Sleep apnea in patients with achondroplasia associated with foramen magnum stenosis.

OBJECTIVE: Sleep apnea is frequently associated with foramen magnum stenosis in patients with achondroplasia and considered a cause of sudden death. The aim of this study was to evaluate the results of respiratory polygraphy (RPG) before and after surgery in patients who underwent foramen magnum decompression (FMD) to elucidate the effectiveness for treatment of sleep apnea. METHODS: The findings of RPG, MRI, and otolaryngological fibroscopy were retrospectively reviewed in 22 children with achondroplasia who underwent FMD at a single institution from 2016 to 2022. The Mann-Whitney U-test was

Thyroarytenoid muscle activity in sleep apneas.

In normal subjects the thyroarytenoid muscle (TA), a vocal cord adductor, has phasic expiratory activity during wakefulness that disappears during non-rapid-eye-movement (NREM) sleep. Fiber-optic studies have reported absent or irregular vocal cord movements during obstructive apneas and vocal cord adduction during central apneas. This study was designed to investigate TA activity during NREM sleep in 14 subjects with sleep apnea by means of intramuscular wire electrodes. During central apneas, which were recorded in three subjects, continuous TA activity was observed. During obstructive apnea

Prevalence of Sleep-Disordered Breathing in Prader-Willi Syndrome.

INTRODUCTION: Sleep-disordered breathing (SDB) is common in patients with Prader-Willi Syndrome (PWS). However, the prevalence of SDB varies widely between studies. Early identification of SDB and factors contributing to its incidence is essential, particularly when considering growth hormone (GH) therapy. OBJECTIVES: The aims of the study were to describe the prevalence and phenotypes of sleep-disordered breathing (SDB) in patients with Prader-Willi syndrome (PWS) and to determine the effects of age, gender, symptoms, GH therapy and body mass index on SDB severity. METHODS: This study was a

Diagnosis of sleep apnea without sensors on the patient's face.

STUDY OBJECTIVES: Thermistors, nasal cannulas, and respiratory inductance plethysmography (RIP) are the recommended reference sensors of the American Academy of Sleep Medicine (AASM) for the detection and characterization of apneas and hypopneas; however, these sensors are not well tolerated by patients and have poor scorability. We evaluated the performance of an alternative method using a combination of tracheal sounds (TSs) and RIP signals. METHODS: Consecutive recordings of 70 adult patients from the Pays de la Loire Sleep Cohort were manually scored in random order using the AASM standar

Sex differences in sleep apnea and comorbid neurodegenerative diseases.

Sleep apnea is a disorder, which increasingly affects people worldwide. Whether the associated hypoxic events during sleep are central or obstructive in origin, the end result is excessive daytime sleepiness and an increased risk for several comorbidities, such as cardiovascular and neurodegenerative disorders. Sleep apnea is diagnosed more frequently in men than women, suggesting a role of sex hormones in the pathology of the disease. Furthermore, there are sex differences in the development and progression of comorbid diseases associated with sleep apnea. Therefore, treatment of sleep apnea

Long-term effects of caffeine therapy for apnea of prematurity on sleep at school age.

RATIONALE: Apnea of prematurity is a common condition that is usually treated with caffeine, an adenosine receptor blocker that has powerful influences on the central nervous system. However, little is known about the long-term effects of caffeine on sleep in the developing brain. OBJECTIVES: We hypothesized that neonatal caffeine use resulted in long-term abnormalities in sleep architecture and breathing during sleep. METHODS: A total of 201 ex-preterm children aged 5-12 years who participated as neonates in a double-blind, randomized, controlled clinical trial of caffeine versus placebo un

Obesity, sleep apnea, the airway and anesthesia.

PURPOSE OF REVIEW: Recent articles indicate that at the present time disastrous respiratory outcomes during the perioperative management of patients with obstructive sleep apnea are a major problem for the anesthesia community. RECENT FINDINGS: A recent review of the literature indicates that disastrous respiratory outcomes are due to intubation failure, respiratory obstruction soon after extubation, and respiratory arrest after narcotic and sedative medication. In obese patients, the basic problem is deposition of adipose tissue in the pharynx. The excessive adipose tissue in the pharynx cau

Classification of sleep apnea through sub-band energy of abdominal effort signal using Wavelets + Neural Networks.

Detection and classification of sleep apnea syndrome (SAS) is a critical problem. In this study an efficient method for classification sleep apnea through sub-band energy of abdominal effort using a particularly designed hybrid classifier as Wavelets + Neural Network is proposed. The Abdominal respiration signals were separated into spectral sub-band energy components with multi-resolution Discrete Wavelet Transform (DWT). The energy content of these spectral components was applied to the input of the artificial neural network (ANN). The ANN was configured to give three outputs dedicated to SA

[Sleep and respiration].

Nocturnal sleep-associated breathing disorders have only been investigated thoroughly since about 15 years. From these studies it became clear that many respiratory disorders, in particular chronic obstructive ones, may severely impair nocturnal breathing and thus oxygen supply. by physiologically reduced ventilation at night, particularly in patients with already reduced pulmonary function, oxygen supply may become insufficient. In order to prevent late sequelae, early invalidity or precocious death, a comprehensive diagnostic approach is mandatory. Therapy consists of thorough anti-obstructi

[Sleep apnea syndrome, arterial hypertension and cardiovascular risks].

Excessive daytime somnolence is the main symptom of the obstructive sleep apnea syndrome (OSAS). Repetitive upper airway obstructions during sleep are followed by arousals and consequent sleep fragmentation. Furthermore, obstructive apneas or hypopneas and arousals are accompanied by fluctuations of blood pressure and heart rate. Several recent studies have found OSAS to be an independent risk factor for arterial hypertension and cardiovascular diseases.

Detection of Respiratory Events during Sleep Based on Fusion Analysis and Entropy Features of Cardiopulmonary Signals.

Sleep apnea hypopnea syndrome (SAHS) is a common sleep disorder with a high prevalence. The apnea hypopnea index (AHI) is an important indicator used to diagnose the severity of SAHS disorders. The calculation of the AHI is based on the accurate identification of various types of sleep respiratory events. In this paper, we proposed an automatic detection algorithm for respiratory events during sleep. In addition to the accurate recognition of normal breathing, hypopnea and apnea events using heart rate variability (HRV), entropy and other manual features, we also presented a fusion of ribcage

Sleep apnea following a pharyngeal flap: a feared complication.

Pharyngeal flaps are often used to correct velopharyngeal insufficiency. They produce a permanent partial obstruction of the velopharyngeal space. Respiratory obstruction and obstructive sleep apnea have been reported following this surgery. We undertook a study to find out the incidence of sleep apnea associated with pharyngeal flap surgery. Forty-one children (aged 2 to 22 years) admitted for a pharyngeal flap underwent a polysomnographic recording prior to their surgery. One child with Steinert's disease showed some episodes of obstructive apnea, and the surgery was canceled. Forty children

Craniofacial growth and function in achondroplasia: a multimodal 3D study on 15 patients.

BACKGROUND: Achondroplasia is the most frequent FGFR3-related chondrodysplasia, leading to rhizomelic dwarfism, craniofacial anomalies, stenosis of the foramen magnum, and sleep apnea. Craniofacial growth and its correlation with obstructive sleep apnea syndrome has not been assessed in achondroplasia. In this study, we provide a multimodal analysis of craniofacial growth and anatomo-functional correlations between craniofacial features and the severity of obstructive sleep apnea syndrome. METHODS: A multimodal study was performed based on a paediatric cohort of 15 achondroplasia patients (me
