An 11-year-old male neutered domestic shorthair cat underwent investigations for right mandibular and prescapular lymph node enlargement and weight loss. Cytology of the mandibular and prescapular lymph nodes revealed marked pyogranulomatous inflammation and mild neutrophilic, non-septic inflammation, respectively. Excisional biopsy of both lymph nodes was performed. Histopathology of the mandibular lymph node revealed severe pyogranulomatous inflammation, with prominent Splendore-Hoeppli phenomenon. Histopathology of the prescapular lymph node was consistent with reactive lymphoid hyperplasia

INTRODUCTION: We present a rare case of tuberculous retroperitoneal lymphadenitis after kidney transplantation that was difficult to diagnose and treat.
CASE PRESENTATION: A 52-year-old man who received a kidney transplantation from his wife presented with right lower quadrant pain. Computed tomography (CT) scan revealed multiple enlarged retroperitoneal lymph nodes, which later formed abscesses. From the positive Mycobacterium tuberculosis polymerase chain reaction test and smear in the drainage fluid, he was diagnosed with tuberculosis. Despite treatment with antituberculosis drugs, the abs…

BACKGROUND: Paradoxical reaction (PR) refers to a worsening of existing or new TB-related lesions after initiating effective anti-tuberculosis therapy. While commonly observed in lymph node or CNS TB, PR involving the pericardium is exceedingly rare in immunocompetent patients.
CASE PRESENTATION: A 22-year-old Vietnamese man with mediastinal tuberculous lymphadenitis and endobronchial TB was started on standard anti-TB therapy. After initial improvement, he developed pericardial effusion three months into treatment. Cultures and PCR were negative, but elevated ADA levels were detected. Based …

Hyper Immunoglobulin M syndrome is rare. We report a 7-month-old infant who suffered Hyper Immunoglobulin M syndrome and developed non-tuberculous mycobacterial (NTM) lymphadenitis. The former was confirmed by next-generation sequencing revealed X-linked recessive, whilst initial immunoglobulin testing displayed high IgM levels. The latter was diagnosed by lymph node pus acid-fast staining smear, culture and PCR. The child received therapy. Meanwhile, he was treated with immune supporting. The child was followed up for 11 months, and he progressed well.

OBJECTIVES: Management of paediatric non-tuberculous mycobacteria cervicofacial lymphadenitis (NTMCL) is variable without standard practice in the U.K. We undertook a survey to evaluate current U.K. practice in tertiary paediatric units.
METHODS: A survey of twenty-one tertiary paediatric centres to evaluate NTMCL management.
RESULTS: We received over 90% response rate. Only 21% have local guidelines, with the majority believing that written national guidance would improve management. Surgery is the most common management choice (69%), with a more limited procedure if skin is involved and/or…

Comorbidities between tuberculosis and leprosy are expected in endemic regions. Pulmonary tuberculosis and cutaneous leprosy are the most prevalent coinfections. One of the common manifestations of tuberculosis is generalized lymphadenopathy. In contrast, leprosy is clinically less suspected to manifest as a generalized lymphadenopathy, and it is pathologically unusual to diagnose leprosy primarily in lymph nodes. Concomitant tuberculous and lepromatous lymphadenitis are unprecedented and clinically unexpected, particularly in nonendemic countries. This imposes diagnostic challenges. We report…

PURPOSE: The significant pathogens associated with paediatric cervical suppurative lymphadenitis (CSL) are unclarified, and there is a lack of clinical evaluations of antibiotic regimens in paediatric CSL. We aimed to (1) explore the bacterial findings and the associated primary sites of infection in paediatric cases of CSL and (2) evaluate the clinical outcomes in patients treated with different antibiotic regimens.
METHODS: All children (< 18 years) treated for non-mycobacterium CSL at the Department of Otorhinolaryngology, Aarhus University Hospital, from 2001 to 2018 were retrospectively …

Cryptococcosis is a fungal infection that is typically associated with acquired immunodeficiency syndrome (AIDS). The advent of highly active antiretroviral therapy has decreased the frequency of this infection, but has led to the emergence of atypical cases of immune reconstitution inflammatory syndrome (IRIS). Here, we describe the case of a 40-year-old man who was diagnosed with HIV infection and cryptococcal meningitis. He was successfully treated with antifungals and then started antiretroviral therapy. The patient returned to the hospital 15 months later complaining of fever, pain, and n

Leprosy is also known as Hansen disease, as in some countries the diagnosis of leprosy carries a negative stigma and patients fear being shunned as outcasts. Presently, leprosy is primarily limited to specific geographical regions in resource-poor countries. As a result, there is increased difficulty for the younger generation of physicians today to correctly identify leprosy due to a lack of exposure and a low-index of suspicion, particularly in developed countries. In this case, the indurated lesions over the face demonstrated a preference for the outer lateral aspects over the maxillary are

BACKGROUND: Children are more likely than adults to develop extra-pulmonary tuberculosis (EPTB), which often presents as cervical lymphadenopathy. The role of surgery in management is uncertain. We reviewed all head and neck EPTB cases presenting to our tertiary care pediatric institution over a twelve-year period.
METHODS: All children 18 years of age and younger with EPTB involving the head and neck were included. We recorded clinical data and age at diagnosis, birth country, BCG vaccination status, as well as radiographic, surgical, histological, and microbiological results.
RESULTS: All

INTRODUCTION: Mediastinal tuberculous lymphadenitis (MTL) is mostly seen in primary tuberculosis in children, uncommon observed in adults. It usually presents the toxic symptoms of tuberculosis but rarely with symptoms characteristic of esophageal compression, such as dysphagia. Such patients can easily be misdiagnosed as esophageal neoplasm and get delayed or faulty treatment.
CASE REPORT: A 32-year-old man presented with dull chest pain of one month and dysphagia of five days. CRP was elevated, and a skin test was strongly positive. At upper endoscopy, a protruding lesion covered by normal …
