Cure for psoriasis - a prospective outlook.

Once considered incurable, psoriasis is now being re-evaluated in light of sustained, drug-free remissions, prompting a redefinition of 'cure' as long-term/drug-free remission. Psoriatic disease is primarily driven by dysregulation of the interleukin-23 (IL-23)/IL-17 axis, and the survival of tissue-resident memory T cells (TRM), key mediators of lesion recurrence and chronicity. Targeting this axis early with high-dose IL-23 inhibitors has shown promise, as exemplified by the KNOCKOUT trial, where intensified induction dosing of risankizumab resulted in marked TRM cell depletion and durable r…

A Case of Strabismus in a Patient With Psoriasis.

Psoriasis is a chronic immune-mediated skin disease that may be associated with ocular manifestations, most commonly uveitis, keratoconjunctivitis, and blepharitis. Strabismus has been rarely reported in association with psoriasis. We present a woman with longstanding psoriasis, who developed acquired intermittent alternating esotropia with diplopia; she was later diagnosed with psoriatic arthritis. This case describes an uncommon ocular finding occurring with psoriasis and underscores the importance of coordinated, multidisciplinary care when new ocular or systemic symptoms arise.

Gender perspective in the management of psoriasis.

Gender medicine has been achieving increasing importance. Gender differences in disease depend on hormonal status and may involve functions of the skin, immune responses and metabolic pathways, and have to do also with indications and response to treatments. Psoriasis is a common chronic inflammatory immune-mediated disease. The prevalence of psoriasis in the population is balanced between males and females, but early onset psoriasis is slightly more prevalent in males, with the latter suffering from a more severe disease. In general, male and female patients receive identical drugs at equival…

The Perspective of Cannabidiol in Psoriasis Therapy.

Psoriasis is a chronic skin condition that can significantly impact the quality of life of those affected. As an autoimmune disease, it can lead to itchy, painful, and scaly patches on the skin. Although various treatments, including topical creams, phototherapy, and systemic medications, are currently available, they may not always offer effective relief and can have side effects. Researchers have thus been exploring the potential benefits of non-psychoactive compounds such as CBD, found in Cannabis sativa plants, for treating psoriasis. CBD treatment may reduce inflammation, oxidative stress…

Targeting the NLRP3 inflammasome in psoriasis.

The NLRP3 inflammasome, a complex consisting of the nucleotide-binding oligomerization domain, leucine-rich repeat, and pyrin domain-containing protein 3, has emerged as a critical mediator of pathological inflammation and a significant therapeutic target for various inflammatory diseases. Psoriasis, a chronic inflammatory skin condition without a definitive cure, has shown promising results in animal models through the inhibition of the NLRP3 inflammasome. This review aims to explore the development of the NLRP3 inflammasome in psoriasis and the molecular mechanisms responsible for its inhibi…

The link between autophagy and psoriasis.

Autophagy is a lysosome-dependent, self-renewal mechanism that degrades and recycles cellular components in eukaryotic cells to maintain the homeostasis of the intracellular environment. Psoriasis is featured by increased inflammatory response, epidermal hyperproliferation and abnormal differentiation, infiltration of immune cells and increased expression levels of both endothelial adhesion molecules and angiogenic mediators. Evidence indicates that autophagy has important roles in many different types of cells, such as lymphocytes, keratinocytes, monocytes and mesenchymal stem cells (MSCs). T…

A Rare Presentation of Erythrodermic Psoriasis.

Erythrodermic psoriasis (EP) is a rare and challenging-to-diagnose severe form of psoriasis. Its presentation can have similarities with other inflammatory skin conditions, complicating subsequent management. We present a case of a 76-year-old woman with EP who presented with fever, tachycardia, leg swelling with pain and redness, and reduced consciousness. Her imaging revealed characteristic features of the disease, providing valuable educational insights. We aim to emphasize the importance of maintaining a high index of suspicion for EP and promptly adjusting treatment.

Platelet Reactivity in the Exacerbation of Psoriasis.

Background: Psoriasis is a chronic, inflammatory, immune-mediated disease with a specific cutaneous presentation. Increased platelet aggregation has been observed in patients with extensive psoriatic lesions. The aim of this study was to evaluate the clinical factors affecting platelet reactivity in patients with an exacerbation of psoriasis. Methods: This was a prospective, single-center, observational study, enrolling patients hospitalized for an aggravation of psoriasis. Enrolled patients underwent single platelet function testing with light transmission aggregometry on the first morning of…

ADVANCES IN THE GENETICS AND IMMUNOLOGY OF PSORIASIS.

Psoriasis is a chronic, immune-mediated skin condition characterised by keratinocyte hyperproliferation and T cell activation. Prior research has revealed environmental and genetic associations that have contributed to the understanding of psoriasis pathophysiology. Further, this research has contributed to advancements in diagnostics, risk stratification, and therapeutics that impact patient care and health outcomes in psoriasis-associated comorbid conditions such as psoriatic arthritis, metabolic syndrome, and hypertension. Here we review recent advances in psoriasis genetics and immunology.

Biometrics and Biomarkers in Patients With Psoriasis.

Psoriasis (PsO) is a chronic, systemic, and autoimmune dermatologic condition characterized by dry, scaly, and erythematous plaques on the skin. PsO can present in various forms, including guttate (small, round lesions commonly over the upper trunk and extremities that can be raised and scaly), inverse (smooth plaques of inflamed skin within skin folds of the groin, buttock, and breasts), pustular (white painful pustules within red inflamed blotches widespread over the body), and erythrodermic (red rash present over most of the body). Individuals with PsO can present differently, with unique s…

The study of psoriasis has yielded fundamental new insights into immunologic regulation and innovative therapies in a way that few other diseases have. In this review, we summarize the main features of current psoriasis research with emphasis on pathophysiological processes and the milestones in the approval of various biologics and small molecule drugs. Thus, through psoriasis research, we are gaining a better understanding of the interplay between the components of the innate and adaptive immune systems. New therapeutics interfere with crucial regulatory networks. Based on current knowledge,…

The Role of Adipokines in the Pathogenesis of Psoriasis.

Psoriasis is a chronic and immune-mediated skin condition characterized by pro-inflammatory cytokines and keratinocyte hyperproliferation. Dendritic cells, T lymphocytes, and keratinocytes represent the main cell subtypes involved in the pathogenesis of psoriasis, while the interleukin-23 (IL-23)/IL-17 pathway enhances the disease progression. Human adipose tissue is an endocrine organ, which secretes multiple proteins, known as adipokines, such as adiponectin, leptin, visfatin, or resistin. Current evidence highlights the immunomodulatory roles of adipokines, which may contribute to the progr…

The role of microRNA in psoriasis: A review.

Psoriasis is a chronic immune-mediated inflammatory skin disease that involves a complex interplay between infiltrated immune cells and keratinocytes. Great progress has been made in the research on the molecular mechanism of coding and non-coding genes, which has helped in clinical treatment. However, our understanding of this complex disease is far from clear. MicroRNAs (miRNAs) are small non-coding RNA molecules that are involved in post-transcriptional regulation, characterised by their role in mediating gene silencing. Recent studies on miRNAs have revealed their important role in the pat…
