Fibular Nail Fixation: Topical Review.

BACKGROUND: Intramedullary implants have been used historically in long bone fractures with success. In recent years, a variety of intramedullary implants for the treatment of fibular fractures have been investigated. These various implants have not been assessed together in a cohesive manner. In this review, we assess implants used for intramedullary fixation of fibular fractures with respect to implant design and clinical results. METHODS: A comprehensive systematic literature review for intramedullary implants in fibular fractures was performed. All publications that assessed intramedullar

Augmentation Plate Fixation for Treating Subtrochanteric Fracture Nonunion.

BACKGROUND: The treatment of subtrochanteric fracture nonunion is challenging. Although revision with either an intramedullary or extramedullary device had been advocated with acceptable results, complications that require secondary procedures still arise. The use of an intramedullary device with augmentation plate fixation is a well-known approach for femoral or tibial diaphyseal nonunion. However, this approach has not previously been reported for subtrochanteric fracture nonunion. MATERIALS AND METHODS: A series of 21 cases of subtrochanteric fracture nonunion treated with an intramedullar

Use of a Percutaneous Pointed Reduction Clamp Before Screw Fixation to Prevent Gapping of a Fifth Metatarsal Base Fracture: A Technique Tip.

Intramedullary screw fixation has become widely accepted as the standard of care for operative treatment of Jones fractures, allowing not only accelerated rehabilitation but also reduction of the risk of repeat fracture. The unique anatomy of the fifth metatarsal--mainly its inherent lateral curvature--makes fixation technically challenging. In general, surgical fixation should be performed with the largest screw possible, in both diameter and length, which will provide the strongest possible construct. However, an increased screw length and width have been associated with complications, inclu

Pediatric and Adolescent Tibial Shaft Fractures: Flexible Nail, Rigid Nail, Plate, or External Fixation.

Tibial shaft fractures are common injuries in children and may occur as a result of trauma, sports-related activity, or falls. Casting has historically been considered the gold standard form of treatment; however, the inherent risks associated with casting, such as malunion and prolonged immobilization, have led to a rise in surgical techniques to treat these injuries. Minimally invasive, elastic stable intramedullary nailing is commonly used, whereas rigid intramedullary nailing is better suited for older children. Patients with complex fractures and severe soft-tissue injury or bone loss are

[Failures of clavicular intramedullary wire fixation and their causes].

The fixation of fractures of the clavicle by intramedullary Kirschner wires is a semirigid method. External fixation of the arm after operation is, therefore, necessary. 25 out of 38 fresh fractures in our series healed in ideal position of the fragments. Another eight fragments healed under angulation of 10 degrees to 35 degrees. Five patients developed pseudarthrosis, three of whom had multiple fragmentation. Only two of a total of eight multiple fractures healed in good position. The causes for negative results are multiple fragmentation or technically insufficient fixation--mainly perforat

The spectrum of intramedullary nailing of the tibia.

Intramedullary nailing of the tibia has been used mainly in selected cases of fresh diaphyseal fractures and nonunions. However, with modern variations of the technique, the indications can be expanded considerably. Interlocking nailing has increased the number of fractures suitable for intramedullary fixation. With this technique or with other additional measures, intramedullary fixation can be used after correction osteotomies and for stabilization of metastases and pathologic fractures. When used on correct indications and in the absence of complications, intramedullary nailing is the metho

[Intramedullary Kirschner wire osteosynthesis in treatment of distal metacarpal fractures].

The intramedullary Kirschner wire fixation of distal metacarpal fractures reported by Foucher et al. combines the known advantages of intramedullary implants with a reduction of iatrogenic soft tissue trauma. We applied this minimal invasive internal fixation technique in 38 patients with fractures dislocated by more than 20 degrees and/or with rotatory deformity. After opening the intramedullary cavity from the base of the respective metacarpal bone and after fracture reduction, two pre-bent Kirschner wires were intramedullary inserted in an orthograde fashion. The pre-bent distal end of the

Difficult Management of a Femur Fracture in a Patient with Dyskeratosis Congenita: A Literature Review and Case Report.

CASE: A 14-year-old boy with dyskeratosis congenita (DKC), status-post unrelated bone marrow transplant (BMT), sustained a femoral shaft fracture. Despite immediate fixation with the widest possible titanium elastic nails, fixation construct rigidity was insufficient and malunion occurred with refracture 5 years later. Revision fixation with rigid intramedullary nailing has maintained position for 1 year, although callus formation remains meager. CONCLUSION: This is the first article to detail fracture care for a DKC patient. Although BMT increases lifespan, patients seemingly remain skeletal

Gamma and other cephalocondylic intramedullary nails versus extramedullary implants for extracapsular hip fractures in adults.

BACKGROUND: Two types of implants used for the surgical fixation of extracapsular hip fractures are cephalocondylic intramedullary nails, which are inserted into the femoral canal proximally to distally across the fracture, and extramedullary implants (e.g. the sliding hip screw). OBJECTIVES: To compare cephalocondylic intramedullary nails with extramedullary implants for extracapsular hip fractures in adults. SEARCH STRATEGY: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (April 2010), The Cochrane Central Register of Controlled Trials (The Cochrane Librar

Comparison of Atypical and Osteoporotic Femoral Shaft Fractures in the Elderly: A Multicenter Study.

BACKGROUND: In atypical femoral fractures, owing to the high rates of complications and delayed healing that accompany the plate fixation, the most favorable treatment is intramedullary nailing. Although there is insufficient evidence, plate fixation is chosen due to anterolateral bowing of the femur. This study compared the bone healing time and rates of complications in atypical femoral shaft fractures and osteoporotic femoral shaft fractures. METHODS: We searched the medical records of 3 institutions in Japan for patients with femoral shaft fractures who visited between 1 January 2010 and

Antegrade Versus Retrograde Approach to Interlocking Nail Fixation of Femoral Diaphyseal Fractures: Is there Difference in Time to Union?

BACKGROUND: Antegrade reamed interlocked intramedullary nailing is the treatment of choice forfemoral diaphyseal fractures. More recently, retrograde intramedullary nailing of the femur has been seen as a viable alternative to the antegrade nail. METHODS: This was a prospective comparative study. Consecutive patients were placed alternately into a study group (retrograde nailing) and a control group (antegrade nailing) in a ratio of 2:1 i.e two retrograde nailing for every antegrade nailing. Forty one fractures had interlocking intramedullary fixation via the retrograde approach (study group)

Fifth Metatarsal Jones Fractures in the Athlete.

UNLABELLED: Fifth metatarsal fractures, otherwise known as "Jones" fractures, occur commonly in athletes and nonathletes alike. While recent occurrence in the popular elite athlete has increased public knowledge and interest in the fracture, this injury is common at all levels of sport. This review will focus on all three types of Jones fractures. The current standard for treatment is operative intervention with intramedullary screw fixation. Athletes typically report an acute episode of lateral foot pain, described as an ache. Radiographic imaging with multiple views of the weightbearing inju

Management of pediatric femoral shaft fractures.

Femoral shaft fractures are the most common major pediatric injuries managed by the orthopaedic surgeon. Management is influenced by associated injuries or multiple trauma, fracture personality, age, family issues, and cost. In addition, child abuse should be considered in a young child with a femoral fracture. Nonsurgical management, usually with early spica cast application, is preferred in younger children. Surgery is common for the school-age child and for patients with high-energy trauma. In the older child, traction followed by casting, external fixation, flexible intramedullary nails, a

Disease Progression Without Fracture in Anterolateral Tibial Curvature: A Case Report and Literature Review.

CASE: A 15-year-old girl presented with isolated anterolateral tibial bowing without fracture, an unusual manifestation. The deformity was initially corrected with a double-level osteotomy sparing the apex and intramedullary nail fixation. A second corrective procedure was performed for residual deformity, resulting in satisfactory alignment and union. CONCLUSION: Congenital tibial pseudarthrosis most often progresses to fracture in early childhood, but this rare presentation without fracture highlights the variability of the condition. Successful correction was achieved through staged, apex-
