CHARGE syndrome: diagnosis and clinical management in the NICU.

CHARGE syndrome is a condition that has historically been diagnosed on the basis of the clinical findings of coloboma, heart disease, choanal atresia, restricted growth, and/or central nervous system anomalies, genital hypoplasia, and ear anomalies and/or deafness. Recently, researchers have discovered a genetic link, specifically, a strong association between the CHARGE phenotype and a mutation of the CHD 7 gene on the long arm of chromosome 8. Diagnosis now can be confirmed but not excluded with a positive mutation of this gene. This article offers an explanation of the diagnostic process as…

[Clinical analysis of 48 cases with Mondini dysplasia].

OBJECTIVE: To improve the ability to diagnosis the Mondini dysplasis, 48 cases of Mondini dysplasis were reported. METHOD: The clinical data of 48 cases were collected. The clinical manifestation and the examination of CT and MRI and treatment of the Mondini dysplasia were discussed. RESULT: Most of them were deaf when they were babies. The prolonged speaking and unsteading were important clinical feature. CONCLUSION: The diagnosis of the disease is based on the examination of the hight resolution tomography and magnetic resonance image of the temporal bone. Hearing aid is common management

[Anterior lenticonus: diagnostic aid in Alport syndrome].

Anterior lenticonus is a rare condition in which the lens presents a conical protrusion of its anterior cortex. Sometimes isolated, it may be an ocular manifestation of Alport syndrome, a hereditary nephritis accompanied by deafness and other ocular signs such as flecked retinopathy or posterior polymorphous corneal dystrophy. All these manifestations are a result of a genetic defect in type IV collagen, a major component of basal membranes in the human body. The clinical aspects and therapy of anterior lenticonus and Alport syndrome are described, as well as the exploration that must begin wh…

Acoustic reflex in patients with cochlear implants (analog stimulation).

Stapedius reflexes were tested in 21 deaf patients with the Vienna cochlear implant using intra- or extracochlear electrodes. Sinusoidal bursts of 125, 500, 1,000 and 2,000 Hz were used as stimuli presented in a randomized sequence. The contralateral acoustic reflex was monitored by synchronized sampling of the acoustic impedance of the ear. Results obtained were compared to psychoacoustic sensation values. The reflex threshold always was located close to the uncomfortable loudness level within the subjective dynamic range, or at least above the most comfortable loudness level. Practical appli

[Prescribing hearing aids in ENT practice. I. General principles of prescription and hearing aid technics].

If it is no longer possible to improve hearing by conservative or surgical measures, provision of a hearing aid to the deaf patient remains an important task for the Otorhinolaryngologist. 98% of hearing aids are prescribed at the expense of the Statutory Health Insurance, 90-95% of them by Otorhinolaryngologists in private practice. The limits of the indication for the provision of hearing aids are explained, with attention to the principles of cash limits or by the Social Security system. The contractually regulated division of responsibility between the Otorhinolaryngologist and the hearing

Incorporation of parental suspicions in screening infants' hearing.

In 1978, in the course of a trial, health visitors in the North and South Nottingham Health Districts asked parents on two occasions 'Do you think your child is hearing normally?'. In that and the following year there was an abrupt increase in referrals of children under one year of age suspected of hearing impairments, which was not observed in the other Health Districts of the Nottinghamshire AHA. There was also an increase in the number of children under one year who were confirmed as having impaired hearing or needing surveillance. It is concluded that the active questioning by health visi

Recent advances in neuro-otology.

Four advances in the field of neuro-otology are described: (1) auditory brain-stem response audiometry, (2) electroneuronography, (3) the cochlear implant, and (4) the infratemporal fossa approach to the skull base. Auditory brain-stem response audiometry is an accurate method for the early detection of hearing impairment and early diagnosis of acoustic neurinomas. Electroneuronography accurately assesses the degree of injury to the facial nerve, thereby improving management of facial nerve lesions. The cochlear implant restores the sensation of sound to totally deaf patients. The infratempora

Waardenburg syndrome: A rare case.

Waardenburg Syndrome is a rare disorder of neural crest cell development. It is genetically inherited. Varying in prevalence from 1:42000 to 1:50,000, it compromises approximately 2-5% of congenital deaf children. The syndrome is not expressed in its complete form, in about 20% cases, which adds for its heterogenisity . Even among people affected in the same family, the features do vary. Unilateral heterochromia that manifests as lighter pigmentation of one iris is associated with Waardenburg syndrome and Parry-Romberg syndrome and less commonly with Hirschsprung disease. A case of ten yrs. old

[Good practices with tinnitus in adult].

The interrogation is essential to trace the genesis of the tinnitus and to appreciate its repercussion. Clinical examination should look for a local, vascular or cervical cause. The ENT consultation with audiogram and tinnitus evaluation is essential to know the characteristics of the tinnitus and to consider the treatment. If tinnitus is accompanied by a decrease in hearing, then wearing hearing aids can correct deafness and decrease the tinnitus. The psychological impact of tinnitus must always be taken into account and the use of a multidisciplinary team is an interesting solution. Medical …

The qualitative similarity hypothesis.

Evidence is presented for the qualitative similarity hypothesis (QSH) with respect to children and adolescents who are d/Deaf or hard of hearing. The primary focus is on the development of English language and literacy skills, and some information is provided on the acquisition of English as a second language. The QSH is briefly discussed within the purview of two groups of cognitive models: those that emphasize the cognitive development of individuals and those that pertain to disciplinary or knowledge structures. It is argued that the QSH has scientific merit with implications for classroom
