Combined chemo-radiotherapy in locally advanced nasopharyngeal carcinomas.

AIM: To provide efficacy and safety data about the combined use of radiotherapy and chemo-radiotherapy in nasopharyngeal carcinoma (NPC). METHODS: We reviewed data of 40 patients with locally advanced NPC treated with induction chemotherapy followed by concomitant chemo-radiotherapy (CCRT) (22/40 patients) or CCRT alone (18/40) from March 2006 to March 2012. Patients underwent fiberoscopy with biopsy of the primitive tumor, and computed tomography scan of head, neck, chest and abdomen with and without contrast. Cisplatin was used both as induction and as concomitant chemotherapy, while 3D con

Whole-organ histopathological study of recurrent nasopharyngeal carcinoma.

OBJECTIVES/HYPOTHESIS: To study the histopathological features of recurrent nasopharyngeal carcinoma and the accuracy of preoperative magnetic resonance imaging (MRI). STUDY DESIGN: Prospective. METHODS: Whole-organ study of nasopharyngectomy specimens. RESULTS: Between 2006 and 2009, 50 specimens were obtained after maxillary swing nasopharyngectomy and sent for whole-organ study. The tumors arose from the fossa of Rosenmüller (68%), posterior wall (18%), or roof of the nasopharynx (14%), and the majority (72%) took the form of an ulcerative tumor. The T-classifications of the recurrent tu

Molecular pathogenesis of juvenile nasopharyngeal angiofibroma in brazilian patients.

Juvenile nasopharyngeal angiofibroma (JNA) is a vascular tumor of the nasopharynx that accounts for 0.5% of all cancers of the head and neck. It primarily affects males aged 14-25 years. Of the many genes that mediate the development of JNA, GSTM1 has been most frequently associated with this vascular tumor. The loss of expression of GSTM1 (null genotype) is linked to the development of these tumors. The aim of this cross-sectional case study was to examine the prevalence of the GSTM1-null genotype in Brazilian patients with JNA. DNA was extracted from the leukocytes of blood samples from 10 p

Nasopharyngeal mucoepidermoid carcinoma - a common entity at an uncommon location.

Mucoepidermoid carcinomas mostly occur in the major salivary glands, the minor salivary glands of oral cavity and in the lacrimal glands. These tumours rarely occur in the sino-nasal tract. When they occur in the sino-nasal tract, the most frequent site is the maxillary antrum, followed by the nasal cavity, the nasopharynx and the ethmoidal sinuses. As per review of literature, nasopharyngeal mucoepidermoid carcinomas account for 0.6% of salivary gland tumours and 4.8% of mucoepidermoid carcinomas. Extensive literature search revealed 21 cases of nasopharyngeal mucoepidermoid carcinomas report

Developing an adaptive radiation therapy strategy for nasopharyngeal carcinoma.

Adaptive radiotherapy (ART) has recently been introduced to restore the planned dose distribution by accounting for the anatomic changes during treatment. By quantifying the anatomic changes in nasopharyngeal carcinoma (NPC) patients, this study aimed to establish an ART strategy for NPC cases. A total of 30 NPC patients treated with helical tomotherapy were recruited. In the pretreatment megavoltage CT images, the anatomic changes of the posterolateral wall of nasopharynx (P-NP), neck region and parotid glands were measured and assessed. One-way repeated measure ANOVA was employed to define t

Characterization of TRIP6-dependent nasopharyngeal cancer cell migration.

Nasopharyngeal carcinoma (NPC) is a leading malignancy most often reported in endemic areas such as in Southeast Asia and the Mediterranean area. NPC remains as a major challenge for clinical management largely due to its high propensity for cancer invasion, metastasis, and recurrence. Therefore, control of NPC cell motility stands as a major obstacle for successful NPC management. The current study sought to identify a new regulator for NPC cell motility in light of previous data showing a similar role of thyroid receptor interactor protein 6 (TRIP6) in other cancer cell types. Results showed

Influence of gender and age on the survival of patients with nasopharyngeal carcinoma.

BACKGROUND: The prognostic value of gender and age in the survival of nasopharyngeal carcinoma (NPC) patients treated with intensity-modulated radiotherapy (IMRT) is unclear. Several studies have suggested a female advantage in the prognosis of solid tumors. We investigated the relationship between gender differences and disease outcome in NPC patients treated with IMRT in South China. METHODS: A total of 299 patients with non-disseminated NPC were analyzed retrospectively. IMRT was delivered with a simultaneous modulated, accelerated radiotherapy boost technique at prescribed doses of 70 Gy/

[Clinical aspects of brain metastases from nasopharyngeal carcinoma].

Metastasis to the central nervous system, either through a hematogenous route or through the cerebrospinal fluid, is extremely rare in nasopharynx cancer. We aim to expose clinical aspects, therapeutic features and prognosis of nasopharyngeal carcinoma with brain metastases. We retrospectively reviewed the medical history of about 420 patients with nasopharyngeal carcinoma treated during 17 years at the university hospital of Sfax (Tunisia). Among them, three patients had brain metastasis. We excluded patients with direct extension to the brain. Tumours of the nasopharynx were locally advanced

Submandibular mass as a rare presentation of advanced nasopharyngeal carcinoma.

Cervical lymphadenopathy is a common presentation of nasopharyngeal carcinoma (NPC). It can be present in the classical location of the level II or VI lymph nodes or at any other levels of the lymph nodes in the neck. NPC should be suspected when a patient presents with cervical lymphadenopathy and conductive hearing loss. A thorough endoscopic assessment of the nasopharynx should be performed to rule out this radiotherapy-curable disease at its early stage.

An unusual case of Waldenstrom's macroglobulinemia presented with nasopharyngeal involvement.

We report a rare case of a 68-year-old male who presented with fever, weight loss, nasal blockage, and epistaxis. Examination revealed cervical and axillary lymphadenopathy with no evidence of organomegaly. On evaluation, bone marrow aspiration showed lymphoplasmacytic infiltration. The computed tomography of the neck showed nasopharyngeal mass and the biopsy of this mass and cervical lymph node showed lymphoplasmacytic lymphoma (LPL) with high serum IgM level. Hence, a diagnosis of Waldenstrom's macroglobulinemia (WM) was made. The patient received six cycles of chemotherapy with a combinatio

Nasopharyngeal bacterial colonisation in HIV-positive children in Cambodia.

OBJECTIVE: To determine patterns of nasopharyngeal colonisation in HIV-positive children. METHODS: Nasopharyngeal, nasal and ear swabs were prospectively taken from all children living in two paediatric nursing homes for HIV-positive orphans in Cambodia from 2004 to 2011. RESULTS: A total of 882 swabs were taken, of which 586 tested positive for bacteria. Staphylococcus aureus was the most frequently isolated species (178 isolates; 30.4%) followed by Streptococcus pneumoniae (103 isolates; 17.6%) and Klebsiella pneumoniae (99 isolates; 16.9%). The rate of S. pneumoniae decreased in 2009 when

Nasopharyngeal detection of respiratory viruses in febrile neutropenic children.

BACKGROUND: Although fever is common in neutropenic children, a microbiological etiology is determined in only 15% to 30% of cases. The authors investigated the frequency of respiratory virus detection in the upper airways of febrile neutropenic children with negative bacterial cultures. METHODS: This is a 3-year prospective study of children younger than 19 years, hospitalized with febrile neutropenia and negative bacterial cultures. Respiratory samples were obtained for amplification of viral nucleic acids via Luminex xTAG technology. RESULTS: There were 50 febrile neutropenic episodes amo

Nasopharyngeal branchial cysts-diagnosis and management: a case series.

Nasopharyngeal branchial cysts (NBCs) have been discussed in the literature in only a limited number of publications. Differing from Tornwaldt cysts, NBCs present laterally and arise from the fossa of Rosenmuller and may track superiorly within the bony confines of the Eustachian tube. Initially patients are asymptomatic but may present with aural fullness, unilateral conductive hearing loss, and serous otitis media as the cyst mass grows. Two of our three patients had the lesion incidentally identified at the time of assessment for another diagnosis. In this case series, imaging characteristi

A retrospective study of salvage surgery for recurrent nasopharyngeal carcinoma.

BACKGROUND: Nasopharyngeal carcinoma is sensitive to radiotherapy. When there is local relapse, re-irradiation treatment is inevitably associated with serious complications and decreased quality of life. Surgical resection offers an alternative treatment option with acceptable morbidity. METHODS: Seventy-one consecutive patients with primary recurrence of nasopharyngeal carcinoma after radiation underwent nasopharyngectomy from January 1, 1990 to June 30, 2006. Follow-up ranged from 12 to 127 months. RESULTS: The actuarial 1-, 2-, 3-, and 5-year survival rates were 88.1, 62.1, 48.9, and 42.1

Nasopharyngeal inflammatory pseudotumor showing abducens nerve palsy.

Inflammatory pseudotumor (IPT) originated from the nasopharynx is very rare. To the authors' best knowledge, nasopharyngeal pseudotumor accompanied by abducens nerve palsy due to cavernous sinus invasion has not been reported previously. A 28-year-old male presented with abrupt-onset diplopia and right side severe headache. Physical examination revealed a mild contour bulging mass without mucosal ulceration or necrosis in the nasopharynx. The mass was thought to be a malignant neoplasm such as a carcinoma due to ill-defined, infiltrative tumor extended to the right parapharyngeal, carotid, pre

Analysis of nasopharyngeal carcinoma risk factors with Bayesian networks.

OBJECTIVES: We propose a new graphical framework for extracting the relevant dietary, social and environmental risk factors that are associated with an increased risk of nasopharyngeal carcinoma (NPC) on a case-control epidemiologic study that consists of 1289 subjects and 150 risk factors. METHODS: This framework builds on the use of Bayesian networks (BNs) for representing statistical dependencies between the random variables. We discuss a novel constraint-based procedure, called Hybrid Parents and Children (HPC), that builds recursively a local graph that includes all the relevant features

Nasopharyngeal carcinoma segmentation using a region growing technique.

PURPOSE: This paper proposes a new image segmentation technique for identifying nasopharyngeal tumor regions in CT images. The technique is modified from the seeded region growing (SRG) approach that is simple but sensitive to image intensity of the initial seed. METHODS: CT images of patients with nasopharyngeal carcinoma (NPC) were collected from Ramathibodi hospital, Thailand. Tumor regions in the images were separately drawn by three experienced radiologists. The images are used as standard ground truth for performance evaluation. From the ground truth images, common sites of nasopharynge
