A tissue-engineered conduit for peripheral nerve repair.

BACKGROUND: Peripheral nerve repair using autograft material has several shortcomings, including donor site morbidity, inadequate return of function, and aberrant regeneration. Recently, peripheral nerve research has focused on the generation of synthetic nerve guidance conduits that might overcome these phenomena to improve regeneration. In our laboratory, we use the unique chemical and physical properties of synthetic polymers in conjunction with the biological properties of Schwann cells to create a superior prosthesis for the repair of multiply branched peripheral nerves, such as the facia

A comparative study of flexible (Polyactive) versus rigid (hydroxylapatite) permucosal dental implants. I. Clinical aspects.

Recently, an elastomeric polyethyleneoxide polybutylene-terephthalate (PEO:PBT) copolymer (Polyactive) was introduced, which exhibits bone-bonding characteristics. In contrast to ceramics, bioglass, titanium and other metals, PEO:PBT copolymers are flexible materials that could reduce undesirable peak stresses along the neck of a permucosal implant. The application of three types of Polyactive permucosal dental implants (one dense and two porous types) and one dense hydroxylapatite (HA) implant were clinically tested and compared during 30 weeks of loading in a dog experiment. With respect to

Initial experience with the Cosgrove-Edwards Annuloplasty System.

BACKGROUND: The mitral valve has a nonplanar shape and a sphincter action. Pathologic dilatation occurs along the posterior annulus. To preserve the physiologic function and correct annular dilatation, we developed an annuloplasty system that is universally flexible and produces a measured plication of the posterior annulus (Cosgrove-Edwards Annuloplasty System). METHODS: The results of 150 consecutive mitral valve repairs using this system were analyzed. Mean age was 58 +/- 13 years; 59% were men. The cause of the valve disease was degenerative in 75% of the patients, rheumatic in 13%, ische

Functional Outcomes and Quality of Life Following Supraclavicular Flap Reconstruction in Oral Cancer Surgery: A Retrospective Analysis.

Background and objective Oral cavity malignancies pose considerable challenges in surgical management, making optimal functional and aesthetic reconstruction crucial after tumor ablation. The supraclavicular flap (SCF) has gained popularity in head and neck reconstruction due to its reliable vascularity, adequate surface area, pliability, and low donor site morbidity. While evidence supporting its use in oral reconstruction is growing, systematic evaluation of functional outcomes and quality of life remains limited. This study aims to assess functional outcomes and quality of life following SC

Clinical considerations in case selection for all-ceramic fixed partial dentures.

All-ceramic systems have been utilized for fixed partial dentures (FPDs) to replace a missing tooth since the early 1990s. Clinical studies that assess the long-term outcome of both anterior and posterior three-unit all-ceramic FPDs are required to determine whether they can serve as a viable treatment option for functional and aesthetic tooth replacement. This article discusses the evolution and development of all-ceramic systems for FPDs as well as the clinical and laboratory considerations for treating patients using these restorations.

Clinical application of a new bioresorbable guided tissue regeneration device: case reports.

Bioresorbable barriers have been recently introduced in clinical practice for guided tissue regeneration therapy. One of these is the Guidor matrix barrier which is made of amorphous polylactic acid softened with a citric acid ester to increase malleability and facilitate clinical handling. The advantages of the bioresorbable barrier include: the elimination of second surgery; better handling and adaptation around the tooth and over the bone; and integration of the connective tissue of the flap with the barrier preventing epithelial migration, gingival recession, and pocket formation. In the c

Bond strengths of resin-to-metal bonding systems.

STATEMENT OF PROBLEM: Resinous materials for the veneers of fixed prostheses commonly require mechanical retention on metal substructures because there is no chemical adhesion. However, mechanical retention does not restrict creation of a marginal gap at the resin-metal interface, which can cause discoloration or detachment of resinous material. The development of a chemical resin-to-metal bonding could resolve this problem and also reduce the need for mechanical retention (pearls, wires) on metal frameworks. PURPOSE: This study evaluated six current methods with the use of various storage co

Use of tape for the management of hypertrophic scar development: A comprehensive review.

INTRODUCTION: Tapes have been used to aid fresh wound closure. For hypertrophic scars, the use of tapes as a therapy to reduce the mechanical forces that stimulate excessive and long-term scarring is yet to be evaluated. The aim of this comprehensive review was to explore the current clinical application of tapes, as a minimally invasive option, as purposed specifically for the management of hypertrophic scarring, regardless of scar causation. METHOD: Databases were searched using MeSH terms including one identifier for hypertrophic scar and one for the intervention of taping. Studies include

Multiscale analyses of the bone-implant interface.

Implants placed with high insertion torque (IT) typically exhibit primary stability, which enables early loading. Whether high IT has a negative impact on peri-implant bone health, however, remains to be determined. The purpose of this study was to ascertain how peri-implant bone responds to strains and stresses created when implants are placed with low and high IT. Titanium micro-implants were inserted into murine femurs with low and high IT using torque values that were scaled to approximate those used to place clinically sized implants. Torque created in peri-implant tissues a distribution

Phenotypic effects of Ehlers-Danlos syndrome-associated mutation on the FnIII domain of tenascin-X.

Tenascin-X (TNX) is an extracellular matrix (ECM) protein and interacts with a wide variety of molecules in the ECM as well as on the membrane. Deficiency of TNX causes a recessive form of Ehlers-Danlos syndrome (EDS) characterized by hyperelastic and fragile skin, easy bruising, and hypermobile joints. Three point mutations in TNX gene were found to be associated with hypermobility type EDS and one of such mutations is the V1195M mutation at the 7th fibronectin Type III domain (TNXfn7). To help elucidate the underlying molecular mechanism connecting this mutation to EDS, here we combined homo

Tricuspid valve annuloplasty with a flexible prosthetic band.

This study evaluates the application to the tricuspid valve of a flexible prosthetic band originally devised for mitral repair. Between March 2001 and May 2005, 53 consecutive patients (age 66.2+/-8.5 years) with significant tricuspid regurgitation and dilatation of the right-sided cardiac chambers underwent tricuspid valve annuloplasty with the band and concomitant mitral repair or replacement. Thirty-one patients (58.5%) were in NYHA class III or IV, and 33 (62.3%) had a history of right heart failure. Follow-up was 19.2+/-14.0 months. Three patients (5.7%) died before discharge, and one dur

Fiberoptic intubation in 327 neurosurgical patients with lesions of the cervical spine.

In patients with lesions of the cervical spine, direct laryngoscopy for endotracheal intubation entails the risk of injuring the spinal cord. In an attempt to avoid this complication, the authors used flexible fiberoptic nasal intubation in a series of 327 patients with cervical lesions undergoing elective neurosurgical procedures. The nasal route was preferred for laryngeal intubation because it is easier than the oral route and a restraining collar or halo device does not impair the intubating maneuver. Bronchoscopic intubation was possible in all patients. In 12 patients (3.6%), anatomic ab

Investigation of pleural effusion: an evaluation of the new Olympus LTF semiflexible thoracofiberscope and comparison with Abram's needle biopsy.

STUDY OBJECTIVES: Recently, pulmonologists have performed thoracoscopy under local anesthesia using rigid thoracoscopes or flexible bronchoscopes. The latter allow greater access within the pleural cavity but are difficult to manipulate. The Olympus LTF semiflexible fiberoptic thoracoscope combines features of both instruments, having a solid body and a flexible terminal section. In the first study with this instrument, we evaluated ease of use and compared diagnostic yield with closed needle biopsy. PATIENTS: Twenty-four patients with pleural effusion were investigated.
