cognitive behavioural therapy for insomnia (CBT-I) is a brief intervention designed to fit the fast-paced nature of primary care but has not been studied in this context. This preliminary study assessed the effectiveness of one-session CBT-I in an integrated primary care clinic. Adult patients (N = 37) with clinically elevated insomnia symptoms were randomly assigned to receive either one-session CBT-I (n = 17) or an active attentional control (n = 20). The primary outcomes included insomnia symptoms, total wake time (

Examination of the Acceptability and Feasibility of a Virtually Delivered Facilitator-Led and Self-Directed Cognitive Behavioral Skills Intervention in a Sample of Physicians and Medical Learners: Mixed Methods Evaluation.

BACKGROUND: The prevalence of various mental health conditions is higher among physicians and medical learners. One common barrier to receiving adequate care includes a lack of time to see a provider and follow treatment plans. As such, virtual forms of cognitive behaviour therapy with mindfulness (CBTm) were introduced to mitigate these barriers and provide care in an efficient and effective manner. OBJECTIVE: The objective of this study was to determine the acceptability and feasibility of a 5-session CBTm program, delivered in 2 virtual formats within a population of medical learners and p

Trainee-delivered school-based intervention with guided narrative writing for adolescent anxiety and depression: a cluster randomized controlled trial.

BACKGROUND: Anxiety and depression affect 20% of children globally, with heightened risk during periods of crisis and substantial burden in low-resource settings where specialist services are scarce. Universal school-based interventions delivered by supervised trainees could promote youth mental health at scale, yet evidence for brief programs remains limited. We evaluated a brief cognitive-behavioral program (BRAVE: Be Emotionally Aware, Recognize Thoughts, Accept Challenges, Voice Emotions, Explore Solutions) with guided narrative writing (GNT-W) versus drawing control (GNT-D) among rural Ch

Multiple Therapeutic Mechanisms of an Automated Self-Guided AI-Enhanced Psychoeducation Intervention for Dementia Family Caregivers: Randomized Waitlist-Controlled Trial.

OBJECTIVE: PDC30, an online psychoeducational intervention, was found to improve depression, burden, and positive gains in dementia family caregivers, compared with a 1-month waitlist control. However, the therapeutic mechanism is unclear. This study investigated whether caregiving self-efficacy and positive reappraisal coping mediated the intervention effects. DESIGN: Longitudinal mediation within a parallel-group randomized waitlist-controlled trial. SETTING: Web-based platform accessible 24/7 globally. PARTICIPANTS: From 43 countries, 274 caregivers who provided ≥10 weekly care hours to

LARS&LISA in the Municipal Health Strategy - Description of a Project for the City-Wide Implementation of a Prevention Programme in Schools Given the high prevalence of mental health problems, their prevention in children and adolescents is an urgent task. In Braunschweig, a city-wide survey of adolescents as part of Communities That Care (CTC) revealed elevated levels of depressive symptoms, prompting a search for evidence-based, universal prevention measures. The CTC Steering Committee chose the universal, school-based, cognitive behavioral depression prevention program LARS&LISA. This artic

Cognitive behavioural therapy for insomnia and epigenetic ageing: secondary analysis from a randomised controlled trial.

BACKGROUND: Insomnia in later life has been associated with accelerated biological ageing; however, little is known about the effects of insomnia treatment on it. In this study, we evaluated the effects of cognitive behavioural therapy for insomnia (CBT-I), compared with sleep education therapy (SET), an active comparator condition, on epigenetic indicators of biological ageing in older adults with insomnia. METHODS: This secondary analysis was conducted within a larger single-site, investigator-initiated randomised controlled trial (ClinicalTrials.govNCT01641263) of adults aged 60 years or o

Fifteen-month drinking reductions in a randomized clinical trial of cognitive-behavioral and mindfulness-based treatments and a matched nontreatment-seeking community sample.

OBJECTIVE: To examine drinking outcomes over time among two samples recruited and assessed using similar criteria, procedures, and measures: heavy drinkers who were not seeking treatment and treatment seekers. METHOD: Heavy-drinking participants were recruited for a naturalistic study of the course of alcohol use in a nonhelp-seeking community sample (N = 190) and a randomized clinical trial (N = 110) of 2-4 sessions of motivational enhancement therapy followed by eight sessions of cognitive-behavioral or mindfulness-based treatment. Alcohol use was assessed with the Timeline Followback at ba

Developing Peer-Mentored Cognitive Behavioural Therapy for Adolescents With Mild-To-Borderline Intellectual Disability and Anxiety: A Qualitative Study.

BACKGROUND: Youth with intellectual disabilities are at increased risk of developing anxiety disorders. Adapted CBT shows potential for youth with intellectual disabilities, but evidence for effectiveness is limited and mixed, urging the need for further optimisation. This study aims to develop a peer-mentored CBT intervention, together with and for youth with mild-to-borderline intellectual disability and anxiety, based on scientific evidence, clinical expertise and patient preferences. METHODS: In the intervention development phase, 40 stakeholders (youth with intellectual disability, paren

Repairing Attachment Insecurities with God and PTSD Symptomology in Adult Survivors of Childhood Trauma: An Exploratory Study Using Spiritually Integrated Cognitive Processing Therapy (SICPT).

Childhood trauma occurs when an individual under the age of 18 survives or witnesses a life-threatening event that surpasses one's ability to cope. To circumvent future danger, numerous developmental changes occur in response to fear, manifesting in a multitude of symptoms that can culminate in posttraumatic stress disorder (PTSD). This debilitating condition arises from the brain and body's heightened sensitivity to environmental stimuli, including interpersonal interactions, which affect attachment styles with others and with God. Research has indicated that high levels of self-reported spir

Deepfake perpetrator conversation for adults with sexual abuse-related posttraumatic stress disorder: intervention development and multiple baseline study protocol.

Background: A conversation between a victim and a perpetrator of sexual abuse has the potential to reduce posttraumatic stress disorder (PTSD) symptoms in the victim. However, an actual conversation may not always be possible or feasible. Deepfake technology may offer a way to simulate a conversation in a clinical context. Therefore, a feasible, acceptable and potentially effective intervention protocol is required. Objective: We aimed to develop an intervention protocol for a deepfake victim-perpetrator conversation to be used in the treatment of patients with sexual abuse-related PTSD, that w

[Intensive outpatient treatment for eating disorders in Mexico, contextualized for Latin America].

Introduction: in Latin America, access to specialized treatment for eating disorders (EDs) remains limited. Despite advances in detection and diagnosis, there are still major gaps in the availability of intensive and appropriate services, particularly outside large urban centers. Most existing treatment models were developed in high-income countries and do not always reflect the sociocultural, economic, and structural realities of the region. Objective: this report aims to document the implementation and evaluation of a scalable intermediate care model for EDs, with implications for adoption i

Gambling Disorder: Diagnosis and Treatment.

Pathologic gambling, also known as gambling disorder, is a behavioral addiction. The prevalence of gambling disorder has increased in recent years as gambling has become more accessible. Gambling disorder is often underdiagnosed and may have disabling effects on patients. Although men are more often affected, the rate of gambling disorder has been increasing in women. Risk factors include comorbid substance use disorder, mental health diagnoses, lower socioeconomic status, and participation in other risky behaviors. The US Preventive Services Task Force has not addressed screening for gambling

Cognitive and behavioural factors associated with improvements in depression outcomes among individuals attending CBT for insomnia.

Cognitive behavioural therapy for insomnia (CBT-I) is the gold-standard insomnia treatment, which also produces medium-to-large antidepressant effects at a group level. However, not everyone benefits equally and CBT-I related factors associated with varying depression outcomes are not well understood, limiting the capacity for treatment optimisation and uptake of CBT-I in routine care. This study examined whether trajectories of change in daytime and bedtime general repetitive negative thinking (RNT), daytime and bedtime sleep-related RNT, and low activity levels (improved behavioural activati

ACTIVATE: A protocol for a randomised controlled evaluation of Thumos: A cognitive-behavioural intervention for medical students.

BACKGROUND: Medical students experience high rates of burnout and mental health distress, with approximately half reporting burnout, one-third experiencing depression, and one-tenth having suicidal thoughts. These issues are exacerbated during clinical placements and contribute to workforce attrition after qualification. Existing interventions are largely generic and show limited effectiveness. Thumos, a cognitive-behavioural therapy-based intervention specifically tailored for healthcare professionals and medical students, addresses the stressful situations intrinsic to medical training and p
