[Decision-making process between plate osteosynthesis vs. endoprosthesis in proximal humerus fractures of the rheumatoid arthritis shoulder].

The shoulder joint is involved in a high percentage of rheumatoid arthritis (RA) cases. At the same time, patients with RA develop osteoporosis at an early stage, which significantly increases the risk of fracture. In recent years, joint-preserving procedures with stable-angle implants have become available for individuals with low disease activity and good medication control. However, this approach is dependent on the fracture morphology and, if not adequately addressed, is associated with a high rate of revision surgery. In rotator cuff reconstruction, recent data show encouraging results re

Anthropometrical study of the human glenoid in a normal Spanish population.

OBJECTIVE: To evaluate the size of the glenoid in a southern Spanish population, to compare it with previous results from other populations and determine the size of the implants that are marketed for shoulder arthroplasty. MATERIAL AND METHODS: Between January 2015 and December 2017, an anthropometrical study of the human glenoid was performed using computed axial tomography scans (CT) of 154 patients over 30 years old. The glenoid dimensions were analysed 3-dimensionally using 2mm interval thicknesses, determining the average height and width of the glenoid. The upper point of the glenoid g

Subscapularis management in stemless total shoulder arthroplasty: tenotomy versus peel versus lesser tuberosity osteotomy.

BACKGROUND: It is unknown whether subscapularis management technique has an influence on the outcomes and complications of stemless total shoulder arthroplasty. The purpose of this study, therefore, was to compare outcomes and complications between subscapularis tenotomy, peel, and lesser tuberosity osteotomy used during stemless shoulder arthroplasty. METHODS: We reviewed 188 stemless anatomic total shoulder arthroplasties and compared clinical and functional outcomes between those performed through a subscapularis tenotomy (n = 68), subscapularis peel (n = 65), or lesser tuberosity osteotom

Humeral head histopathological changes in cuff tear arthropathy.

PURPOSE: The aim of this study was to investigate the histopathological changes in the humeral head in cuff tear arthropathy (CTA) compared with those in glenohumeral osteoarthritis (OA) and humeral neck fracture, which served as non-cuff tear controls. METHODS: Twenty-three humeral heads extracted at the time of shoulder prosthesis arthroplasty between June 2014 and July 2015 were evaluated in the present study. The diagnoses included four-part humeral neck fracture ( n = 4; average age, 85.0 years), glenohumeral OA ( n = 4; average age, 71.0 years), and CTA ( n = 15; average age, 73.0 years…

Biomechanical Study of a Subscapularis Repair Technique for Total Shoulder Arthroplasty.

Secure subscapularis repair is an essential element of total shoulder arthroplasty. Some surgeons prefer subscapularis peel because of ease of use, but some evidence suggests that lesser tuberosity osteotomy provides better fixation. The authors developed a novel, through-implant repair technique and performed a biomechanical study of its strength with cadaveric specimens. The authors obtained 20 matched pairs of cadaveric shoulders and inserted an uncemented short-stemmed humeral prosthesis that facilitates through-implant repair in all specimens. For each cadaver, the subscapularis was repai

Reverse shoulder arthroplasty with a short metaphyseal humeral stem.

PURPOSE: Reverse shoulder prostheses have been gaining popularity in recent years. A short metaphyseal stem design will allow bone stock preservation and minimize stem related complications. We examined the clinical and radiographic short-term outcome of a short metaphyseal stem reverse shoulder arthroplasty. METHODS: Thirty-one patients, with a mean follow-up of 36 months (24-52), were evaluated clinically with the Constant-Murley score, patient satisfaction and pain relief scores. The fixation of the glenoid and humeral components, subsidence and notching were evaluated on radiographs. The

Use of a caged, bone ingrowth, glenoid implant in anatomic total shoulder arthroplasty technique and early results.

Shoulder arthroplasty represents one of the fastest growing orthopaedic procedures in the USA. In comparison to hemiarthroplasty, total shoulder arthroplasty has higher patient satisfaction and lower revision rates. Aseptic glenoid loosening remains a concern for longevity of total shoulder arthroplasties. Cement fixation on the glenoid side represents the primary and only mode of fixation in the majority of implants in the USA. Metal backed glenoid designs have demonstrated higher failure rates, primarily due to thinning of the polyethylene bearing surface. We reviewed our early experience an

Cement Spacer versus Endoprosthesis as Reconstruction Modality after Limb Salvage for Shoulder Malignancies: Systematic Review and Meta-Analysis.

BACKGROUND: Limb salvage is the mainstay of surgical treatment for primary sarcomas as well as metastatic lesions of the proximal humerus. This involves wide excision of the involved humeral segment followed by a reconstruction of the defect. The purpose of this review was to determine which reconstruction modality offers a better overall functional outcome. METHODS: After registering a review with the PROSPERO database (ID No. CRD 42025642215), PubMed and Cochrane Library databases were systematically screened for relevant studies meeting our inclusion criteria. The Methodological Index for

Factors Affecting Length of Stay, Readmission, and Revision After Shoulder Arthroplasty: A Population-Based Study.

BACKGROUND: Increased length of hospital stay, hospital readmission, and revision surgery are adverse outcomes that increase the cost of elective orthopaedic procedures, such as shoulder arthroplasty. Awareness of the factors related to these adverse outcomes will help surgeons and medical centers design strategies for minimizing their occurrence and for managing their associated costs. METHODS: We analyzed data from the New York Statewide Planning and Research Cooperative System on 17,311 primary shoulder arthroplasties performed from 1998 to 2011 to identify factors associated with extended

Biscapular Protraction Force and Excursion in Adult Patients With Traumatic Brachial Plexus Injuries: A Comparative Study.

PURPOSE: Traditional body-powered upper limb prostheses use a Bowden cable to translate shoulder and residual upper limb movements into forces that control the terminal device. The necessary force and excursion required to use a traditional body-powered prosthesis in patients with traumatic brachial plexus injuries (BPI) has yet to be ascertained. This study compared the force and excursion generated during biscapular protraction in adults with traumatic BPI compared to healthy volunteers. METHODS: Thirty nonamputee volunteers, divided into BPI and uninjured control groups and matched for age

Humeral amputation following total elbow arthroplasty.

PURPOSE: Total elbow arthroplasty (TEA) is associated with a relatively high complication rate, and exceptionally catastrophic complications might lead to amputation. The purpose of this study was to determine the incidence and aetiology of amputation performed at our institution in upper extremity limbs with a prior TEA. METHODS: Between 1973 and 2018, 1906 consecutive TEAs were performed at our institution. Upper extremity amputation was performed in seven (0.36%) elbows with five transhumeral amputations and two shoulder disarticulations. The group consisted of five females and two males w

Bionic reconstruction to restore hand function after brachial plexus injury: a case series of three patients.

BACKGROUND: Brachial plexus injuries can permanently impair hand function, yet present surgical reconstruction provides only poor results. Here, we present for the first time bionic reconstruction; a combined technique of selective nerve and muscle transfers, elective amputation, and prosthetic rehabilitation to regain hand function. METHODS: Between April 2011, and May 2014, three patients with global brachial plexus injury including lower root avulsions underwent bionic reconstruction. Treatment occurred in two stages; first, to identify and create useful electromyographic signals for prost

Long-term outcomes of metallic hemiarthroplasty for degenerative and inflammatory shoulder pathologies; a systematic review of minimum 10-year follow-up.

BACKGROUND: Metallic hemiarthroplasty (HA) remains a valuable treatment option in certain situations for treatment of degenerative and inflammatory arthritic conditions. When glenoid bone stock is preserved or eroded such that total shoulder arthroplasty or reverse shoulder arthroplasty (rTSA) is contraindicated, HA may be a useful surgical treatment for arthritic conditions. Although clinical indications for metallic HA have narrowed due to the utility of rTSA, the long-term outcomes and failure mechanisms of metallic HA have not been synthesized. The purpose of this project is to systematica
