Quality of life of patients with segmental mandibular resection and immediate reconstruction with plates.

PURPOSE: The surgical management of some head-and-neck pathology affects a patient's form and fundamental function and, eventually, the quality of the individual's daily life. Restoring mandibular continuity after resection will improve form, function, and eventually, quality of life (QOL). The aim of this study was to evaluate the effect of our limited restoration with immediate plate reconstruction after segmental mandibular resection on the QOL of patients. PATIENTS AND METHODS: A total of 13 patients were included in the study. They were treated for benign but aggressive mandibular pathol

Palatal reconstruction.

PURPOSE OF REVIEW: The palate is a critical structure, playing pivotal roles in speech, swallowing, and mastication. Reconstruction of the palate is among the most difficult challenges faced by head and neck reconstructive surgeons. The primary aims of this review are to catalog the evolution of the classification systems for palatal defects, discuss decision making surrounding the various options for hard palate reconstruction, and address the special challenges and techniques involved in soft palate reconstruction. RECENT FINDINGS: The Okay Classification System has become the standard by w

Relationships between geometry and kinematic characteristics in the temporomandibular joint.

Motions of the temporomandibular joint (TMJ) involve both translation and rotation; however, there may be substantial variations from one human to another, and these variations present significant difficulties when designing TMJ prostheses. The disc-condyle glides along the temporal bone and the condyle centre describe a curve that depends on the individual morphology. This study analyses disc-condyle rotatory and translatory displacements moving all along the temporal bone facets which are mainly composed of two areas: the articular tubercle slope (ATS) and the preglenoid plane separated by t

Natural coral skeleton used as onlay graft for contour augmentation of the face. A preliminary report.

The skeletons of two different genera of Madreporaria were used as onlay graft material for contour augmentation of the face. With coral blocks or granules in a subperiosteal fashion, patients with hemifacial microsomia, maxillary hypoplasia, microgenia, and Treacher Collins' syndrome have been adequately treated. Sixteen augmentations were performed in 14 patients, followed up for 6-34 months. One graft had to be removed because its inadequate fixation resulted in mobility. Delayed healing was observed in another case for the same reason. Coral skeleton is a cheap, natural biomaterial with ex

The modular endoprosthesis for mandibular body replacement. Part 1: mechanical testing of the reconstruction.

INTRODUCTION: In this paper we present the results of the mechanical testing of a new generation modular endoprosthesis, which has been designed to improve the results of mandibular reconstruction. MATERIALS AND METHODS: The new cementless endoprosthesis consists of a male part, a female part (both with screws on the stems), connected via a dove-tailed connection and secured with a coronal screw. The endoprosthesis was fitted into standardized blocks of synthetic bone (Synbone AG, Malans, Switzerland). The set-up was fixed to an ElectroPuls testing machine at one end and loaded at the other e

Use of magnetic abutments for short endosseous implants following a fibula bone graft in an oral cancer patient: a case report.

Dental implant treatment is an indispensable portion of oral rehabilitation in patients who are to undergo reconstructive surgery following the removal of an oral cancerous lesion. However, ideal dental implant treatment cannot be achieved easily in patients who have undergone mandibular reconstruction with a free vascularized flap, usually because of the limited length, height, and width of the bone graft. Shorter implants (< 10 mm in length) therefore are occasionally used in vascularized fibula graft sites. In such cases, however, shorter implants tend to be overloaded, thereby endangering

Comparing the importance of implant customization vs. usage of a fibular graft using a novel mandibular implant for improved reconstruction: a finite element study.

This study proposes and analyses a novel standard conformal implant (SCI) design, combining the benefits of standard implants (SI) and custom implants (CI) for mandibular defect reconstruction. Using finite element analysis, the performance of all three implant types was compared during unilateral clenching tasks, both with and without fibular graft. The analysis focused on stress and strain distribution in implants and fixation screws. Without fibular graft, CI exhibited lower maximum stress compared to SI and SCI. Reconstruction with fibular graft significantly reduced stress and strain valu

A novel modification of a bone repositioning device and a new technique for reestablishing facial contours after mandibular resection surgery.

A novel modification of a bone repositioning device previously published by the same authors is introduced. A flexible tube to define the intersegmental bony relationship is filled with light-cured resin. It solidifies following exposure to strong visible light for about 1 min. This technique can be used for bone positioning after mandibular resection surgery and during positioning of the proximal segment after sagittal split ramus osteotomy. The authors also propose a simple method for determining the contour of the reconstructed mandible to regain the original shape and form. The advantage o

Functional contribution of mandibular advancement to awake upper airway patency in obstructive sleep apnea.

In the narrowed upper airway of patients with obstructive sleep apnea (OSA), a neuromuscular compensatory mechanism augments the activity of the upper airway dilator muscles in defense of upper airway patency, particularly during inspiration. We hypothesized that mechanical enlargement of the upper airway by a mandibular advancement oral appliance would permit a reduction in this neuromuscular compensation during wakefulness. To test this hypothesis, we focused on changes in the cross-sectional (CS) area of the upper airway before and after emplacement of a ventrally titrated oral appliance in

Chin augmentation with porous hydroxyapatite blocks.

The results of chin augmentation by porous hydroxyapatite blocks were evaluated clinically and radiographically in nine patients. Recovery of a satisfactory facial profile was achieved by the procedure, and all patients but one, with removal of the implant because of infection, were satisfied with the results of the surgery. Radiographically, the implants were incorporated into the bony structure of the mandible by deposition of bone at and around the bone-implant interface and ingrowth of bone into pores. Cephalometrically, the procedure was found to provide very stable results, with little c

Role of interproximal contacts in implant-supported crown stability: A finite element analysis study.

STATEMENT OF PROBLEM: Screw loosening remains a frequent complication with implant-supported prostheses. While clinical evidence suggests that posterior single implant crowns present a high rate of complications, whether the absence of interproximal contacts with adjacent natural teeth is a contributing factor to these complications remains unclear. PURPOSE: The purpose of this finite element analysis study was to evaluate the biomechanical behavior of single implant-supported crowns placed in the posterior mandible. Two variables were investigated: the impact of implant position (first molar

Effect of connector size and design on the fracture resistance of monolithic zirconia fixed dental prosthesis.

Background. Designing a high strength all-ceramic fixed partial denture with favorable esthetics can be challenging for clinicians; this study aimed to evaluate the effect of connector size and design on the fracture resistance of monolithic zirconia fixed dental prostheses. Methods. Two groups of twenty 3-unit monolithic zirconia (Sirona inCoris TZI, Sirona Dental Systems GmbH) bridges, extending from the mandibular first premolar to the first molar with different connector sizes (9 mm2 and 12 mm2), were divided into two subgroups with different connector designs (round and sharp). Neurosensory issues after lateralisation of the inferior alveolar nerve and simultaneous placement of osseointegrated implants.

Our aim was to evaluate neurosensory symptoms after lateralisation of the inferior alveolar nerve (IAN). We studied a retrospective case series with one-year follow up that included 139 procedures in 123 patients. After the IAN had been located it was deflected from the mandibular body and the implant placed. Sensitivity was mapped 24hours, one month, six months, and one year after the intervention by gently pressing the skin and lips with the tip of a probe. A total of 337 implants were placed in 123 patients aged between 44 and 68 years. There were 33 men and 90 women and they all recovered.
