Acute flexion contracture correction for failed distal femoral extension osteotomy in arthrogryposis: early results and complications.

Distal femoral extension osteotomy (DFEO) is a surgical option for knee flexion contractures in arthrogryposis multiplex congenita (AMC); however, recurrence rates are high in skeletally immature patients. This study evaluated posterior knee release with femoral shortening osteotomy as a revision procedure following failed DFEO in AMC. A retrospective review was performed on patients with AMC with failed prior DFEO who underwent revision surgery between 2021 and 2023. Six patients, ten knees, were identified. Radiographic and clinical measurements were compared using paired t tests. The primar…

Growth arrest following posterior knee release in children with arthrogryposis multiplex congenita: a previously unreported complication.

Knee flexion contractures in children with arthrogryposis multiplex congenita (AMC) significantly impair function and ambulation. Posterior knee soft tissue release is a widely used surgical intervention when conservative management fails. However, literature describing complications following this procedure remains limited. This study aims to report distal femoral and proximal tibial physeal arrest as a previously undocumented complication following posterior knee release in children with AMC. Following institutional review board approval, a retrospective case series was performed. Patients w…

Novel Type of Complicated Autosomal Dominant Hereditary Spastic Paraplegia Associated with Congenital Distal Arthrogryposis Type I.

Hereditary spastic paraplegia (HSP) is one of the most genetically heterogeneous neurological disorders. HSP is classified as pure when only … individuals who had progressive, adult onset spastic paraparesis that was associated with a distal arthrogryposis (DA) in every affected individual. They also had episodes of rhabdomyolysis without any clinical signs of myopathy. Exhaustive genetic analysis including targeted sequencing of known HSP and DA genes and whole exome sequencing did not identify the d …

A novel TPM2 gene splice-site mutation causes severe congenital myopathy with arthrogryposis and dysmorphic features.

To date, only two splice-site mutations within the TPM2 gene have been shown to be causative for congenital myopathies. While the majority of TPM2 gene mutations are causative for nemaline myopathy, cap disease or distal arthrogryposis, some mutations in this gene have been found to be associated with non-specific congenital myopathy. We report on a patient with such an unspecified congenital myopathy associated with distinctive facial dysmorphic features and distal arthrogryposis. Using the whole exome sequencing (WES) approach we were able to identify a novel heterozygous splice-site mutatio…

Arthrogryposis, hydranencephaly and cerebellar hypoplasia syndrome in neonatal calves resulting from intrauterine infection with Aino virus.

To determine the teratogenic potential of Aino virus (AINOV) in cattle, pregnant cows and fetal cattle were infected with a fresh isolate of … 56 to 263, i.e., less than the standard gestation term; they had congenital abnormalities including arthrogryposis, hydranencephaly and cerebellar hypoplasia. Antibodies specific to AINOV were detected in their precolostral serum. These results demonstrate that AINOV is a potential etiological agent of congenital malformation of cattle.

Caring for Pediatric Clients With Arthrogryposis Multiplex Congenita: The Development of an Occupational Therapy Continuing Education Course.

Occupational therapy practitioners (OTPs) are vital members of the multidisciplinary team involved in caring for pediatric clients diagnosed with arthrogryposis multiplex congenita (AMC). However, a shortage of educational resources on AMC has led to gaps in knowledge and OTPs feeling inadequately prepared to provide comprehensive care. A 4-h continuing education (CE) course on caring for pediatric clients with AMC was designed using evidence-based literature and structured using the person-environment-occupation model. The occupation-based model guided learners to understand how therapy can i…

Sudden Release of a Fixed Contracture as a Clue to Occult Humeral Fracture in a Neonate With Arthrogryposis Multiplex Congenita.

A neonate with multiple congenital contractures consistent with arthrogryposis multiplex congenita was found to have an initially clinically occult humeral shaft fracture immediately after axillary cleansing. The first abnormal sign was the sudden release of a previously fixed upper-limb posture, which initially resembled improvement in contracture, before swelling, erythema, or deformity became apparent. Radiography confirmed a spiral fracture of the midshaft of the left humerus, which healed with conservative treatment. This case highlights that an abrupt apparent improvement in a fixed cont…

CRISPR Activation Reveals the Spliceogenicity of an Intronic NEB Variant in Fetuses With Arthrogryposis Multiplex Congenita 6.

Whole-genome sequencing identifies intronic variants whose pathogenicity can be predicted with tools like SpliceAI. However, an actionable classification of such variants may require RNA-based validation, which can be limited by low expression in clinically accessible tissues. We report two fetuses from one family with Arthrogryposis multiplex congenita 6 (AMC6 [OMIM # 619334]) and biallelic NEB variants: a paternally inherited likely pathogenic frameshift variant, Chr2(GRCh38):g.151579391del; NM_001164508.2:c.16653del; NP_001157980.2:p.(Asp5552ThrfsTer5), and a maternally inherited intronic v…

Arthrogryposis and axonopathy in the spinal cord in offspring of beef cattle grazing regrowth Sorghum spp. in Brazil.

Sorghum-associated arthrogryposis and axonopathy is a rare congenital condition reported in ruminants and horses. Here, we describe the epidemiologic, clinical, and pathologic aspects of nine outbreaks of this condition in beef cattle in midwestern and southeastern Brazil (2014-2023). These regions are the most productive grain sorghum areas, where livestock producers use the straw extensively, producing a high regrowth during drought periods. All the affected pregnant cows had grazed sorghum stover for periods ranging from 25 days to 4 months. They showed abortion in the final and middle thir…

Using Telerehabilitation to Deliver a Home Exercise Program to Youth With Arthrogryposis: Single Cohort Pilot Study.

BACKGROUND: Arthrogryposis multiplex congenita (AMC) is characterized by joint contractures and muscle weakness, which limit daily activities. Youths with AMC require frequent physical therapeutic follow-ups to limit the recurrence of contractures and maintain range of motion (ROM) and muscle strength; however, access to specialized care may be limited because of geographical distance. Telerehabilitation can offer a potential solution for delivering frequent follow-ups for youth with AMC, but research on the use of telerehabilitation in children with musculoskeletal disorders is scarce. A 63-bp insertion in exon 2 of the porcine KIF21A gene is associated with arthrogryposis multiplex congenita.

A recessive form of arthrogryposis multiplex congenita (AMC) was detected 20 years ago in the Swiss Large White (SLW) pig population. A diagnostic marker test enabled the identification of carrier animals, but the underlying causal mutation remains unknown. To identify the mutation underlying AMC, we collected SNP chip genotyping data for 11 affected piglets and 23 healthy pigs. Association testing using 47 829 SNPs confirmed that AMC maps to SSC5 (P = 9.4 × 10-13 ). Subsequent autozygosity mapping revealed a common 6.06 Mb region (from 66 757 970 to 72 815 151 bp) of extended homozygosity in …
