Diastolic murmurs in the presence of Starr-Edwards mitral prosthesis.

Diastolic murmurs associated with the Starr-Edwards mitral prosthesis have not been described previously. In this report, five patients with mitral prostheses are described in whom apical mid-diastolic and presystolic murmurs resulted from two different causes. Three patients had clots obstructing the prosthetic orifice. The other two had normally functioning protheses and moderately severe aortic insufficiency. The occurrence of mid-diastolic and presystolic murmurs in the presence of a normally functioning prosthetic mitral valve demonstrates that 1) the mid-diastolic Austin Flint murmur can

Transcatheter pulmonary valve implantation using the Edwards SAPIEN transcatheter heart valve.

BACKGROUND: Conduits placed in the right ventricular outflow tract (RVOT) have limited longevity which often requires increasingly complex reoperations. Transcatheter pulmonary valve implantation improves conduit hemodynamics through a minimally invasive approach. We present data for 7 patients treated with the Edwards SAPIEN transcatheter heart valve (THV). PATIENTS: Patients' ranged in age from 16 to 52 years, one was female, and all had NYHA class II-III symptoms. Patients had pulmonary homografts that had been placed 2-25 years earlier during the Ross procedure (n = 4), repaired double ou

Rare complication of ventricular septal defect in three patients following transcatheter aortic valve replacement.

Transcatheter aortic valve replacement (TAVR) is a highly-effective but technically challenging procedure. Despite improvement in device technology and operator techniques, complications are common and previously unknown procedural-related complications continue to arise. In this report, we present a case series of three patients with acquired perimembranous ventricular septal defects following transfemoral TAVR with an Edwards SAPIEN prosthesis.

Percutaneous closure of an iatrogenic ventricular septal defect following mechanical aortic valve replacement using the transseptal technique.

Iatrogenic ventricular septal defects (VSDs) following aortic valve replacement are relatively uncommon and are challenging to close percutaneously given their typical proximity to the prosthetic valve. We describe the successful percutaneous closure of an iatrogenic VSD in a patient with a recent mechanical AVR using the transseptal approach.

Multimodality noninvasive imaging for transcatheter aortic valve implantation: a primer.

Transcatheter aortic valve implantation (TAVI) has recently emerged as a treatment option for patients with severe aortic valve stenosis (AS). For patients who are deemed inoperable for surgical aortic valve replacement (SAVR), TAVI has a significant mortality benefit compared to medical therapy. This review discusses established and emerging roles for multimodality imaging and focuses on the application of these technologies for patient selection, intraprocedural guidance, and the detection and quantification of acute and chronic complications of this novel procedure.

Selective antegrade cardioplegic perfusion complicated by left main stem dissection.

Selective antegrade coronary artery perfusion is a commonly used procedure to obtain myocardial preservation during cardiac surgery. This report describes a patient operated for severe aortic valve stenosis and insufficiency, mitral valve and tricuspid insufficiency. Cardioplegia was administered by selective antegrade coronary artery blood perfusion. Antegrade blood cardioplegia was complicated by dissection of the left coronary main stem. The dissection induced a myocardial infaction and the patient finally died due to heart failure.

Results of triple valve replacement: perioperative mortality and long term results.

Between 1970 and 1984, 37 patients underwent simultaneous triple valve replacement by the same surgeon, all for rheumatic valve disease. Björk-prosthesis were used for aortic, Starr, Björk or St. Jude for mitral and bioprosthesis for tricuspid replacements. The median follow-up was 68 months (range, 2 months to 9.6 years). Two patients died in the first postoperative month (5.4%). Most late deaths were of cardiac origin. 85% of all deaths were in Functional Class IV. Five and ten year actuarial survival rate was 75% and 58% for the entire group. Twenty three long-term survivors were followed;

Non-invasive assessment of the Ionescu-Shiley pericardial xenograft heart valve.

This study was undertaken to describe the auscultatory, phonocardiographic, M-mode and two-dimensional echocardiographic features of the Ionescu-Shiley prosthesis in a group of unselected subjects. Twenty-seven valves were studied - 15 aortic, 10 mitral and 2 tricuspid. An opening sound was noted by phonocardiography and auscultation from the majority of valves in the three positions. A mid-systolic murmur was heard from 14 aortic valves and two had regurgitant murmurs. Three mitral valves had apical diastolic murmurs and two of these had mitral regurgitation. Two-dimensional echocardiography

Evaluation of mechanical prosthetic valves: the role of three dimensional echocardiography in calculating effective orifice area in obese vs non-obese individuals.

Calculation of effective orifice area (EOA) is crucial for the evaluation of prosthetic valve (PV) function and there is lack of data on the best method, particularly in obese patients, in whom two-dimensional (2D) transthoracic echocardiography (TTE) is cumbersome. We sought to compare two methods of calculating EOA through Continuity equation; one using standard 2D-TTE and other three-dimensional (3D) stoke volume (SV), in patients with bileaflet mechanical PV stratified by body mass index (BMI). On conventional TTE, SV mas measured using standard 2D derived data and 3D derived SV in 38 aort

Transcatheter Aortic and Mitral Valve Implantation (TAMVI) in Native Rheumatic Valves.

A 68-year-old female with moderate to severe aortic stenosis and severe mitral stenosis, deemed too high risk for surgery (STS mortality risk = 12.3%) with a porcelain aorta, was successfully treated with a transcatheter aortic and mitral valve implantation (TAMVI) via a transapical approach. A 23 mm Sapien valve (Edwards Lifesciences, Irvine, CA, USA) was placed in the aortic position and a 29 mm inverted Sapien valve (Edwards Lifesciences) in the mitral position.

Simultaneous transaortic transcatheter aortic valve implantation and off-pump coronary artery bypass: An effective hybrid approach.

INTRODUCTION: Transcatheter aortic valve implantation (TAVI) techniques are increasingly being adopted into clinical routine for various risk groups. Coronary artery disease (CAD) is seen in up to 75% of patients with severe aortic valve stenosis (AS) presenting with typical angina pectoris. Due to high mortality rates and procedural complications in these patients, a hybrid concept of simultaneous transaortic TAVI and off-pump coronary artery bypass (OPCAB) can be a feasible treatment option. METHODS: Between April 2014 and July 2020, 10 consecutive high-risk patients underwent concomitant t

Early results from a prospective, single-arm European trial on decellularized allografts for aortic valve replacement: the ARISE study and ARISE Registry data.

OBJECTIVES: Decellularized aortic homografts (DAH) may provide an additional aortic valve replacement option for young patients due to their potential to overcome the high early failure rate of conventional allogenic and xenogenic aortic valve prostheses. METHODS: A prospective, European Union-funded, single-arm, multicentre, safety study was conducted in 8 centres evaluating non-cryopreserved DAH for aortic valve replacement. RESULTS: One hundred and forty-four patients (99 male) were prospectively enrolled between October 2015 and October 2018, mean age 33.6 ± 20.8 years; 45% had undergone
