HISTORY AND CLINICAL FINDINGS: A 23-year-old woman had received a mechanical bileaflet mitral valve prosthesis because of severe mitral valve insufficiency caused by an acute bacterial endocarditis with vegetations. One year after the operation the patient suffered on two miscarriages under oral anticoagulation by phenprocoumon. The course of 76 pregnancies is reported in 51 women who became pregnant after replacement of one or more heart valves. Age at conception ranged from 17 to 39 years (mean 25). There have been 71 deliveries and five women are still pregnant at the time of writing. The problem of sizing the graft, annulus, and left ventricular outflow tract for the David reimplantation type of aortic valve preserving procedure remains a concern, particularly in patients with Marfan syndrome. To resolve this issue, a modified method used in over 20 patients is described of doing the repair around a Hegar's dilator. In essence, the size chosen is according to the patient's normalized annular size based on body surface area. This technique also creates a neosinus in the root.

The use of porcine bicuspid prosthesis might be a technical alternative, although comprehensive studies on the long-term use of these prostheses are not available.

We report an unusual case of a Starr-Edwards aortic valve malfunction with fatal dislodgment and embolization of its ball portion to the distal abdominal aorta, just above the bifurcation, leading to sudden death.
