BACKGROUND: Although existing literature supports durations of 5-7 days for skin and soft tissue infections (SSTIs), longer durations are commonly used. Obesity and heart failure (HF) have been associated with increased risk for treatment failure of SSTIs; however, whether prolonged antibiotic durations reduce the risk of treatment failure is unknown. We evaluated practice patterns for SSTIs in patients with obesity and/or HF and whether short antibiotic durations (≤8 days) were associated with treatment failure.
METHODS: We performed a single-center, retrospective cohort study of inpatients …

OBJECTIVES: Methicillin-resistant Staphylococcus aureus (MRSA) skin and soft tissue infections (SSTIs) represent a major clinical problem in Colombia. The aim of this study was to evaluate the risk factors associated with MRSA SSTI in Colombia.
METHODS: A multicenter cohort study with nested case-control design was performed. Patients with an SSTI with at least 48h of inpatient care were included. Patients with an MRSA SSTI were considered the case group and patients with either a non-MRSA SSTI or with an Methicillin-susceptible S. aureus (MSSA) SSTI were the control groups. A multivariate lo…

BACKGROUND: Skin and soft tissue infections (SSTIs) in hospital and community settings impose a substantial socio-economic burden. Therapeutic uncertainty due to the availability of a wide range of antibiotics and the need for empirical treatment decisions complicate SSTI clinical management. Completion of numerous pairwise meta-analyses to account for this variability in antibiotics is impractical, and many head-to-head comparisons of potential interest are likely not available. In comparing multiple antibiotics simultaneously, this network meta-analysis aims to identify the antibiotic(s) wit…

The efficacy and safety of amoxicillin plus clavulanic acid, a combination of antibiotics potentially effective against beta-lactamase producing bacteria, was compared with that of amoxicillin, erythromycin and co-trimoxazole in an open label, randomized trial in 200 children with mild to severe skin and soft tissue infections. The majority (47%) had impetigo. Fifty (25%) children had mild pyodermas, 56 (28%) had moderate and 94 (47%) children had severe pyodermas. A pure growth of Staphylococcus aureus was isolated from 130 (65%) children. S. pyogenes was isolated from 42 (21%), children and …

Introduction Bone, native joint and soft tissue infections are frequently referred to orthopaedic units although their volume as a proportion of the total emergency workload has not been reported previously. Geographic and socioeconomic variation may influence their presentation. The aim of this study was to quantify the burden of such infections on the orthopaedic department in an inner city hospital, determine patient demographics and associated risk factors, and review our current utilisation of specialist services. Methods All cases involving bone, native joint and soft tissue infections a…

BACKGROUND: Blood cultures (BCs) are commonly performed on children admitted to hospital for skin and soft tissue infections (SSTIs). In recent years, this practice has been questioned in patients with uncomplicated SSTIs because of its low yield. At the same time, however, an increase in community-acquired methicillin-resistant Staphylococcus aureus (CA-MRSA) infections has been described; its influence on rates of bacteremia in patients with SSTIs is unclear. The aims of the study were to describe the performance and the yield of BC in immunocompetent patients with uncomplicated SSTIs and to…

OBJECTIVES: Skin and soft tissue infections (SSTIs) are increasing in incidence, yet there is no consensus regarding management of these infections in the era of community-associated methicillin-resistant Staphylococcus aureus (CA-MRSA). This study sought to describe current pediatric emergency physician (PEP) management of commonly presenting skin infections.
METHODS: This was a cross-sectional survey of subscribers to the American Academy of Pediatrics Section on Emergency Medicine (AAP SoEM) list-serv. Enrollment occurred via the list-serv over a 3-month period. Vignettes of equivocal SSTI…

BACKGROUND: In critically injured patients with multiple injuries and unstable pelvic fracture, late mortality almost invariably results from sepsis.
METHODS: We analyzed the clinical features of 11 patients with sepsis caused by soft tissue infections surrounding the fractured pelvis, of 830 patients with pelvic fracture after blunt trauma treated at our level I trauma center over the past 25 years. Soft tissue infection was defined as abscess formation in subcutaneous tissue or muscle diagnosed by computed tomography or an operation.
RESULTS: Mean injury severity score was 48, and mean sys…

BACKGROUND: Skin and soft tissue infections remain among the most frequently encountered infections in surgery, and their severity ranges from mild cellulitis to severe, necrotizing infections with high incidences of morbidity and mortality. Most commonly, these disorders result from skin lesions in a susceptible host, but sometimes develop following hematogenous spread from a previously unknown focus.
METHODS: The current literature was reviewed to provide an up-to-date summary of the microbiology, diagnosis, pathogenesis, and therapy of complicated skin and soft tissue infections. Special c…

In a prospective study 105 children hospitalized with soft tissue infection, 11 children with suppurative arthritis and 9 children with osteomyelitis were treated with either parenterally administered ampicillin/sulbactam or ceftriaxone. Treatment was randomized using a computer-generated table in a 2:1 fashion: 84 patients received ampicillin/sulbactam and 41 patients received ceftriaxone. Organisms isolated from wound site or blood cultures included Staphylococcus aureus (33), Streptococcus pyogenes (19), Haemophilus influenzae (9) including 4 beta-lactamase-positive organisms, Streptococcus

BACKGROUND: The Laboratory Risk Indicator for Necrotizing Fasciitis (LRINEC) score is a widely used adjunctive diagnostic tool for necrotizing soft tissue infection (NSTI). However, patients with high LRINEC scores do not uniformly have NSTI and often represent a diagnostic gray zone requiring urgent differentiation from other severe soft tissue infections. This study aimed to clarify the clinical characteristics of NSTI among patients with high LRINEC scores.
METHODS: We retrospectively reviewed patients hospitalized for severe soft tissue infections with sepsis between January 2014 and Dece…

Necrotizing soft tissue infections (NSTIs) are severe infections that cause destruction of tissues, including the skin, fascia, and muscle. Here, we discuss a case of a monomicrobial NSTI caused by methicillin-resistant Staphylococcus aureus (MRSA). The patient presented with signs of sepsis and a large, pustular, purulent, and draining skin infection, consisting of 50-60 lesions that had coalesced into a large collection in the right lumbar region and tracked inferolaterally toward the buttocks. This case provides an opportunity to discuss the pathophysiology underlying the progression of MRS…

Necrotizing soft tissue infections (NSTIs) are life-threatening infections that most commonly affect the extremities, perineum, and abdominal wall. These infections begin with the presence of toxin-producing bacteria that invade through a defect in the skin barrier, such as a wound, laceration, trauma, or recent surgical incision. These bacteria cause subsequent tissue destruction and necrosis that can involve the superficial skin, subcutaneous tissue, fascia, and/or muscle. NSTIs can progress quickly, leading to severe sepsis, shock, and even death. NSTIs associated with the breast are an exc…

Necrotizing soft tissue infection is a rare and severe infection of the skin and fascial tissues, characterized by extensive and rapid subcutaneous soft tissue infection. The onset of necrotizing fasciitis often occurs in areas such as the perineum, limbs, abdomen, oral cavity, and neck. In this case, a 49-year-old male patient with a history of type 2 diabetes presented with right shoulder pain for three days. An MRI of the shoulder indicated edema and hematoma formation in the supraspinatus muscle, subscapularis muscle, and surrounding soft tissues of the clavicle. After receiving anti-infec

Necrotizing soft tissue infections (NSTIs) are severe infections affecting the skin, subcutaneous, and muscle tissue. Tissue destruction, systemic toxicity, and increased morbidity are usually observed. An aggressive form of NSTI is called Fournier's gangrene: it starts as a necrotizing fasciitis of the perineal and genital area and affects adjacent soft tissue. It exhibits an increased rate of morbidity, thus requiring a high index of clinical suspicion to secure early surgical debridement. A 58-year-old patient presented in the emergency department complaining of pain in the labia majora. Cl…

BACKGROUND: Lactobacilli are gram-positive, lactic acid-producing, facultative anaerobes of the human microbiota located in the human gastrointestinal tract, genitourinary tract, and the oral cavity and are considered non-pathogenic. When certain risk factors are present, they have the potential to cause serious infections. The incidence of localized infections associated with Lactobacilli are rare and to our knowledge we present the first known case of severe soft tissue infection of the extremity linked to a Lactobacillus strain.
CASE PRESENTATION: We describe the case of a 41-year-old man

Extravasation injuries are uncommon, underreported, and often misdiagnosed in patients. The signs and symptoms of extravasation injuries vary from simple pain and tenderness to tissue necrosis and potentially fatal secondary infections. Extravasation may progress to more severe conditions such as necrotizing fasciitis (NF) or cellulitis, so special care is needed by physicians to identify and treat these injuries correctly. Here, we explore a case study on extravasation injuries mimicking NF leading to infectious complications and discuss the proper diagnosis and treatment of extravasation inj

BACKGROUND: The incidence of skin and soft tissue nontuberculous mycobacteria infection (SSTNI) is increasing. But it may be challenging to diagnose and treat SSTNI.
OBJECTIVE: To summarize course, clinical features, diagnosis and treatment of SSTNI and discuss the association between SSTNI and procedures.
METHODS: A retrospective case series study was conducted among patients diagnosed with SSTNI after surgical procedures. Medical history was collected, disease course and characteristics were analyzed.
RESULTS: 49 patients were collected. Among them, 39 (80%) patients were infected with My

Liposuction is a very common aesthetic procedure nowadays. The complication rate is very minimal, and it increases relatively when combined with other procedures. Infection is an expected complication in liposuction and its risk is below 1% in isolated procedures. Although the risk is very low, it might lead to fatal consequences. In this manuscript, the authors present a previously healthy female who presented to the authors' emergency department post-vibration amplification of sound energy at resonance (VASER)-assisted liposuction and lipofilling done in a private center. Her signs and sympt
