Laterocaudal approach to the inferior rim of the orbit.

PURPOSE: Fractures of the orbital floor and orbital rim are frequently treated in maxillofacial departments. Various approaches to the orbital floor are described in the literature. We present an investigation of a modified subciliary approach (laterocaudal approach) with respect to its feasibility and clinical outcome. MATERIALS AND METHODS: Our retrospective study investigated 130 well-documented cases of patients with orbital floor fractures who received surgical reconstruction using a laterocaudal approach, and compared the results with the current literature, with a focus on subciliary a…

Autologous fat graft in irradiated orbit postenucleation for retinoblastoma.

Autologous fat grafting has been extensively and successfully adopted in a number of pathologic conditions in regenerative surgery especially on irradiated fields in order to improve pain symptoms and tissue trophism promoting scar release. In the present study, we report our experience with autologous fat grafting for the treatment of postirradiation fibrosis and pain on three consecutive patients undergoing orbital enucleation for locally advanced retinoblastoma (RB) and subsequent radiotherapy. We selected three consecutive patients who underwent orbital enucleation for locally advanced RB …

Congenital Arteriovenous Malformation of the Scalp Involving the Orbit.

BACKGROUND: Arteriovenous malformations (AVMs) of the scalp are rare and infrequently encountered by the neurosurgeon. CASE DESCRIPTION: We report a unique case of a 42-year-old patient who presented with a progressive worsening of visual acuity in the right eye (lower quadrantanopia) and palpebral ptosis. Physical examination revealed a right exophthalmos and a right frontoparietal scalp soft swelling when the patient was in the supine position. Neurologic work-up showed a scalp AVM extending into the orbit and connected to an intraorbital cavernous angioma. The patient was treated with a fr…

Orbit, zygoma, and maxilla growth patterns in Crouzon syndrome.

The facial malformations of Crouzon syndrome involve the entire cranio-orbito-zygomatic region. The detailed sequence of changes in orbit, zygoma, and maxilla over time, the mutual influence among these three anatomical structures, and their relationship with the cranial base were studied to determine the sequence and timing of deformity. Preoperative CT scans of 36 patients with Crouzon syndrome (mean age 10.84±14.70years; 14 male, 22 female) and CT scans of 54 control subjects (mean age 8.53±13.22years; 29 male, 25 female) were divided into five subgroups by age: 0-6 months, 6 months-2 years…

Nodular Colloid Milium of the Conjunctiva and Anterior Orbit.

Colloid milium is a rare deposition disorder with accumulation of a characteristic amorphous substance in both protected and sun-exposed are … lity, and a Congo red stain was negative. Based on histopathologic findings, a diagnosis of nodular orbital colloid milium was made. To our knowledge, the adult and nodular forms of colloid milium have not been previously described as occurring in the conjunctiva or anterior orbit.

Inflammatory myofibroblastic tumor of the orbit: a case report.

Inflammatory myofibroblastic tumor (IMT) is a neoplasm most commonly found in the abdominal-pelvic region, lung, and retroperitoneum. The tumor tends to affect soft tissues of children and young adults and can locally recur but rarely metastasizes. Histologically, the appearance is one of bland spindle cell proliferation with a prominent, chronic inflammatory infiltrate. This article describes 1 case of IMT found in the orbit that is presented with rapidly progressive painless proptosis. In the authors' review of the literature, they have only found 2 other case reports involving the orbit.

Management of the 'tight orbit' and associated visual loss.

PURPOSE OF REVIEW: To review the most recent literature on the management of the orbit under high pressure (the tight orbit) with threatened visual loss. RECENT FINDINGS: Definitive management of traumatic optic neuropathy remains unclear, lacking randomized head-to-head treatment option trials. Instead, numerous case series and anecdotal reports show benefit in each of the four treatment modalities currently in use, but a multitude of variables confound cross-study comparison. In contrast, the management of orbital hemorrhage and orbital emphysema is well known. A number of immunomodulatory …

Mucus extravasation into the orbit during frontal sinus irrigation.

During minitrephination and irrigation of the frontal sinus, mucus extravasated into the orbit through a defect in the sinus floor. The mucus incited a foreign body reaction and became encapsulated within the orbit necessitating excision via an anterior orbitotomy.

Neuroendocrine tumor metastatic to the orbit treated with radiotherapy.

Neuroendocrine tumors are rare neoplasms that infrequently metastasize to the orbit. Given that patients with these tumors may have prolonged survival despite dissemination, maintaining quality of life by providing early diagnosis and effective treatment to preserve vision and comfort is a fundamental issue. We report the case of a 79-year old woman who presented with well-differentiated metastatic neuroendocrine tumor to the liver with no carcinoid syndrome and was started on intramuscular long-acting octreotide with disease stabilization. Two years later she developed right-sided diplopia as…

A 55-mm object inside a 40-mm orbit.

A 43-year-old man was struck in the left orbit with his board while surfing and sustained a 3-cm laceration above his left eyebrow. The laceration was sutured closed primarily at a local emergency room. When he presented to UCSD oculoplastics for suture removal, he complained of diplopia with upgaze and was found to have hypoglobus on clinical exam. An orbital CT demonstrated a hyperintense linear signal within the orbit. The patient underwent surgical exploration. A 55-mm-length piece of fiberglass from the patient's surfboard was removed from his orbit. This case demonstrates the importance …

Biomechanical modeling of eye trauma for different orbit anthropometries.

In military, automotive, and sporting safety, there is concern over eye protection and the effects of facial anthropometry differences on risk of eye injury. The objective of this study is to investigate differences in orbital geometry and analyze their effect on eye impact injury. Clinical measurements of the orbital aperture, brow protrusion angle, eye protrusion, and the eye location within the orbit were used to develop a matrix of simulations. A finite element (FE) model of the orbit was developed from a computed tomography (CT) scan of an average male and transformed to model 27 differen…

Solitary fibrous tumour of the orbit: an unusual presentation.

Solitary fibrous tumour is a rare tumour that usually arises from the pleura and very uncommonly from the orbit. This unusual unilateral orbital mass can be differentiated from the more common haemangiopericytoma by the use of histopathological and immunohistochemical studies. The tumour is strongly Immunoreactive for CD34, which supports its diagnosis.

A Rare Case of Myoepithelioma around the Left Orbit.

Myoepithelioma is a rare neoplasm of the salivary glands, generally occurring in the parotid gland and less often in the mi-nor accessory salivary glands of the oral cavity. It is known to be a rare entity occurring at unusual locations, which makes it difficult to diagnose. Such a rare case of myoepithelioma in salivary glands present at an unusual location around the left orbit is presented here.

Solitary fibrous tumor of the orbit: a case series.

PURPOSE: Solitary fibrous tumor (SFT) occurs rarely in the orbit. We present four such cases, representing one of the largest case series reported to date of this rare orbital tumor. METHOD: The four patients ranged from ages 20 to 50, all of whom presented with lid swelling or bulging in the involved eye. All four patients underwent CT scan to confirm the diagnosis of an orbital mass, which was then excised. RESULTS: The tumors of all four specimens contained spindle-shaped cells with bland nuclei and rare mitotic figures. They all stained positively with CD34 and vimentin. Resection margin…
