Risk of Revision Shoulder Arthroplasty After Anatomic and Reverse Total Shoulder Arthroplasty.

INTRODUCTION: The objective of this study was to determine the survivorship of anatomic total shoulder arthroplasty (aTSA) and reverse TSA (rTSA) over a medium-term follow-up in a large population-based sample and to identify potential risk factors for revision surgery. METHODS: The State Inpatient Database from the Healthcare Cost and Utilization Project was used to identify patients who underwent aTSA or rTSA from 2011 through 2015 using ICD9 codes. We modeled the primary outcome of time to revision or arthroplasty using the Cox proportional hazards model. The predictors of revision surgery

Epidemiology and trends of adult ice hockey injuries presenting to United States emergency departments: A ten-year analysis from 2007-2016.

PURPOSE: The adult population is under-represented in existing ice hockey injury studies, despite the number of United States (US) adult ice hockey players increasing from 103,533 in 2007 to 180,400 in 2016 (74%). This study establishes trends in demographics, injury location, and injury type for adult ice hockey players (≥19 years old) in the United States. METHODS: The National Electronic Injury Surveillance System (NEISS) was queried for all ice hockey injuries from January 1, 2007 to December 31, 2016. Cases under age 19 were excluded. Each injury's narrative text field was reviewed to de

Reverse Total Shoulder Arthroplasty: Indications and Techniques Across the World.

Reverse total shoulder arthroplasty (RTSA) is a common treatment for rotator cuff tear arthropathy. We performed a systematic review of all the RTSA literature to answer if we are treating the same patients with RTSA, across the world. A systematic review was registered with PROSPERO, the international prospective register of systematic reviews, and performed with Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) guidelines using 3 publicly available free databases. Therapeutic clinical outcome investigations reporting RTSA outcomes with levels of evidence I to IV wer

Allograft-prosthesis composite reconstruction of the proximal part of the humerus: surgical technique.

BACKGROUND: Limb salvage following resection of a tumor in the proximal part of the humerus poses many challenges. Reconstructive options are limited because of the loss of periarticular soft-tissue stabilizers of the glenohumeral joint in addition to the loss of bone and articular cartilage. The purpose of this study was to evaluate the functional outcome and survival of the reconstruction following use of a humeral allograft-prosthesis composite for limb salvage. METHODS: An allograft-prosthesis composite was used to reconstruct a proximal humeral defect following tumor resection in thirty-

Specific occupant and collision characteristics are associated with motor vehicle collision-related blunt cerebrovascular artery injury.

BACKGROUND: Blunt cerebrovascular artery injury (BCI) remains difficult to diagnose but is recognized with increasing frequency after motor vehicle collisions (MVCs). Failure to detect this injury in a timely fashion can be devastating. Criteria that can be used to heighten the suspicion of this injury have been suggested; however, more encompassing screening has been recommended. To address this need, we sought to describe occupant, vehicle, and collision characteristics among MVC occupants who sustained a BCI. METHODS: All cases of BCI identified in the National Automotive Sampling System C

Increased Risk of Medical and Implant-Related Complications in Patients With Atrial Fibrillation After Primary Total Shoulder Arthroplasty: A 1:1 Propensity-Matched Cohort Analysis.

BACKGROUND: Atrial fibrillation (AF) is a common cardiac comorbidity associated with increased risk of bleeding and complications following surgery. Few studies have evaluated the impact of preoperative diagnosis of AF on complications following primary total shoulder arthroplasty (TSA). The purpose of this study is to evaluate the risk of 90-day and two-year postoperative medical and implant-related complications in patients undergoing TSA with a diagnosis of AF. METHODS: A multicentre database, TriNetX (TriNetX, LLC, Cambridge, MA, USA), was queried for patients who underwent primary TSA be

Beyond the "two-midnight rule": social factors drive prolonged stay after outpatient total shoulder arthroplasty.

BACKGROUND: Total shoulder arthroplasty (TSA) has been removed from the Center for Medicare and Medicaid Services "in-patient only" list. Thus, the onus of TSA outpatient versus inpatient classification has become more complex, leading to failed outpatient TSA and unintended extended hospital stays. While most patients can be safely treated with outpatient TSA, a select vulnerable population may benefit from inpatient designation. This study aims to identify the rate of failure to discharge after 2 midnights, perioperative reasons for failure to discharge, and independent risk factors associat

Trends in Use of Medications for Opioid Use Disorder among Commercially Insured U.S. Surgical Patients, 2016 to 2022.

BACKGROUND: The optimal management of perioperative pain in patients using medications for opioid use disorder (MOUD) is unclear. To motivate and inform efforts to develop evidence-based guidelines for perioperative pain management in these patients, it is important to evaluate whether the prevalence of MOUD use in surgical patients is increasing and to identify which procedures have the highest rate of MOUD use. METHODS: This cohort study analyzed adults 18 to 64 yr undergoing 1 of 1,083 major surgical procedures from 2016 to 2022 from the Merative MarketScan Commercial Database, which inclu

Letter-only discharge process for virtual fracture clinic, a safe alternative to telephone discharge, outcomes and 12 month follow up for 1140 patients.

Telephone consult has become the accepted discharge method for virtual fracture clinic (VFC) within the United Kingdom. Telephone consultations are time consuming; many orthopaedic units lack the resources and staff to deliver large numbers of daily telephone consultations which may block the development of an effective VFC. Our study aim was to validate a letter only VFC discharge process for safety and efficacy. A letter only discharge VFC was instigated in response to the COVID-19 pandemic (April 2020). No ethical approval was required, the protocol was designed as a phased service evaluati

Antegrade intramedullary fixation for adolescent fifth metacarpal neck fracture and its impact on epiphyseal growth.

BACKGROUND: Antegrade intramedullary nailing (AIMN) with Kirschner wire (K-wire) is a minimally invasive osteosynthesis technique. This procedure has been widely performed to treat the fifth metacarpal neck fracture (FMNF) in adults. This study was performed to determine whether using AIMN with a single K-wire to treat FMNF in adolescents would have good clinical and radiographic outcomes. METHODS: In this retrospective study, 21 children (aged 11-16 years) with FMNF were treated using AIMN with a single K-wire from May 2017 to January 2020 in our hospital. Indications for intervention were s
