Acute phase reactants predict the risk of amputation in diabetic foot infection.

BACKGROUND: prediction of amputation would aid clinicians in the management of diabetic foot infections. We aimed to assess the predictive value of baseline and post-treatment levels of acute phase reactants in the outcome of patients with diabetic foot infections. METHODS: we collected data prospectively during minimum follow-up of 6 months in patients with infected diabetic foot ulcers hospitalized in Dokuz Eylul University Hospital between January 1, 2003, and January 1, 2008. After excluding patients who did not attend the hospital for follow-up visits regularly (n = 36), we analyzed data

Synovitis in angioimmunoblastic lymphadenopathy with dysproteinemia simulating rheumatoid arthritis.

We describe a patient with angioimmunoblastic lymphadenopathy with dysproteinemia who developed a symmetric, rheumatoid-like, peripheral polyarthritis. Radiographs of the involved joints revealed soft tissue swelling without erosions or cartilage loss. Rheumatoid factor and fluorescent antinuclear antibodies were negative, and C-reactive protein and erythrocyte sedimentation rate were normal. Synovial fluid analysis showed an inflammatory effusion (white blood cell count of 3,500/mm3, with 76% polymorphonuclear leukocytes). A closed synovial biopsy of the wrist revealed a mononuclear infiltrat

Benign congenital erythropenia.

Normochromic anaemia was diagnosed in two brothers in early infancy. At the time of this report they were aged 10 (M.H.) and 12 (T.H.) years. A defect of the atrial septum was found in both patients. They had slight skeletal malformations. Their haemoglobin values have remained constantly at the level of about 100 g/l and the RBC count at about 3.5 x 10(12)/l. At repeated examinations, both the patients and their parents had low or borderline low erythrocyte enolase levels. Erythropoietin levels were normal and so were numerous other laboratory tests, including analysis for abnormal haemoglobi

B and T lymphocytes in viral hepatitis.

Peripheral blood lymphocytes were investigated in 20 patients with acute viral hepatitis. For assessment of B lymphocytes the rosette test was carried out using sheep erythrocytes coated with antibodies and murine complement. T lymphocytes were recognized by their ability of binding uncoated sheep erythrocytes. In 16 cases the results of EAC test in patients with viral hepatitis resembled those in healthy donors. In 3 cases a significant decrease was observed and in one case a significant rise was found in the number of cells forming EAC rosettes. The T lymphocyte count was within normal range

Are neutrophil, platelet and eosinophil-to-lymphocyte ratio and red blood cell distribution width can be used for nasal polyposis?

INTRODUCTION: The aim of this study is to explore the relationship of neutrophil, platelet and eosinophil-to-lymphocyte ratios and red blood cell distribution width values with nasal polyposis, and whether this could be a predictive parameter that can be used for the severity and recurrence risk of the disease. METHOD: Neutrophil/lymphocyte, platelet/lymphocyte, eosinophil/lymphocyte ratios and red blood cell distribution width values were compared between nasal polyposis patients who were reviewed in a retrospective manner and the control group comparable for age and sex. Moreover, the nasal

Effects of physical stress on complete blood count and venous blood gas profile of individuals with sickle cell trait.

The association between sickle cell trait (SCT) and adverse effects of exercise has been controversial. While individuals with SCT are at higher risk of sudden death, the mechanism for this outcome remains to be elucidated. In order to shed light on this controversy, we have monitored venous blood count and blood gas parameter values in normal and SCT subjects during treadmill exercise. White and red blood cell counts and hemoglobin changed significantly over time in both the SCT and normal groups, with peak exercise values different from pre-exercise or post-exercise values. Red blood cell co

Susceptibility of dogs to infection with Ehrlichia chaffeensis, causative agent of human ehrlichiosis.

Ehrlichia chaffeensis, the newly recognized agent of human ehrlichiosis, is closely related to E canis, the causative agent of canine ehrlichiosis. Eight pups were inoculated IV with E chaffeensis-, or with E canis-infected DH82 cells, or organisms released from these host cells. Two additional pups served as nonexposed controls. Marked thrombocytopenia was observed in the E canis-infected pups, but not in those infected with E chaffeensis. Homologous serologic response was observed in the E chaffeensis-exposed pups by postinoculation day (PID) 14 and in the E canis-exposed pups by PID 21. Ehr

The Systemic Immune Signature of Cholesteatoma: A Comprehensive Cross-Sectional Analysis of Hematological Markers and Clinical Staging.

BACKGROUND: Cholesteatoma is an inflammatory disease of the middle ear, which is characterized by hearing loss, bony destruction, and increased potential for complications. The inflammation plays a major role in the pathogenesis of disease; however, the relationship between systemic haematological inflammatory markers and disease severity remains inadequately understood. This study attempts to evaluate this association between conventional and novel inflammatory markers and the staging of cholesteatoma. METHODS: This study is a prospective cross-sectional analytical study, conducted at a tert

Abnormalities of blood selenium and glutathione peroxidase activity in patients with acquired immunodeficiency syndrome and aids-related complex.

Severe protein-calorie malnutrition is common in patients with AIDS and could contribute to the progressive deterioration characteristic of that disease. Selenium deficiency could also have a negative impact on immune function and other organ functions vital for recovery from infectious diseases. Therefore, to assess any role for selenium in AIDS we determined plasma and erythrocyte selenium levels and glutathione peroxidase activity in 13 patients with AIDS compared to 8 patients with AIDS-related complex (ARC) and 14 healthy controls. Plasma selenium levels were significantly reduced in AIDS

Genetic variants associated with white blood cell count amongst individuals with sickle cell disease.

BACKGROUND: Sickle cell disease (SCD) is a Mendelian disorder characterized by a point mutation in the β-globin gene that leads to sickling of erythrocytes. Several studies have shown that absolute neutrophil count is strongly associated with clinical severity of SCD, suggesting an apparent role of white blood cells (WBC) in SCD pathology. However, the mechanism by which genetic variants lead to WBC count differences in SCD patients remains unclear. METHODS: Genome-wide association (GWA) analyses were carried out amongst a cohort of 2409 Brazil SCD participants. Association of WBC count and g

[One nursing case of nasal cavity paranasal sinus and maxillofacial huge myoepithelial carcinoma patient].

A patient diagnosed with myoepithelial carcinoma was recently treated in our department. The neoplasm was huge, located in the left maxillofacial region, blocking both eyes, impeding feeding. About one month before admission, the tumor began to bleed frequently, about 100 ml each time, causing headache, dizziness, fatigue, and cold sweats. CT showed the maximum diameter of the tumor was about 23 cm, with uneven density, and maxillofacial bone destruction. MRI revealed internal bleeding and necrosis inside the tumor. After admission, blood routine test showed erythrocyte count 3.64 x 10(12)/L(↓…

Pelvic osteomyelitis mimicking septic hip arthritis: a case report.

Peripelvic infections are rare, compared with the incidence of septic hip arthritis, but are serious, requiring emergent treatment. They often are not included in differential diagnoses for patients presenting with fever, pain, inability to bear weight, elevated white blood cell count, and elevated erythrocyte sedimentation rate. Most patients are treated initially as a septic hip arthritis. Early diagnosis and treatment are crucial to outcome in peripelvic abscess. Use of MRI may help to elucidate the correct diagnosis. Previously reported peripelvic infections included obturator internus and

Epiploic appendagitis and omental infarction.

Epiploic appendagitis and omental infarction are benign self-limiting disorders. They are uncommon, though more common than is generally assumed. In both diseases the main clinical symptom is non-specific focal abdominal pain, with a normal or moderately raised white blood cell count and erythrocyte sedimentation rate. These findings often mimic an abdominal surgical emergency, which leads to clinical misdiagnosis of more common conditions such as appendicitis or diverticulitis. This may result in an unnecessary laparotomy. Ultrasonography (US) and computed tomography (CT) show characteristic

Rosai-Dorfman disease of the subglottis.

A case of subglottic narrowing caused by an extranodal deposit of sinus histiocytosis in a 29-year-old man is reported. The characteristic features of Rosai-Dorfman disease, such as massive painless cervical lymphadenopathy, fever, raised erythrocyte sedimentation rate and raised white cell count, were all found to be absent. However, typical histological features, such as diffuse lymphoplasmacytic infiltrate, Russel bodies, foamy histiocytes and histiocytes with emperipoiesis, confirmed the diagnosis. Rosai-Dorfman disease is an uncommon disorder and its presentation with progressive subglott

[Subacute toxicity of triphenyltin acetate (TPTA) in the rabbit: clinical observations and hematologic, hematochemical and enzymatic changes].

Twenty-eight immature male rabbits were divided into four experimental groups and administered a standard diet containing respectively 0, 15, 75 and 150 ppm of triphenyltin acetate (TPTA) for 70 days. The highest dose group showed a decrease in erythrocyte count, haemoglobin content and packed cell volume. As early as on day 35, leucopenia and lymphopenia were recorded in all treated groups, thereby confirming the well-known immunosuppressive activity of organo-tin compounds. Both total serum protein and albumin levels apparently altered by TPTA treatment. Conversely, there was a dose-dependen…

Evaluation of acute gait abnormalities in preschool children.

The charts of 60 consecutive children aged less than 5 years hospitalized for evaluation of a new onset limp or refusal to bear weight were reviewed. Only 1 of 22 patients with a normal complete blood count (CBC), erythrocyte sedimentation rate (ESR), and temperature had an infection. Of the 14 patients with a diagnosis of infection, only one had a normal CBC, ESR, and temperature. Radiographs were diagnostic in only four cases, whereas aspiration identified nine of 13 infections. Thirty-five bone scans were performed; 18 led to a definitive diagnosis including synovitis osteomyelitis, Perthes

Acute febrile neutrophilic dermatosis (Sweet's syndrome). Successful treatment with short term corticosteroids.

Acute febrile neutrophilic dermatosis is a disease of unknown etiology characterized by fever, tender skin lesions, and arthritis. Histopathologic examination shows a perivasculitis of the skin with lack of inflammation of vessels. It occurs in a female: male ratio of 4:1 with middle age females affected most frequently. We report a 17-year-old male with acute febrile neutrophilic dermatosis in whom the relationship between the arthritis, skin eruptions, white blood cell count and erythrocyte sedimentation rate are presented. The young man was treated with prednisone for only 5 days with compl
