Pharmacotherapy for older adults with cancer: a digest of the Japanese clinical practice guidelines (2nd edition)-general principles.

BACKGROUND: The global population is aging rapidly, and the incidence of cancer in older adults is increasing accordingly. However, older patients often present with complex clinical conditions that are not adequately reflected by chronological age or performance status alone, making the optimization of pharmacotherapy in this population a major challenge in oncology practice. METHODS: The Japanese Society of Medical Oncology and the Japanese Society of Clinical Oncology developed the second edition of the Clinical Practice Guidelines for Pharmacotherapy in Older Adults with Cancer in accorda

Outpatient clinic for older patients with chronic kidney disease: a model of a multidisciplinary care model.

BACKGROUND: The number of older patients with chronic kidney disease (CKD) is increasing worldwide. Managing clinical conditions in this population is challenging due to multimorbidity, including frailty, malnutrition, sarcopenia, and functional status impairment. This clinical scenario, in association with the decrease in kidney function, requires specific care. METHODS: We propose a new care model for older patients with moderate/advanced CKD involving a multidisciplinary and interdisciplinary team. This team includes nephrologists, geriatric physicians, nurses, and dietitians. To achieve t

Counselling regarding a planned provision of medical aids is currently neither reimbursed nor regulated in Germany. It is offered by a range of providers with employees with varying levels of qualification and may be provided in a personal conversation, by phone, or remotely. In many cases, counselling is omitted entirely between the prescription and trial use or delivery of the product. The quality of medical aids counselling, which plays a key role in temporary, supportive or palliative care for urinary incontinence, is therefore considered problematic. In 2019, the working group "Geriatric U

Preliminary evidence for preserved synaptic density in late-life depression.

Late-life depression has been consistently associated with lower gray matter volume, the origin of which remains largely unexplained. Recent in-vivo PET findings in early-onset depression and Alzheimer's Disease suggest that synaptic deficits contribute to the pathophysiology of these disorders and may therefore contribute to lower gray matter volume in late-life depression. Here, we investigate synaptic density in vivo for the first time in late-life depression using the synaptic vesicle glycoprotein 2A receptor radioligand 11C-UCB-J. We included 24 currently depressed adults with late-life d

The association between prealbumin concentration at admission and mortality in elderly patients with hip fractures: a cohort study.

UNLABELLED: Malnutrition is associated with complications and mortality in patients of hip fracture. Prealbumin may be more suitable than albumin to accurately predict the prognosis of hip fracture in elderly patients. We found that prealbumin concentration was nonlinearly associated with mortality in elderly patients with hip fracture, and an inflection point effect was observed. OBJECTIVE: To evaluate the association between prealbumin concentration at admission and mortality in elderly patients with hip fractures. METHODS: Elderly patients with hip fractures were screened between Jan 2015

The 2023 WSES guidelines on the management of trauma in elderly and frail patients.

BACKGROUND: The trauma mortality rate is higher in the elderly compared with younger patients. Ageing is associated with physiological changes in multiple systems and correlated with frailty. Frailty is a risk factor for mortality in elderly trauma patients. We aim to provide evidence-based guidelines for the management of geriatric trauma patients to improve it and reduce futile procedures. METHODS: Six working groups of expert acute care and trauma surgeons reviewed extensively the literature according to the topic and the PICO question assigned. Statements and recommendations were assessed

Application of novel burst wave lithotripsy and ultrasonic propulsion technology for the treatment of ureteral calculi in a bottlenose dolphin (Tursiops truncatus) and renal calculi in a harbor seal (Phoca vitulina).

Marine mammals may develop kidney stones, which can be challenging to treat. We describe burst wave lithotripsy (BWL) and ultrasonic propulsion to treat ureteral calculi in a 48-year-old female bottlenose dolphin (Tursiops truncatus) and to reduce renal stone burden in a 23-year-old male harbor seal (Phoca vitulina). BWL and ultrasonic propulsion were delivered transcutaneously in sinusoidal ultrasound bursts to fragment and reposition stones. Targeting and monitoring were performed with real-time imaging integrated within the BWL system. Four dolphin stones were obtained and fragmented ex viv
