Pelvic asymmetry in children with neuromuscular scoliosis: a computed tomography-based 3D analysis.

PURPOSE: Pelvic fixation in pediatric neuromuscular patients with scoliosis is performed when significant pelvic obliquity is present. Instrumentation to the pelvis can be technically demanding and challenging, often associated with a high complication rate, prolonged operative time, increased radiation exposure, and increased intraoperative bleeding. The high complexity of the pelvic instrumentation technique, particularly in severe cases, led us to develop computed tomography (CT)-based 3D in a series of 53 consecutive neuromuscular patients. This analysis aimed to improve the understanding

Finite element analysis of different cephalomedullary nail treatments for basicervical femoral neck fracture.

Cephalomedullary nail (CMN) is an ideal internal implant for the treatment of Basicervical femoral neck fracture (BFNF). This study uses finite element analysis techniques to compare the biomechanical characteristics of three types of CMN systems, namely proximal femoral nail antirotation (PFNA) InterTAN and proximal femur bionic nail (PFBN), in fixing BFNF using three cannulated screws (CSs) as controls, and analyzed their clinical significance. Based on femoral CT and internal implant data, a finite element analysis model for BFNF internal fixation was established: CSs, PFNA, InterTAN, and P

Delayed posterior sternoclavicular joint dislocation in a young adult managed with plate fixation and cardiothoracic collaboration.

INTRODUCTION: Posterior sternoclavicular joint (SCJ) dislocations are rare, accounting for <1 % of all joint dislocations. Despite their rarity, these injuries warrant urgent recognition due to the SCJ's proximity to mediastinal structures, including the trachea, esophagus, and great vessels. While not always surgical emergencies, delayed or unstable cases can result in life-threatening complications if not managed in an appropriately equipped hospital setting. CASE PRESENTATION: A 28-year-old male presented two weeks after sustaining a right SCJ injury while sliding during a softball game. H

Preventing secondary screw perforation following proximal humerus fracture after locking plate fixation: a new clinical prognostic risk stratification model.

INTRODUCTION: After locking plate (LP) fixation, secondary screw perforation (SSP) is the most common complication in proximal humerus fracture (PHF). SSP is the main cause of glenoid destruction and always leads to reoperation. This study aimed to identify independent risk parameters for SSP and establish an individualized risk prognostic model to facilitate its clinical management. METHODS: We retrospectively reviewed the medical information of patients with PHF who underwent open reduction and internal LP fixation at one medical center (n = 289) between June 2013 and June 2021. Double screw versus angular stable plate fixation of scaphoid waist nonunions in combination with intraoperative extracorporeal shockwave therapy (ESWT).

INTRODUCTION: Over the past years, different fixation techniques focused on rotational stability in order to increase stability and stimulate union rates. Additionally, extracorporeal shockwave therapy (ESWT) has gained importance in the treatment of delayed and nonunions. Purpose of this study was to compare the radiological and clinical outcome of two headless compression screws (HCS) and plate fixation in scaphoid nonunions, in combination with intraoperative high energy ESWT. MATERIALS AND METHODS: Thirty-eight patients with scaphoid nonunions were treated by using a nonvascularized bone

Open Screw Fixation of Large Anterior Glenoid Rim Fractures Using a Deltopectoral Approach With Subscapularis Splitting.

Anterior glenoid rim fracture is a consequence of the humeral head impacting the glenoid fossa. The management of large glenoid fractures involving more than 20% of the articulating glenoid requires surgical treatment. The 2 main techniques are open reduction internal fixation (ORIF) by screws and arthroscopic treatment using suture anchors or transcutaneous screws. Next to the technical equipment, a surgeon requires extensive experience to achieve good results with the arthroscopic technique. The main disadvantage using the ORIF technique is the detachment of the subscapularis muscle, which i

Dynamic hip screw fixation of subtrochanteric femoral fractures.

INTRODUCTION: A subtrochanteric proximal femur fracture occurs in the 5 cm of bone immediately distal to the lesser trochanter. UK national guidelines advise that adults with subtrochanteric fractures should be treated with an intramedullary nail (IMN). This study aims to compare peri-operative outcome measures of patients with subtrochanteric fractures treated with either an IMN or a dynamic hip screw (DHS) construct. MATERIALS AND METHODS: We retrospectively reviewed subtrochanteric fractures presenting at our institution over 4.5 years (October 2014-May 2019), classifying them into two tre

Addressing Antibiotic Hip Spacer Instability via Hybrid Screw-cement Fixation of a Constrained Liner and Cement-rebar Interface Techniques: A Technical Narrative.

Prosthetic joint infection is a challenging and devastating complication after total hip arthroplasty. The benchmark for treatment remains two-stage revision arthroplasty, in which an antibiotic-impregnated spacer is used to eradicate the infection. Although several types of spacer constructs have been described, they have historically been associated with high rates of mechanical complications, namely, dislocation, spacer fracture, and periprosthetic femur fracture. Spacer dislocation is the most common, with reported rates as high as 41%. Here, the authors present a surgical technique to imp

Pseudoaneurysm of the subclavian artery following clavicle fracture due to blunt traumatism: a case report.

The clavicle fractures are frequent, vascular injuries associated with closed fractures of clavicle are rare. The pseudoaneurysms of the subclavian artery constitute an exceptional complication. We report a case of a 40-year-old who presented an expanding hematoma of the right side of the neck after a road traffic accident. Radiography of the right shoulder showed a midclavicular fracture. An arterial doppler of vessels showed a circulating hematoma in the contact of the right subclavian artery with a correct distality flow. Computed tomographic angiogram of the chest confirmed the diagnosis o

Finite Element Analysis of 2- and 3-Point Internal Fixation Methods for the Treatment of Zygomaticomaxillary Complex Fracture.

Open reduction followed by internal fixation (ORIF) has been regarded as the most effective technique for surgical repair of zygomaticomaxillary complex (ZMC) fractures. However, the ideal internal fixation method to achieve stable reduction remains controversial. This research aims to assess and compare the stability of the 2- and 3-point screw-plate fixation methods using finite element method (FEM). Based on computed tomography (CT), the finite element models of 2-point (ZFS + IOR) and 3-point (ZFS + IOR + ZMB) fixation for isolated displaced ZMC fracture were reconstructed. The force of 12

Suture anchor fixation strength in the Latarjet procedure: a biomechanical study in cadavers.

BACKGROUND: The use of metallic screws for graft fixation during the Latarjet procedure is not devoid of complications. The purpose of the present cadaver study was to determine the initial strength of coracoid graft fixation using suture anchors and compare it to that of the traditional screw fixation of the graft using a fresh frozen human shoulder cadaver model. MATERIALS AND METHODS: Twelve unpaired fresh frozen cadaver shoulders were utilized. In the first group, suture anchor fixation of the graft was used, while 3.75-mm cannulated screws were used in the second group. The specimens wer

Titanium Lag Screw Versus Miniplate Fixation in the Treatment of Anterior Mandibular Fractures.

PURPOSE: The use of plates for open reduction and internal fixation of mandibular fractures has become a widely accepted method in the past 3 decades. However, the anterior mandible is well suited to lag screw fixation owing to the thickness of its bony cortices. Hence, the purpose of the present study was to comparatively evaluate clinical outcomes of fixation using lag screws and miniplates in anterior mandibular fractures. PATIENTS AND METHODS: Fifty patients reporting to the department of oral and maxillofacial surgery with noncomminuted anterior mandibular fractures were randomly divided

Lift-Off Screw Results in Accurate Sagittal Tilt Correction in a Distal Radius Fracture Model.

PURPOSE: To determine the accuracy of a distal-first open reduction internal plate fixation technique in achieving correction of volar tilt in dorsally angulated distal radius fractures. METHODS: Twenty foam radius models were divided evenly into groups N (normal) and O (osteoporotic). Dorsally angulated extra-articular distal radius fractures were then created. Group O underwent further modification to simulate an osteoporotic model. After static pinning in various degrees of dorsal angulation, opaque fiducial markers were placed and fluoroscopy was used to measure prereduction volar tilt. A

Distal locking screw length for volar locking plate fixation of distal radius fractures: Postoperative stability of full-length unicortical versus shorter screws.

We compared post-operative distal radius fracture (DRF) displacement after volar locking plate fixation using full-length unicortical and shorter-length distal locking screws. In this non-inferiority, retrospective cohort study, DRFs treated with volar locking plate fixation were evaluated on X-rays. In the full-length group, volar locking plate fixation was performed with full-length unicortical distal locking screws. In the shorter-length group, the distal locking screws were planned pre-operatively to be approximately 75% of the distal radius depth based on the lunate depth, and the same de

[Research progress of Hoffa fracture].

Hoffa fracture is a rare type of fracture confined to the coronal plane of the femoral condyle. High-energy is a common reason of Hoffa fracture, and low-energy trauma and iatrogenic injury can also cause Hoffa fracture in some cases. The commonly used classifications include Letenneur classification, CT classification, AO classification and modified AO classification. X-ray is the first choice to diagnose Hoffa fractures, fracture lines can be found. If the X-ray is negative, CT scan and MRI should be performed. Nondisplaced fractures can be managed conservatively with cast immobilization, ho
