Impact of Long-Term CPAP Adherence on Recurrence After Atrial Fibrillation Ablation in Patients With Severe Sleep Apnea.

BACKGROUND: Obstructive sleep apnea represents an arrhythmia recurrence risk following catheter ablation of atrial fibrillation (AF). Continuous positive airway pressure (CPAP) therapy may reduce recurrence, but individual adherence to CPAP varies, potentially leading to inconsistent results across studies. METHODS AND RESULTS: We analyzed the impact of CPAP adherence on AF recurrence after catheter ablation in patients with severe obstructive sleep apnea (apnea-hypopnea index ≥30). Of 1864 consecutive patients undergoing an index catheter ablation of AF, those with severe obstructive sleep a

Sleep and periodontal health.

Sleep is fundamental for health and well-being. An adequate amount and quality of sleep is a cardinal component of a healthy lifestyle at the basis of the prevention of many non-communicable chronic diseases. Recent evidence suggests that sleep disorders, particularly obstructive sleep apnea, represent an emerging risk factor for periodontal health. This review article provides a critical appraisal of the existing literature concerning the association between sleep duration, sleep quality, sleep disorders in general, and obstructive sleep apnea with periodontal diseases, including gingivitis a

Sleep apnea syndrome: implications on cardiovascular diseases.

Global risk assessment is the standard of care for coronary artery disease management. In this setting, sleep apnea syndrome, which includes obstructive sleep apnea and central sleep apnea, is being increasingly recognized as a potentially modifiable risk factor for coronary artery disease. Emerging evidence points toward a cause and effect relationship between sleep apnea syndrome and medical conditions like insulin resistance, hypertension, heart failure, and myocardial ischemia. The effects of sleep apnea on coronary artery disease can be independent of many traditional risk factors. Contin

[The effects of clomipramine on diurnal sleepiness and respiratory parameters in a a case of Prader-Willi syndrome].

The association of Prader-Willi-syndrome with breathing disturbances such as sleep apnea syndrome and/or hypoxemia during REM sleep, REM sleep abnormalities and excessive daytime sleepiness is well known. We report the case of an 11-year-old boy who presented with Prader-Willi syndrome, obesity (body mass index [BMI] = 45.6), severe obstructive sleep apnea syndrome and significant daytime sleepiness on multiple sleep latency test. Behavioral disorders did not allowed the use of continuous positive pressure in this patient. Therefore, clomipramine (20 mg per day) was administered. Sleep examina

Hypoxemia in patients on chronic opiate therapy with and without sleep apnea.

OBJECTIVE: Animal models have shown a quantal slowing of respiratory pattern when exposed to opioid agonist, in a pattern similar to that observed in central sleep apnea. We postulated that opioid-induced hypoventilation is more likely to be associated with sleep apnea rather than hypoventilation alone. Since we did not have a direct measure of hypoventilation we used hypoxemia as an indirect measure reasoning that significant hypoventilation would not occur in the absence of hypoxemia. METHODS: We conducted a retrospective analysis of 98 consecutive patients on chronic opioid medications who

Sleep apnea syndromes.

The sleep apnea syndromes have been recognized clinically in the United States only within the past ten years. The true extent of the problem is not known, but it seems certain that these syndromes are much more common than was generally assumed five years ago. Every clinician should be aware of the signs and symptoms of sleep apnea because of the rapid and prompt response to therapeutic measures. Sleep apnea syndromes, whether obstructive or central, can result in systemic or pulmonary hypertension, arterial blood gas abnormalities, life-threatening cardiac arrhythmias, chronic respiratory fa

Hypoglossal nerve trunk stimulation: electromyography findings during drug-induced sleep endoscopy: a case report.

BACKGROUND: Literature has demonstrated hypoglossal nerve stimulation to be a safe and effective treatment for patients with obstructive sleep apnea nonadherent to positive airway pressure therapy. However, the recommended criteria for patient selection are still unable to identify all the unresponsive patients, highlighting the need for improved understanding about hypoglossal nerve stimulation for obstructive sleep apnea. CASE PRESENTATION: A 48-year-old Caucasian male patient with obstructive sleep apnea had been successfully treated with electrical stimulation of the hypoglossal nerve tru

Epilepsy and sleep-disordered breathing as false friends: a case report.

Because signs of nocturnal seizures can overlap with sleep respiratory events, clinicians can have difficulty distinguishing abnormal events related to sleep disorders from epileptic seizures. We describe the case of a 3-year-old child presenting with ictal electroencephalographic (EEG) activity associated with a particular form of atypical obstructive sleep apnea, characterized by increased respiratory rate, paradoxical breathing, desaturations, and tonic-dystonic posture associated with movement artifacts. Following cardiorespiratory polysomnography, the patient was initially misdiagnosed as

Central sleep apnea is associated with increased ventilatory response to carbon dioxide and hypersecretion of growth hormone in patients with acromegaly.

Although obstructive sleep apnea (OSA) occurs commonly in acromegaly, we have recently reported an unexpectedly high prevalence of central sleep apnea (CSA) in these patients. Acromegalic patients with CSA have increased growth hormone (GH) and insulin-like growth factor-1 (IGF-1) levels compared with their counterparts with OSA. Studies in animals, normal humans, and patients with sleep apnea have suggested that CSA is associated with increased gain of the respiratory control system. To examine the relationship between sleep apnea, respiratory control, and hormonal activity in acromegaly, we

Hypopharyngeal airway surgery.

Obstructive sleep apnea (OSA) is the result of upper airway obstruction during sleep. Hypopharyngeal airway obstruction can be caused by the prominence or relaxation of the base of the tongue, lateral pharyngeal wall, and occasionally, the aryepiglottic folds or epiglottis. Although nasal continuous positive airway pressure (CPAP) is considered as the first treatment for obstructive sleep apnea, surgery has been shown to be a viable option for patients who are intolerant of positive pressure therapy. This article presents the current state of hypopharyngeal surgery for sleep apnea. Preoperativ

Sleep Apnea and Cardiovascular Disease: Lessons From Recent Trials and Need for Team Science.

Emerging research highlights the complex interrelationships between sleep-disordered breathing and cardiovascular disease, presenting clinical and research opportunities as well as challenges. Patients presenting to cardiology clinics have a high prevalence of obstructive and central sleep apnea associated with Cheyne-Stokes respiration. Multiple mechanisms have been identified by which sleep disturbances adversely affect cardiovascular structure and function. Epidemiological research indicates that obstructive sleep apnea is associated with increases in the incidence and progression of corona

Breathing Segmentation for Per Breath Sleep Apnea Detection.

Sleep apnea is a common disorder characterized by episodes of complete (apnea) or partial (hypopnea) cessation of breathing during sleep. Apneas may result from upper airway obstruction (obstructive apnea) or reduced respiratory effort (central apnea). Although expert scoring is the clinical standard, significant inter-and intra-rater variability has spurred the development of automated detection methods. In this work, we propose a method to segment respiratory modalities from nocturnal polysomnography into individual breaths and to classify each breath as obstructive apnea, central apnea, hy-

Do weather-related ambient atmospheric-pressure changes influence sleep disordered breathing?

OBJECTIVE: High-altitude studies of sleep disordered breathing (SDB) show increases in apnea hypopnea indices with elevation gains. Hypoxic changes, rather than reductions in atmospheric pressure (AP), are thought to be the driving factor. Ambient pressure-related changes in SDB have not been extensively studied at low altitude. We performed a cross-sectional study of weather-related AP effects on measures of SDB at the University of WashingtonMedicine Sleep Institute, a Seattle, Washington-based polysomnography lab located 200 feet above sea level. METHOD: Obstructive, central, and apnea-hyp

Polysomnography and ApneaGraph in patients with sleep-related breathing disorders.

PURPOSE: To evaluate whether ApneaGraph (AG) and polysomnography (PSG) deliver comparable results in patients with sleep-related breathing disorders. PROCEDURES: A prospective study was performed, which included 14 patients with obstructive sleep apnea syndrome. Apnea-hypopnea index (AHI), hypopnea index (HI), apnea index (AI), obstructive, central and mixed apnea, oxygen saturation (SaO2), pulse and body position were simultaneously assessed by PSG and AG in each individual. RESULTS: There was a good correlation between measurements of AG and PSG for AHI, pulse, SaO2, body position and cent

Sleep apnea, sleep disorders, and hypothyroidism.

The symptoms of hypothyroidism are protean and include apathy, somnolence, lethargy, personality change, and intellectual deterioration. Many of these symptoms may be related to hypothyroid-induced sleep disorders. Hypothyroidism is associated with abnormal ventilatory drive, abnormal sleep architecture, and sleep apnea. Central, obstructive, and mixed patterns of sleep apnea are commonly observed in hypothyroidism. A case of severe sleep apnea in a grossly myxedematous patient who improved dramatically following thyroid replacement alone is presented. Myxedema is a reversible cause of sleep a

Classifying hypopneas as obstructive or central can enhance transvenous phrenic nerve stimulation therapy patient selection and outcomes.

STUDY OBJECTIVES: While not all sleep laboratories distinguish between obstructive and central hypopneas, recent research suggests that patients may receive an incorrect primary diagnosis without this effort. The remedē System Pivotal Trial studied transvenous phrenic nerve stimulation in patients with moderate-to-severe central sleep apnea. Entry criteria required apnea-hypopnea index (AHI) ≥ 20 events/h with central apnea index greater than obstructive apnea index and obstructive apneas < 20% of AHI but did not classify hypopneas. This analysis re-examined sleep studies from the trial to ass

Sleep disorders increase the risk of dementia, Alzheimer's disease, and cognitive decline: a meta-analysis.

Sleep disorders, particularly insomnia and obstructive sleep apnea, are increasingly implicated as significant contributors to cognitive decline, dementia, and neurodegenerative diseases such as Alzheimer's disease (AD) and vascular cognitive impairment and dementia (VCID). However, the extent and specificity of these associations remain uncertain. This meta-analysis evaluates the impact of common sleep disorders on the risk of developing dementia and cognitive decline. A comprehensive search of the literature was conducted to identify prospective cohort studies assessing sleep disorders and d

[Respiratory tests in sleep disorders in children].

Nocturnal polysomnography is the reference tool for the exploration of sleep disorders in children. Noninvasive methods are to be preferred for children. Recording respiratory signals identifies respiratory events (episodes of apnea, hyponea) and their effect on gas exchange. Measuring esophageal pressure, the only quantitative measure of respiratory exertion, is required for the diagnosis of high-resistant upper airways syndrome. Recordings of neurophysiolgical signals can be used to analyze sleep organization and to quantify cortical awaking reactions. The most frequent indication in pediatr

Craniofacial and upper airway morphology in pediatric sleep-disordered breathing: Systematic review and meta-analysis.

INTRODUCTION: Pediatric sleep-disordered breathing is a continuum, with primary snoring at one end, and complete upper airway obstruction, hypoxemia, and obstructive hypoventilation at the other. The latter gives rise to obstructive sleep apnea. An important predisposing factor in the development and progression of pediatric sleep-disordered breathing might be craniofacial disharmony. The purpose of this systematic review and meta-analysis was to elucidate the association between craniofacial disharmony and pediatric sleep-disordered breathing. METHODS: Citations to potentially relevant publi

Sleep in heart failure.

Sleep plays a large role in patients with heart failure. In normal subjects, sleep is usually in a supine position with reduced sympathetic drive, elevated vagal tone and as such a relatively lower cardiac output and minute ventilation, allowing for recuperation. Patients with heart failure may not experience the same degree of autonomic activity change and the supine position may place a large strain on the pulmonary system. More than half of all heart failure patients have one of two types of sleep apnea: either obstructive or central sleep apnea. Some patients have both types. Obstructive s
