In previous randomised clinical trials of thromboprophylaxis after total hip replacement, low-molecular-weight heparin has been given for an arbitrary 7 to 14 days. Binding of heparin and low molecular weight heparin fragments (CY 222, Mr range 1500-8000) to human vascular endothelial cells was studied. Primary culture of human umbilical vein endothelial cells and either 125I or 3H-labeled heparin or [125I]CY 222 were used. Slow, saturable and specific binding was found. Treatment of the underlying cause and supportive care constitute the basic principles of management of disseminated intravascular coagulation. The role of anticoagulation with heparin is controversial in the absence of any controlled studies. This case report describes two patients with acute obstetric disseminated intravascular coagulation in whom the use of heparin resulted in marked clinical improvement. Treatment with heparin may be of help in situations where there is a delay in elimination of the underlying cause.

The effect of low-dose heparin prophylaxis on venous thrombosis and bleeding after major elective surgery was studied in a prospective controlled study of 820 patients. The total incidence of venous thrombosis detected with leg-scanning using fibrinogen labeled with radioactive iodine (125I) was reduced from 16.0% in the control group to 4.2% in treated patients. The effect of different dilutions with heparin solutions or saline on blood PCO2, pH and standard bicarbonate was investigated. Blood was first equilibrated to give about 40 or 60 mmHg PCO2. The solutions were in equilibrium with room air. The effect on blood PCO2 etc. could be fully explained by the dilution with a medium having a much lower PCO2. Thus, correction of the heparin solution to pH 7.40 and PCO2 40 mmHg eliminated the effect on PCO2, pH and standard bicarbonate. With ordinary procedure for blood heparinization (about 2% dilution) the effect is practically negligible.
