Diagnostic difficulties in 44 cases of tuberculosis of hip are emphasized and the serious nature of sequelae stressed. A definite diagnosis (cytology positive for Kochs bacillus, positive biopsy) was possible in only 18 cases, presumption of the nature of the affection in the other cases depending on clinical, radiologic and biologic arguments and failure to respond to immobilization one month.

Phocaeicola (Bacteroides) dorei is a Gram-negative anaerobic bacillus that is rarely isolated from human specimens. Its accurate identification can be hampered by its close taxonomic relationship with Bacteroides vulgatus. We report on two patients with bacteremia due to P. (B.) dorei, which was initially identified as B. vulgatus by MALDI-TOF MS.

Human anthrax cases necessitate rapid response. We completed Bacillus anthracis nanopore whole-genome sequencing in our high-containment laboratory from a human anthrax isolate hours after receipt. The de novo assembled genome showed no evidence of known antimicrobial resistance genes or introduced plasmid(s). Same-day genomic characterization enhances public health emergency response.

An orthopedic hardware infection with Clostridium disporicum is described. C. disporicum is a gram positive anaerobic bacillus which can contain two subterminal spores. C. disporicum had not previously been reported in musculoskeletal infections. Gram stains demonstrating gram positive bacilli with two subterminal spores should alert practitioners to the possibility of C. disporicum infection.

T helper (Th) 1 and 17 cells play important roles in host protective responses against tuberculosis. Early Secretory Antigenic Target 6; a Region of Difference 1 (RD1) encoded protein, mounts Th17-responses in the lung. Therefore, lack of RD-1 region makes Bacillus Calmette-Guérin (BCG) less vaccine efficacious than parent strains.

Mycobacterium bovis bacillus Calmette-Guérin (BCG) is the only vaccine available against tuberculosis, and the strains used worldwide represent a family of daughter strains with distinct genotypic characteristics. Here we report the complete genome sequence of M. bovis BCG Moreau, the strain in continuous use in Brazil for vaccine production since the 1920s.

Leclercia adecarboxylata, a motile, gram-negative bacillus of the enterobacteriaceae family, is rarely recovered as a pathogen. We report an exceptionally rare case of L. adecarboxylata endocarditis complicated by embolization of the kidney and spleen in a 48-year-old woman with endometrial cancer. After 4 weeks of antimicrobial treatment, she recovered without sequelae.

Tuberculosis is an important cause of mortality due to its high prevalence, considering that one third of the worlds population is infected with the tuberculosis bacillus. We report the first case of carcinomatous lymphangitis associated with active pulmonary tuberculosis. Carcinomatous lymphangitis is a rare event that may be confounded with tuberculosis because of its radiographic and clinical characteristics.

This is the first identified case of Mycobacterium bovis bacillus Calmette-Guerin (BCG)-derived cutaneous tuberculosis that localizes at a place different from the vaccination site in hosts without immune deficiency. A healthy baby with a developing abscess is described. A multiplex PCR identified the abscess as originating from M. bovis BCG Tokyo 172.

Sphingobacterium hotanense is a gram-negative bacillus identified in 2013 from soil samples that rarely causes infection in humans. We describe 2 cases of S. hotanense bacteremia secondary to skin and soft tissue infection in immunocompromised patients in Minnesota, USA, highlighting S. hotanense as a potential pathogen in immunocompromised hosts with environmental exposure.

Two articles in Immunity and Cancer Cell describe that bladder cancer immunotherapy with Bacillus Calmette-Guerin (BCG) alone or in combination with β-glucan, another agonist of trained immunity (TRIM), involves reprogramming of bone marrow (BM) hematopoiesis. These findings provide additional evidence of the therapeutic potential of BM-mediated TRIM against cancer.

Yersinia pseudotuberculosis is a rare Gram-negative bacillus that cause enterocolitis and terminal ileitis. We report the first Danish case with Y. pseudotuberculosis multiple pyogenic liver abscess presenting with 6 weeks intermittently fever, fatigue, and weight loss. The patient was successfully treated with percutaneous drainage and intravenous piperacillin/tazobactam and oral ciprofloxacin.

Melioidosis is predominantly a tropical disease caused by Burkholderia pseudomallei, a soil-dwelling gram-negative, aerobic bacillus that is distributed primarily in southeast Asia and northern Australia. In this pictorial essay, we provide an illustrated review and conceptual framework of the protean imaging features of this infection, along with a brief description of its clinical manifestations and demographic features.

Treatment with Bacillus Calmette-Guérin vaccine is mainly in use for superficial bladder cancer, for which it is given intravesically. Overall, this therapy is safe with only minor side effects. Arthritis secondary to such therapy has been rarely reported and usually responds well to NSAID treatment. The mechanisms leading to such a complication, as well as the clinical data, are discussed.

A patient with Whipple's disease aged 34 years is presented. At muscle biopsy, the patient presented with tubular aggregates of the quadriceps muscle. The patient did not exhibit any clinical and electromyographical indications of muscular involvement nor direct or indirect histological indications of muscular infiltration with Whipple's bacillus. The possible etiopathogenetic factors creating these aggregates are discussed.

Sadly, despite the discovery of the tuberculosis bacillus over a century ago by Robert Koch, tuberculosis remains a major killer and modern day plague. Progress in the eradication of tuberculosis has been very slow and will require determined efforts on multiple fronts to make substantial inroads to lower the currently stagnant incidence of around 2% globally.

Sphingobacterium spiritivorum, a Gram-negative bacillus, is abundant in nature and is rarely involved in causing human infections. However, it is intrinsically resistant to many commonly administered antibiotics and can thus be a life-threatening microorganism. We describe a case of an 89-year-old Caucasian man who presented with sepsis from S. spiritivorum cellulitis.

A case of tuberculosis in a pediatric AIDS patient, submitted to bronchoscopy and transbronchial biopsy as a part of chronic pulmonary disease evaluation, is presented. No AFB (acid-fast bacillus) detection was identified by gastric lavage before the bronchoscopy, nevertheless after the bronchoscopic procedure the patient became contagious, and persisted with positive baciloscopy until the 56th day of therapy.

We report a rare case of suppurative epididymo-orchitis caused by Pseudomonas pseudomallei in a 56-year-old male. This is a gram negative bacillus found mainly in tropical zones. Diagnosis was reached by culture of the organisms after drainage of the scrotal abscess, and the patient was treated by a course of oral chloramphenicol 500 mg qid for 6 months.

Pericardial involvement with legionnaires' disease is rare. Pericarditis with legionnaires' pneumonia developed in a patient with previous mediastinal irradiation for Hodgkin's disease. Subsequently, the patient had progressive symptoms of pericardial constriction that required pericardiectomy. Acute infection with agents such as legionnaires' bacillus may precipitate the late appearance of pericardial disease in patients with previous mediastinal irradiation.
