Impact of organizational infrastructure on beta-blocker and aspirin therapy for acute myocardial infarction.

BACKGROUND: Although organizational change has been advocated as a critical component of quality improvement, there is little data available on the variation and effectiveness of organizational elements in the care of acute myocardial infarction (AMI). PURPOSE: This study was designed to examine the impact of organizational infrastructure on the use of aspirin and beta-blockers during and after AMI. METHODS: We assessed organizational infrastructure for AMI care in 44 hospitals in Kansas and linked these data to patient-specific process of care data collected in Kansas as part of the Coopera

Identification and validation of an intratumor heterogeneity-related prognostic signature in triple-negative breast cancer: a study based on integrative machine learning and single-cell sequencing.

INTRODUCTION: While intratumor heterogeneity (ITH) is recognized as a fundamental driver of treatment resistance and disease progression in triple-negative breast cancer (TNBC), existing prognostic models often fail to quantify ITH as a biomarker robustly. METHODS: This study obtained a training set from the METABRIC, while validation sets were sourced from the GEO database. ITH scores were calculated using the DEPTH2 algorithm, a computational method to infer heterogeneity from bulk transcriptomic data. Patients were stratified into distinct high-ITH and low-ITH groups based on these scores.

Impaired fertility and perinatal outcomes in adenomyosis: insights from a novel murine model and uterine gene profile alterations during implantations.

BACKGROUND: Adenomyosis is a uterine disorder closely linked to infertility and adverse pregnancy outcomes. Despite its clinical significance, the mechanisms by which adenomyosis impairs embryo implantation and perinatal outcomes remain incompletely defined. Previous studies have indicated that alterations in the uterine microenvironment may contribute to these reproductive challenges. OBJECTIVE: To investigate the effects of adenomyosis on fertility and perinatal outcomes using a novel murine model and to identify molecular pathways involved in implantation failure and pregnancy loss. Tryptophan Metabolism in Cardiometabolic Diseases: Focus on the Kynurenine Pathway.

Tryptophan (TRP) metabolism has emerged as a critical interface linking inflammation, immune regulation, oxidative stress, and cellular energetics. The kynurenine pathway, the predominant route of TRP degradation, is highly responsive to inflammatory stimuli and generates a spectrum of bioactive metabolites with divergent and context-dependent biological effects. Indoleamine 2,3-dioxygenase 1 (IDO1)-mediated TRP catabolism integrates immune activation with downstream metabolic signaling, influencing redox homeostasis, endothelial function, and mitochondrial energetics, in part by regulating ni

miR-145 inhibits colorectal cancer progression and metastasis by targeting SNAI1 and the cAMP/PKA pathway.

BACKGROUND: Transcriptional repressor 1 of the Snail family (SNAI1) and miR-145 play crucial roles in the development of colon cancer. Tumor invasiveness and epithelial-mesenchymal transition (EMT) are associated with SNAI1, whereas miR-145 is considered a tumor suppressor. Although cAMP/PKA signaling is critical for regulating cell survival and invasiveness, the precise mechanisms through which SNAI1 and miR-145 influence tumor cell behavior via this pathway remain unclear. OBJECTIVES: To examine the mechanisms by which SNAI1 and miR-145 regulate colon cancer cell invasion and survival throu

Crosstalk of the IκB Kinase with Spliced X-Box Binding Protein 1 Couples Inflammation with Glucose Metabolic Reprogramming in Epithelial-Mesenchymal Transition.

Epithelial-mesenchymal transition (EMT) plays a critical role in airway injury, repair, and structural remodeling. IκB kinase (IKK)-NFκB signaling regulates late EMT-associated gene expression. However, IKK-mediated mesenchymal transition occurs earlier than NFκB/RelA subunit-dependent EMT gene expression, leading us to investigate the hypothesis that IKK plays an independent mechanism in transforming growth factor-β (TGFβ)-induced EMT. Time-resolved dissection of early proteome and phosphoproteome changes in response to TGFβ and a specific IKK inhibitor, BMS-345541, revealed that IKK regulate

The Cat Flea (Ctenocephalides felis) Immune Deficiency Signaling Pathway Regulates Rickettsia typhi Infection.

Rickettsia species are obligate intracellular bacteria with both conserved and lineage-specific strategies for invading and surviving within eukaryotic cells. One variable component of Rickettsia biology involves arthropod vectors: for instance, typhus group rickettsiae are principally vectored by insects (i.e., lice and fleas), whereas spotted fever group rickettsiae are exclusively vectored by ticks. For flea-borne Rickettsia typhi, the etiological agent of murine typhus, research on vertebrate host biology is facilitated using cell lines and animal models. However, due to the lack of any st

Wnt4 signaling is associated with the decrease of proliferation and increase of apoptosis during age-related thymic involution.

The thymus is a central lymphoid organ that is responsible for T-lymphocyte development and maturation. Through negative and positive selection, lymphoid progenitor cells, which initiate from the bone marrow, develop into mature T cells in the thymus, and are subsequently involved in peripheral cell immunity. It has been reported that the Wnt signaling pathway exists widely in thymic epithelial cells and T lymphocytes. Wnt signaling affects the shape and function of thymic epithelial cells and has an important role in maintaining pro‑T‑cells, and in the subsequent T‑cell differentiation. Previ

Evidence that NMDA-dependent limbic neural plasticity in the right hemisphere mediates pharmacological stressor (FG-7142)-induced lasting increases in anxiety-like behavior: study 3--the effects on amygdala efferent physiology of block of NMDA receptors prior to injection of FG-7142 and its relationship to behavioral change.

The findings of this study support the hypothesis that N-methyl-D-aspartate (NMDA) receptors mediate the initiation of long-term potentiation (LTP) and behavioral changes induced by the anxiogenic beta-carboline, FG-7142. Unlike previous work, this study examined the effects of FG-7142 on LTP of amygdala efferents in both hemispheres. 7-amino-phosphono-heptanoic acid (AP7), a competitive NMDA receptor blocker, given prior to administration of FG-7142, prevented LTP in amygdala efferent transmission to the medial hypothalamus and periacqueductal gray (PAG). When given FG-7142 alone, cats showed

A multi-omics characterization of the gut microbiota of weaned piglets in response to Lactobacillus plantarum supplementation.

Weaning is a critical developmental stage in piglets and is frequently associated with intestinal dysfunction, diarrhea, and growth retardation due to abrupt dietary and environmental changes. Probiotics have been explored as antibiotic alternatives to alleviate postweaning stress, and Lactobacillus plantarum has shown potential for modulating the gut microbiota and improving intestinal health. However, most previous studies have relied on single-omics approaches, limiting mechanistic understanding of host-microbiome interactions. This study employed an integrated multi-omics approach combinin

Beyond the Mandibular Canal: A Narrative Review of Inferior Alveolar Nerve Variations and Their Clinical Significance.

The inferior alveolar nerve (IAN) is commonly presented as a relatively constant branch of the mandibular nerve that gives rise to the mylohyoid nerve (MHN), enters the mandibular foramen, follows a single mandibular canal, and terminates as mental and incisive branches. However, direct human anatomical evidence shows substantially greater complexity. This critical narrative review synthesizes cadaveric, microdissection, histological, imaging, and osteological evidence on proximal IAN roots, relationships with the maxillary artery, communications with neighboring mandibular nerve branches, MHN

Emerging Therapeutic Protein Targets of Inflammation: A Comprehensive Review.

Conventional anti-inflammatory therapies, including nonsteroidal anti-inflammatory drugs (NSAIDs) and corticosteroids, remain the mainstay of treatment for inflammatory disorders. However, their long-term use is often limited by adverse effects, insufficient target specificity, and an inability to achieve sustained resolution of chronic inflammation. Advances in the understanding of inflammatory signalling pathways have led to the identification of several promising protein targets for the treatment of chronic inflammatory and autoimmune diseases. This review provides a comprehensive overview

Restoration of infectivity-virulence and toxin biosynthesis in Botrytis cinerea B459 via upstream-targeted marker-free CRISPR/Cas9 editing of the transcription factor Bcin04g03490.

Botrytis cinerea is a polyphagous fungal pathogen affecting a wide range of ornamental and agricultural crops. Previous studies on natural populations have revealed substantial intraspecific morphological, physiological, and genetic diversity. The field isolate B459 exhibits a distinctive mycelial phenotype, characterized by the lack of sporulation and sclerotia formation, and is unable to infect Vitis vinifera or Phaseolus vulgaris leaves, gerbera petals and tomato fruits. Genetic analysis traced these phenotypes to a mutation in the transcription factor gene Bcin04g03490, introducing a prema

The double policy squeeze on nursing faculties: A critical policy analysis of higher education reform, nursing workforce expansion and nurse educator sustainability.

Nursing workforce policy now asks education systems to expand domestic nurse supply while preparing graduates for older populations, community and primary care, digital health and more complex care pathways. At the same time, universities are expected to demonstrate curriculum innovation, student-centred teaching, quality assurance and institutional performance. The educator capacity required to carry this combined agenda is often assumed rather than planned. This critical policy analysis develops the concept of the double policy squeeze. Using China as an illustrative policy context and linki

Divergent responses of the gill, hepatopancreas, and eyestalk to acute alkalinity stress in Penaeus vannamei: Osmoregulatory compromise, metabolic trade-off, and endocrine disruption.

The expansion of aquaculture into inland saline-alkali waters is constrained by high carbonate alkalinity (CA), a severe environmental stressor for crustaceans. However, the systemic molecular mechanisms underlying its lethal toxicity remain poorly understood. In this study, we employed a comparative transcriptomic approach to investigate the tissue-specific responses of Pacific white shrimp, Penaeus vannamei, under acute lethal stress (48 h-LC50). We focused on three functionally distinct organs: the gill, hepatopancreas, and eyestalk. The results revealed a systemic but highly tissue-specifi
