CASE: We describe a 10-year-old girl who sustained an anterior radial head dislocation with ulnar plastic deformation. Closed reduction under sedation failed to maintain radial head alignment. A novel technique of ulnar contour plating without osteotomy was used for definitive stabilization. In contrast to using a traditional ulnar osteotomy, this case highlights a unique technique for treating pediatric Monteggia fracture-dislocation equivalents with ulnar plastic deformation. At the 28-month follow-up, successful clinical and radiographic outcomes were observed.
CONCLUSION: Contour plating

Fibular head instability is a rare, isolated injury in veterinary medicine, with no previous case reports in the veterinary literature. The fibular head is secured proximo-distally by the lateral collateral ligament and craniocaudally by the cranial and caudal fibular head ligaments. A 1-year-old, 26-kg, spayed female border collie mix dog was referred for chronic bilateral Salter Harris-II tibial tuberosity avulsion fractures. The dog was non-weight-bearing on the right pelvic limb, with pain and mobility localized to the right fibular head. Preoperative radiographs showed no fibular patholog

CASE: A 20-year-old man presented after a fall from an 8-foot ladder. Radiographs revealed pronation external rotation injury. Closed reduction and splinting was performed in the emergency department. Surgery was planned 6 days later to include open reduction and internal fixation of the fibula and syndesmosis, and open repair of deltoid ligament. Intraoperatively, a myotendinous rupture of the posterior tibial tendon (PTT) was identified and repaired.
CONCLUSION: PTT ruptures are rarely reported but should be suspected in high-energy mechanism ankle fractures in all patients. Direct repair c

Based on our experience, the local anesthetic concentrations used to date in perineural blocks may be excessively high. Ropivacaine 0.2% with the addition of dexmedetomidine and dexamethasone can provide surgical anesthesia in perineural blocks. Here we present a case report of 3 patients who underwent osteosynthesis of the distal radius epiphysis and distal homerus after a traumatic fracture. We decided to perform a supraclavicular block with a low concentration of local anesthetic (ropivacaine 0.2%) with adjuvants to obtain complete anesthesia of the limb.

We discuss the study by Uğur Bezirgan et al., titled "Comparison of Locking Plate and Conservative Treatment in Elderly Patients with Displaced Proximal Humerus Fractures." The authors are commended for their prospective study, which offers valuable insights into the comparison of locking plate fixation and conservative treatment. The study found no significant difference in functional recovery between the two groups; however, the locking plate group exhibited a higher incidence of complications. We express concerns regarding the potential selection bias arising from the non-randomized allocat

The tendon rupture was caused by the protruded distal edge of the palmar plate inserted for fixation of the distal radial fracture 2 years earlier. The patient was a 28-year-old patient who had stopped bending the distal phalanx of his right thumb 2 months prior to the presentation. Performance of anaesthesia and operation is presented in details. The operation went smoothly, bleeding was minimal despite the relatively extensive wound, and the patient felt only little pain at the end of the operation.

There remains debate regarding the treatment of ulnar styloid fractures associated with distal radius fractures. We present a novel technique for basal ulnar styloid fracture fixation using absorbable pins and a suture tension band construct, leveraging the rotational stability of the tension band while minimizing impingement and hardware irritation. The procedure begins with an incision over the ulnar styloid to expose the fracture. Fracture fragments are cleared of soft tissue and reduced with a dental pick before temporary fixation with a K-wire. Absorbable nails are placed into the distal

BACKGROUND: The acromion projects laterally in a curving orientation. When a fracture of the acromion occurs, despite being rare, matching the best-fit fixation plate is challenging due to few options and limited availability of fixation plate types. Alternative fixation methods can carry risk of nonunion complications.
PURPOSE: The objective of this study was to investigate the morphological curvature pattern of the acromion to assess the level of the fixation plates fitness and provide the suitable imaging modalities for evaluating the acromion curvature.
BASIC PROCEDURES: The correlation

This study aimed to compare the biomechanical efficacy of rim plating versus conventional posterior buttress plating for fixing posterolateral tibial plateau fractures (PLTPFs). Sixty synthetic tibial models were divided into three groups: posterior buttress plating with a 3.5-mm locking compression T-plate, rim plating with a 2.7-mm variable-angle cloverleaf plate and two anteroposterior screws, and rim plating alone. Static compression and cyclic loading tests were conducted to assess mechanical stability under simulated physiological conditions. Failure loads averaged 2235.75 N, 2017.46 N,

OBJECTIVE: To determine the effect of plate-bone distance and screw number on 2.0-mm locking compression plate (LCP) torsional stiffness and plate strain for a fixed working length in a diaphyseal fracture gap model.
STUDY DESIGN: In vitro biomechanical study.
METHODS: The LCP was tested with nine different combinations of plate-bone distance (PBD) (1, 1.5, and 3 mm) and screw number (SN) (two, three, or four screws per fragment) across the same working length. Six replicates per group were tested. Constructs were subjected to quasi-static, non-destructive torsion producing overall construct

Percutaneous medial plating of distal femur fractures, particularly comminuted fractures, offers improved biomechanical stability without the soft tissue insult of an open approach. The addition of a medial plate neutralizes the cantilever bending force that arises from the offset between the single lateral locking plate and the femur's anatomical axis. This cantilever bending force has been theorized to be a contributor to the high nonunion rate of distal femur fractures. Although concerns about the proximity of the superficial femoral artery have previously limited widespread adoption of med

Midface fractures present a clinical challenge in otorhinolaryngology due to their often complex injury pattern and nonspecific symptoms. Precise diagnostics, including differentiated imaging procedures, are required. Interdisciplinary consultation between otorhinolaryngology, maxillofacial surgery, neurosurgery, and ophthalmology is often necessary. When selecting radiographic modalities, radiation hygiene should be taken into account. Sonography provides a radiation-free imaging alternative for fractures of the nasal framework and anterior wall of the frontal sinus. The goal of treatment is

PURPOSE: To provide a biomechanical comparison of dorsal plating, lateral plating and intramedullary screw [IMS] fixation for extra-articular proximal phalangeal fractures.
METHODS: Midshaft osteotomies were performed on 36 cadaveric proximal phalanges. The phalanges were fixed by dorsal plating, lateral plating or IMS fixation, and subjected to a four-point bending force. Force was applied to achieve displacement of 1 mm/s, until construct failure or to a maximum of 10 mm of displacement. Clinical failure was defined as 2 mm of displacement, and force required to result in 1 mm and 2 mm of d

Flexor pollicis longus rupture is an uncommon but potentially debilitating complication after volar locking plate fixation of distal radius fractures, occurring secondary to tendon attrition against the implant. This nature of tendon injury typically precludes primary repair. This paper will illustrate 2 reconstruction techniques, an interpositional tendon graft and a tendon transfer, that have been utilized successfully by the authors.

The mainstay orthopedic surgical technique for fracture fixation involves metal plates, screws, and rods. While these methods are effective, they exhibit high rates of complications within specific populations, particularly among patients with pathologic and insufficiency fractures. IlluminOss represents a novel photodynamic bone stabilization system, approved for use in multiple countries, that serves as an alternative to traditional fracture fixation approaches for patients experiencing pathologic, traumatic, and fragility fractures. Despite the initial success of the system in fostering fra

INTRODUCTION: Traditionally, plate osteosynthesis of the anterior column combined with an antegrade posterior column screw is used for fixation of anterior column plus posterior hemitransverse (ACPHT) acetabulum fractures. Replacing the posterior column screw with an infraacetabular screw could improve the straightforwardness of acetabulum surgery, as it can be inserted using less invasive approaches, such as the AIP/Stoppa approach, which is a well-established standard approach. However, the biomechanical stability of a plate osteosynthesis combined with an infraacetabular screw instead of an

Axial micromotion between bone fragments can stimulate callus formation and fracture healing. In this study, we propose a novel mechanically compliant locking plate which achieves up to 0.6 mm of interfragmentary motion as flexures machined into the plate elastically deflect under physiological load. We investigated the biomechanical performance of three compliant plate variations in comparison to rigid control plates with small and large working lengths in a comminuted bridge plating scenario using humeral diaphysis surrogates. Under static axial loading, average interfragmentary motion was 6
