The Ulnar Olecranon-Coronoid Notch Angle Affects Terminal Elbow Extension in Children and Adolescents.

BACKGROUND: Limitations to terminal elbow extension (TEE) in pediatric populations have been commonly associated with the degree of ligament … hypoextension, normal TEE, and elbow hyperextension (P<0.001). The most common injuries were distal radius fractures (182, 26%), elbow sprains and contusions (111, 16%), distal both bone forearm fractures (95, 14%), single or both bone shaft fractures (77, 11%), and supracondylar fractures (74, 11%). CONCLUSION: These results show that the orientation of the open …

Pathological fracture in paediatric bone tumours and tumour-like lesions: A predictor of benign lesions?

OBJECTIVE: To determine the incidence and causes of pathological fractures in paediatric bone tumours and tumour-like lesions, and to determine if they are predictive of benign lesions. METHODS AND MATERIALS: Retrospective review of children with suspected bone tumours referred to a specialist musculoskeletal oncology service between September 2019 and August 2020. Data recorded included patient age and gender, lesion location, the presence of a pathological fracture on the initial plain radiograph, and the final diagnosis made either by image-guided biopsy/curettage or based on typical imagi

Fragility Fracture Risk in Elderly Patients With Cervical Myelopathy.

STUDY DESIGN: Retrospective review. OBJECTIVE: To identify and compare the incidences of fragility fractures amongst three elderly populations: the general population, patients with surgically treated cervical spondylotic myelopathy (CSM), and patients with CSM not surgically treated. SUMMARY OF BACKGROUND DATA: CSM is a common disease in the elderly. Progression of myelopathic symptoms, including gait imbalance, can be a source of morbidity as it can lead to increased falls. METHODS: Records of elderly patients with Medicare insurance from 2005 to 2014 were retrospectively reviewed. Three

Assessment of bone microarchitecture in postmenopausal women on long-term bisphosphonate therapy with atypical fractures of the femur.

Reports of atypical femoral fractures (AFFs) in patients receiving long- term bisphosphonate therapy have raised concerns regarding the genesis of this rare event. Using high-resolution peripheral quantitative computed tomography (HR-pQCT), we conducted a study to evaluate bone microarchitecture in patients who had suffered an AFF during long-term bisphosphonate treatment. The aim of our study was to evaluate if bone microarchitecture assessment could help explain the pathophysiology of these fractures. We compared bone volumetric density and microarchitectural parameters measured by HR-pQCT i

Removal of forearm plate leads to a high risk of refracture: decision regarding implant removal after fixation of the forearm and analysis of risk factors of refracture.

INTRODUCTION: Plate fixation is the gold standard for the treatment of forearm fractures at present, and whether or not to remove the implant after bone union remains controversial. This study demonstrated some cases of refracture in adult forearm fractures after bone union and discussed the risk factors for decision-making regarding implant removal. METHODS: We reviewed patients with forearm diaphyseal fractures (including the radius, ulna, or both bones) who received open reduction and internal fixation (ORIF) from January 2008 to May 2011 in our institute. Fracture type was classified acco

Prevention and management of early treatment failures in elbow injuries.

The appropriate management of early treatment failures in patients with elbow injuries requires the identification of the cause of failure. In this work, six types of elbow injury are considered: (i) identification of early failed reduction of a dislocation or fracture-dislocation of the elbow should lead to a repeat reduction procedure, testing for elbow instability, and ligament repair, followed by the use of a hinged external fixator to allow early mobilisation. Differentiating an isolated dislocation from a dislocation combined with a fracture of the coracoid process is crucial. Re-implant

Radioulnar Synostosis After Isolated Undisplaced Distal Ulna Fracture: A Rare Case Report and Review of the Literature.

CASE: A 50-year-old man developed radioulnar synostosis and presented with restricted forearm rotation, 15 months after sustaining an undisplaced fracture of the ulna. Radiographs and computed tomography revealed a mature bony bridge between the forearm bones. Surgical excision of the heterotopic bone was performed, and bone wax was applied to mitigate hemorrhage and prevent recurrence. Postoperatively, the patient achieved near-complete pronation and supination and no recurrence was found at the end of 2 years of follow-up. CONCLUSION: Radioulnar synostosis is an infrequent complication asso

Radial Head Incarceration After Closed Reduction of a Pediatric Elbow Dislocation With a Radial Neck Fracture: A Case Report.

CASE: A 10-year-old girl sustained a radial neck fracture with a posterior elbow dislocation. She was treated with closed reduction of the elbow with subsequent intra-articular displacement of the radial head, which necessitated open reduction and pinning of the radial neck fracture. CONCLUSION: Displacement of the radial neck fracture from impingement of the capitellum on the anterior radial head during closed reduction of the elbow dislocation is a rare injury pattern. It is important to examine the radial neck in high-energy posterior elbow dislocations before attempted reduction. We prese

[Radial head anterior dislocation and ulna bowing in children: About 3 cases and review of the literature].

We present three cases of anterior dislocation of the radial head associated with plastic deformation of the ulna in children. It is a Monteggia equivalent lesion type 1 according to the Bado's classification. Radiological diagnosis often goes unnoticed to the untrained eye. The relationship between the radius and the capitulum identifies the dislocated radial head on radiographs AP or lateral. Line Mubarak highlights the bowing of the ulna on radiographs lateral. Evolution is pejorative in the absence of early orthopedic care. Older is the patient and longer is the time between injury and tre

Attritional rupture of the extensor pollicis longus tendon by an osseous spur more than 30 years after wrist injury: a case report.

A 46-year-old woman sustained a rupture of the extensor pollicis longus tendon more than 30 years after wrist injury. She was successfully treated with palmaris longus tendon graft and excision of the osseous spur. Attrition of the extensor pollicis longus tendon by a newly formed osseous spur was the major mechanism.

Missed elbow fractures misdiagnosed as radial head subluxations.

Over a 36-month period, 11 children were treated in our department for fractures about the elbow which initially went undiagnosed, as they had first undergone reduction maneuvers for a suspected radial head subluxation, without prior imaging. The most frequent final diagnosis was supracondylar humerus fracture (n = 4). A fall was reported for six children (54.5%); the mechanism of injury remained unknown in four children (36.4%). In four of eleven children (36.4%) there was moderate or major soft tissue swelling at the time of first presentation, which is uncommon in radial head subluxation. R

[Rupture of the flexor tendons on an anterior plate for distal radial fracture: four cases and a review of the literature].

We report four cases of flexor tendon ruptures which occurred after distal radial fracture and reviewed the 25 other cases reported in the literature since 1932. Analysis of these 29 cases disclosed the causes of these ruptures. A deformed callus after distal radial fracture or presence of an anterior osteosynthesis plate can under certain conditions lead to secondary flexor tendon tears. It was also found that tears of the flexor pollicis longus rupture predominate, followed by injury to the flexor digitorum profundis and superficialis of the index finger. Other tendons have only been involve

Extensor tendon involvement in Smith's and Galeazzi's fractures.

Extensor tendon involvement in three Smith's and two Galeazzi's fractures are reported. In two cases of previous Smith's fracture, the extensor indicis proprius tendon had been entrapped and ruptured in one case, and in the other the extensor digitorum communis tendon of the index was pierced by the fragment. Both showed a severe tenodesis effect on the index finger and thumb. In a new case of Smith's fracture, long extensor tendons of the fingers and thumb had been trapped beneath a dorsally displaced third fragment making closed reduction impossible. In the two cases of Galeazzi's fracture;
