Distal phalangeal reconstruction for recurrent intraosseous epidermoid cyst of the finger - a case report.

Intraosseous epidermoid cyst of the finger phalanx is rare. We report a case of postoperative recurrent intraosseous epidermoid cyst of the distal phalanx of the ring finger. To prevent further recurrence while maintaining morphology and function, the distal half of the distal phalanx that included the epidermoid cyst was resected to completely remove the lesion. The distal phalanx was then reconstructed by grafting corticocancellous bone from the ilium and shaped into a distal phalanx. The operation was performed using a through-the-nail approach, temporarily removing the nail and placing a l

PHACE syndrome: report of a case with a glioma of the anterior skull base and ocular malformations.

PHACE syndrome consists of the constellation of manifestations including Posterior fossa anomalies of the brain (most commonly Dandy-Walker malformations), Hemangiomas of the face and scalp, Arterial abnormalities, Cardiac defects, and Eye anomalies. We present the case of a patient who presented with respiratory distress at birth secondary to a large nasal glioma. She was subsequently found to have a ventricular septal defect (VSD), a facial hemangioma, and a malformation of the eye and optic nerve head. The nasal glioma, which extended to the cribriform plate, has not been described in this

Case report of bone mastocytosis: total hip arthroplasty for osteoarthritis and open reduction for condylar fracture of the knee.

A 72-year-old woman was admitted in 1984 for painful protrusive osteoarthritis of the left hip diagnosed as systemic mastocytosis with bone lesions and clinical features of intestinal malabsorption but no clinical skin lesion. The total hip replacement, refused in a first step because of the bone pathology, was carried out two years later. Signs of loosening appeared after one year. In 1990, following a traumatic bicondylar fracture of the left knee, an osteosynthesis was carried out. Ten days later, a shaft pathologic fracture of the femur above the osteosynthesis plate implied another open r

Stability of Le Fort III advancement surgery in children with Crouzon's, Apert's, and Pfeiffer's syndromes.

The records of 19 children who had undergone Le Fort III advancement surgery were used to assess the stability of the midfacial segment following surgery. Cephalometric radiographs of each individual were analyzed longitudinally. The average forward movement of the maxilla at the time of surgery was 12.4 mm. The average amount of relapse for the group was 9.4 percent in the horizontal plane of space and 5.5 percent in the vertical plane of space. The data obtained from this sample indicate that the Le Fort III advancement is a relatively stable procedure. The amount of relapse or stability tha

[Can one determine the age at which epiphysiodesis should be performed in the treatment of genu valgum in the adolescent?].

The authors describe a simplified method for the assessment of the correct time at which tibial epiphyseal arrest may be performed to correct genu valgum in adolescents. The calculation is based on measurements of the length of the tibia, the length of the upper tibial plate and the intermalleolar distance. These measurements make it possible to establish an index of prediction of the amount of growth necessary to bring about correction. This index is then associated with the tables of Green and Anderson in relation to bone age and residual potential for growth. In this way, the age at which e

Functional and Radiological Outcomes of Distal Femur Fractures Fixed With Bicolumnar Plating: A Retrospective Study.

BACKGROUND: Distal femur fractures are challenging orthopedic injuries, especially when involving the intra-articular extension or occurring in osteoporotic bone. With the increased use of dual-column fixation, bicolumnar plating has shown promise in improving stability and outcomes. OBJECTIVES: This study aimed to evaluate the functional and radiological outcomes of distal femur fractures treated with bicolumnar plating using the Knee Society Score (KSS) and the Modified Radiological Union Score of Tibia (RUST) at the three-month, six-month, and one-year follow-up. MATERIALS AND METHODS: Th

Age-specific over-the-top techniques for physeal sparing anterior cruciate ligament (ACL) reconstruction in skeletally immature patients: Current concepts for prepubescents to older adolescents.

Tailored surgical strategies for anterior cruciate ligament (ACL) reconstruction in skeletally immature patients are presented, emphasizing techniques to minimize growth plate damage and ensure stability. As ACL injuries in youth increase, delaying surgery can lead to joint damage and poor recovery outcomes. Using magnetic resonance imaging-based assessments of skeletal maturity, the authors propose the 'over-the-top (OTT)' approach with lateral tenodesis, adapted for three growth stages: prepubescents, young adolescents and older adolescents. For prepubescents, the extra-physeal approach avoi

Radiographic evaluation of the forelimb bone development in maned wolves (Chrysocyon brachyurus).

This study aimed to assess the fusion of growth plates and the development of secondary ossification centres in the forelimb bones of maned wolves (Chrysocyon brachyurus), contrasting the findings with established data from domestic dogs. Three maned wolves, comprising one male and two females, initially aged between 3 and 4 months, were subjected to monthly radiographic evaluations until 10-11 months of age, followed by bimonthly assessments until 18-19 months of age, encompassing both forelimbs. The closure times of growth plates were observed as follows: supraglenoid tubercle (7-8 months),

Use of volar distal radius plate dorsally for wrist arthrodesis in patients with upper limb spasticity: A retrospective review.

Standard wrist arthrodesis implants are generally designed for adult joints with arthritis. They are often too big for patients with limb spasticity who generally tend to have osteopenic and smaller bones for their age, resulting in high complication rates. We previously described the novel use of volar distal radius variable-angle locking plate (APTUS® Wrist distal radius system 2.5, Medartis AG, Basel, Switzerland) on the dorsum for wrist arthrodesis in patients with limb spasticity. This study aimed to further establish the use of the implant by reporting the outcomes in cohort A (nondistal

Stable internal fixation with dual plate technique for osteoporotic distal femur fractures.

BACKGROUND: Osteoporotic distal femur fractures are difficult in terms of fracture treatment and recovery. We hypothesized that the minimally invasive plate osteosynthesis (MIPO) with dual plate technique increased fixation stability and encouraged early mobilization and return to pre-fracture activity. MATERIAL AND METHODS: Between 2016 and 2019, 22 patients were treated with the dual plate technique for osteoporotic distal femur fractures. To evaluate the clinical outcome, the modified Western Ontario and McMaster Universities (WOMAC) score, Tegner activity score, knee range of motion (ROM)

Minimally invasive plate osteosynthesis using the oblong hole of a locking plate for comminuted distal fibular fractures.

BACKGROUND: Nonunion is a rare complication for distal fibular fractures. However, when there is a high degree of comminution, nonunion may occur. In this article, we describe a novel technique that uses the oblong hole of a locking plate to lengthen the fibula for fracture reduction. This technique is straightforward and allows for easy control of the comminuted fracture to restore length and rotation at the time of plate application without opening the fracture site. METHODS: Thirty-five consecutive patients, who were treated with the minimally invasive plate osteosynthesis (MIPO) technique

Minimally invasive plate osteosynthesis with dual plating for periprosthetic distal femoral fractures following total knee arthroplasty.

INTRODUCTION: Adequate treatment for periprosthetic distal femur fractures is challenging because of various reasons, including severe osteoporosis and distal fragments that are too small or too distal. We have introduced a new surgical technique for dual plating of periprosthetic distal femur fractures following total knee arthroplasty (TKA) and determined the clinical and radiological outcomes of minimally invasive plate osteosynthesis (MIPO) with a dual locking compression plate (LCP). MATERIALS AND METHODS: Between January 2010 and July 2019, 18 patients [mean age, 74.8 (68-89) years; ave

Biomechanical analysis of four augmented fixations of plate osteosynthesis for comminuted mid-shaft clavicle fracture: A finite element approach.

Sufficient stabilization of comminuted mid-shaft clavicle fractures via plate fixation is difficult to achieve. Various augmentations, including interfragmentary screws and cerclage wiring, have been adopted to reinforce fixation stability. The present study aimed to assess the biomechanical stability of augmented plate fixations using the finite element method. First, a clavicle fracture model was created from CT data. Fixation was then induced using a locking compressive plate (LCP) with the following four augmentations: i) Double inner cerclage wirings (DICW), ii) double outer cerclage wiri

Distal Radius Fractures in the Elderly: Use of the Volar Bearing Plate.

BACKGROUND: Distal radius fractures represent some of the most common injuries to the upper extremity, yet current evidence demonstrates great variability in the management of this injury. Elderly patients, in particular, stand to benefit from the early mobilization provided by operative fixation with a volar bearing plate. METHODS: We conducted a retrospective chart review on all patients 65 years or older who underwent unilateral open reduction internal fixation of distal radius fractures using a volar bearing plate at a single institution between January 2014 and January 2016. We excluded

Use of an Additional Nonlocking Screw in Olecranon Fracture Osteosynthesis Changes Failure Mechanism.

Hardware-related complications can occur when plate fixation is used to stabilize osteoporotic fractures involving the olecranon. The use of an additional nonlocking screw, placed retrograde into the proximal fracture segment, may improve stability under load. The purpose of this study was to conduct a biomechanical comparison of olecranon repair constructs with and without this additional retrograde screw. Nine matched pairs of elderly fresh-frozen cadaveric upper extremities were used. Two-part olecranon fractures were modeled, and fracture stabilization was performed. Olecranon plates were

Outcomes of distal femur fractures treated with the Synthes 4.5 mm VA-LCP Curved Condylar Plate.

PURPOSE: Given the recent controversy in the literature and the alarming reports of early mechanical failure associated with the use of the Synthes 4.5 mm VA-LCP Curved Condylar Plate in acute distal femur fractures, the goal of our study was to examine the outcomes and mechanical failure rates of this implant in a larger patient population. METHODS: Patients 18 years of age and older who underwent plate fixation of their acute distal femoral fracture using the Synthes 4.5 mm VA-LCP Curved Condylar Plate were included in this retrospective study. Increased wound complication with intramedullary screw fixation of clavicle fractures: Is it thermal necrosis?

PURPOSE: Adult mid-shaft clavicle fractures are common injuries. For displaced fractures, open reduction with plate or intramedullary (IM) fixation is the widely used techniques. All methods have their own potential drawbacks, especially related to local soft tissue complications. There is little information about outcome and management of local wound complications after clavicle fracture fixations. METHODS: Ninety-seven patients underwent open reduction and internal fixation, 17 were treated with IM screw fixation and 80 with plate fixation. Wound complication occurred in eight patients (8.2
