A group of 70 patients who had attended the Keratoconus Clinic at Moorfields Eye Hospital over a 10-year period were analysed retrospectively. At the end of this period 23 eyes had undergone corneal transplantation. This gave an overall probability of 16%, various information obtained at the initial visit was then analysed for a possible relationship with the probability of needing a transplant within ten years. Keratoconus is an ectatic corneal disorder that results in a central or inferior conelike anterior protrusion. This results in irregular myopic astigmatism, causing considerable visual impairment. We have completed a retrospective clinical study of keratoconus in the central Kentucky area. When compared to other national studies of keratoconus, our study patients generally had more severe disease at the time of diagnosis and had a higher rate of keratoplasty. Keratoconus treatment is highly successful, and early diagnosis is warranted to prevent unnecessary visual loss.

By means of a Digilab pneumotonometer the author determined every 2 hours (from 7 a.m. till 10 p.m.) the IOP in 73 eyes of 37 patients with a keratoconus; there were 25 men and 12 women aged 16 to 40 years. The mean upper value for the R. eye amounted 19.243 +/- 2.842 mm Hg, for the L. eye 18.056 +/- 2.797 mm Hg. The means oscillation of the IOP in a day equalized 7.000 +/- 2.415 mm Hg in the R. eye and 6.000 +/- 2.378 mm Hg in the L. eye.

Forty cases of keratoconus were surgically treated with epikeratoplasty. The surgical technique is aimed at reducing the degree of high irregular myopic astigmatism of keratoconus and providing the ectatic cornea with a permanent mechanical support. Nearly all patients showed a satisfactory improvement in uncorrected and best corrected visual acuity. This reduction of preoperative astigmatism and spherical equivalent with a regularization of the corneal surface permitted the fitting of contact lenses for daily wear in patients who were preoperatively intolerant of them.

Epikeratophakia surgery is a relatively new surgical technique which involves suturing of prelathed corneal tissue on top of the recipient's cornea. The epikeratophakia lens is lathed according to the recipient's refractive needs and therefore can correct myopia and hyperopia; a plano lenticle can be used to correct keratoconus.
