Reconstruction of a Devastated Ureter: Multi-institutional Experience with Robotic Intracorporeal Ileal Ureter Replacement.

BACKGROUND AND OBJECTIVE: To report our multi-institutional outcomes with robotic intracorporeal ileal ureter replacement (RIUR). METHODS: We performed a retrospective review of patients who underwent RIUR at three institutions from April 2016 to June 2025. Surgical success was defined as the absence of flank pain and of ureteral obstruction on functional imaging and/or endoscopic evaluation, and freedom from operative intervention related to RIUR at ≥6-mo follow-up. KEY FINDINGS AND LIMITATIONS: Of 48 patients, 31 (64.6%) had unilateral and 17 (35.4%) had bilateral reconstruction. Twenty (4

The role and mechanism of diosgenin glucoside in modulating megakaryopoiesis through promotion of autophagy via the AMPK/mTOR/ULK1 signaling pathway.

BACKGROUND: Radiation-induced thrombocytopenia (RIT) is a severe, dose-limiting complication of cancer radiotherapy with limited clinical management options. This study investigated the therapeutic efficacy and molecular mechanism of Diosgenin glucoside (DG), a natural constituent uniquely prioritized through a graph-based deep learning virtual screening pipeline utilizing the directed graph intersection network (DGIN) architecture. METHODS AND RESULTS: In vitro evaluations in Meg-01 and K562 cells demonstrated that non-cytotoxic concentrations of DG effectively promoted megakaryocytic lineag

Stereotactic radiosurgery for pituitary metastases: a systematic review and single-arm meta-analysis of local control and survival outcomes.

BACKGROUND: Pituitary metastases (PM) are rare intracranial secondary tumors, most often arising from breast and lung primaries. They are clinically significant due to their association with advanced systemic malignancy and potential to cause visual, neurological, and endocrinological morbidity. Conventional therapies such as surgery and fractionated radiotherapy have limited efficacy or high morbidity. Stereotactic radiosurgery (SRS) has emerged as a minimally invasive alternative, but available evidence is limited to small, heterogeneous series. OBJECTIVE: To evaluate the effectiveness and

Density override-based VMAT optimization to improve robustness at tissue-air interfaces in head and neck radiotherapy.

BACKGROUND: Volumetric Modulated Arc Therapy (VMAT) is integral to head and neck (HN) radiation therapy; however, plan robustness may be compromised at air-tissue interfaces. Highly modulated fluence directed at these areas can produce significant unexpected high-dose regions in the event of minor variations in tissue density. PURPOSE: This study characterizes an instability in VMAT planning for HN radiation therapy (RT) occurring at the interface between target volume (e.g. mucosal primary tumor) and internal air spaces (e.g. pharyngeal lumen, sinuses, nasal cavity, etc.). A density-override

Augmenting interferon-β and anti-PD-L1 antibody bolsters non-ablative radiotherapy to leverage potent abscopal effect in metastatic cancers.

BACKGROUND: The abscopal effect is a systemic anti-tumor immune response induced by local radiotherapy (RT), resulting in regression of distant, non-irradiated tumors. Although ablative RT is more effective than conventionally fractionated RT in inducing this response, the underlying mechanisms remain incompletely understood, and strategies to enhance the abscopal effect following conventional RT are still needed. METHODS: We compared the immunological and therapeutic effects of ablative RT (15 Gy in a single fraction) and non-ablative RT (4 Gy in 10 fractions; biologically equivalent to 40 G

The efficacy of metastasis-directed external beam radiotherapy for castration-resistant prostate cancer: A retrospective multicenter study.

BACKGROUND: To assess the efficacy of metastasis-directed external beam radiotherapy (MDT) in patients with castration-resistant prostate cancer (CRPC), we conducted a multicenter retrospective study. MATERIALS AND METHODS: We retrospectively analyzed data from patients with metastatic CRPC treated with MDT between January 2013 and July 2023 across 14 hospitals. Patients who received palliative or local radiation therapy or had insufficient clinical data were excluded. The primary endpoint was the change in prostate-specific antigen (PSA) levels from pre- to post-MDT. Secondary endpoints incl

Non-convex IMRT treatment planning using deep inverse optimization.

BACKGROUND: Intensity-modulated radiation therapy (IMRT) utilizes inverse optimization to calculate beamlet intensities, achieving clinical dose objectives for tumors while minimizing dose to organs-at-risk (OARs). However, incorporating non-convex dose-volume constraints into IMRT planning is challenging due to their combinatorial nature and computational complexity. PURPOSE: This study explores a novel approach to IMRT fluence map optimization (FMO) by leveraging deep inverse optimization (DIO). Our method aims to approximate non-convex constraints efficiently while preserving convexity, im

Case Report: Palliative radiotherapy monitoring using computed tomography and diffusion-weighted magnetic resonance imaging in a cat with ceruminous gland adenocarcinoma.

BACKGROUND: Ceruminous gland adenocarcinoma is the most common malignant tumor of the feline external ear canal. These tumors are often locally invasive and may extend into adjacent musculature and lymph nodes, limiting the feasibility of surgical excision. When surgery is not an option, radiation therapy can provide palliative benefit. Advanced imaging modalities, including computed tomography (CT) and magnetic resonance imaging (MRI), play a crucial role in treatment planning and therapeutic monitoring. CASE DESCRIPTION: A 14-year-old spayed female Persian cat with a confirmed diagnosis of

Stereotactic body radiation therapy for inoperable non-metastasized pancreatic adenocarcinoma: a randomised phase II study (TORPEDO).

BACKGROUND: Patients with pancreatic ductal adenocarcinoma face a poor prognosis, with radical resection being the only potential cure. Because symptoms typically appear late, many patients are diagnosed with advanced disease. Locally advanced pancreatic tumours are characterised by extensive vascular involvement, which precludes R0 resection. Systemic therapy is therefore indicated, not only for palliation but also for reducing the risk of metastatic disease. Locoregional control remains paramount, irrespective of distant metastases as progressive tumours can cause considerable morbidity, neg

Local and systemic biodistribution of a small-molecule radiopharmaceutical probe after transcatheter embolization and intra-arterial delivery in a porcine orthotopic renal tumor model.

BACKGROUND: Small-molecule biomacromolecules target tumor-specific antigens. They are employed as theranostic agents for imaging and treatment. Intravenous small-molecule radioligands exhibit rapid tumor uptake and excretion. However, systemic administration for peptide receptor radionuclide therapy still lacks the therapeutic index to completely treat solid tumors beyond palliation. We study intra-arterial delivery with tumor embolization of a small molecule as a means to deliver local intertumoral brachytherapy for curative internal ablation. RESULTS: 18F-fluorodeoxyglucose (FDG) was used a

Breast radiation therapy fluence painting with multi-agent deep reinforcement learning.

BACKGROUND: The electronic compensation (ECOMP) technique for breast radiation therapy provides excellent dose conformity and homogeneity. However, the manual fluence painting process presents a challenge for efficient clinical operation. PURPOSE: To facilitate the clinical treatment planning automation of breast radiation therapy, we utilized reinforcement learning (RL) to develop an auto-planning tool that iteratively edits the fluence maps under the guidance of clinically relevant objectives. METHODS: With institutional review board (IRB) approval, 70 patients treated with 6MV tangential

A clinical decision instrument to safely reduce abdominopelvic CT use in minor blunt trauma patients.

BACKGROUND: Abdominopelvic (A/P) computed tomography (CT) is frequently used in trauma evaluation, yet often yields clinically insignificant findings, especially in minor blunt trauma. Overuse of CT is associated with radiation exposure, contrast risks, and unnecessary healthcare costs. OBJECTIVE: To derive a clinical decision instrument (DI) to guide selective A/P CT imaging in adult patients presenting with minor blunt trauma. METHODS: We conducted a retrospective cohort study of adults presenting to the emergency department with minor blunt trauma and underwent A/P CT in the emergency dep

Immune-modulatory effects of low-dose radiotherapy through macrophage polarization and transcriptional rewiring in triple-negative breast cancer.

BACKGROUND: Despite recent advancements with immune checkpoint inhibitors, a significant proportion of patients, particularly those with triple-negative breast cancer, exhibit resistance to these therapies. To address this challenge, we investigated whether adding low-dose-per-fraction radiotherapy (LDRT) could remodel the tumor microenvironment (TME) and enhance the efficacy of PD-1 blockade. METHODS: Luciferase-expressing 4T1 breast-cancer cells were inoculated subcutaneously at multiple sites in 6-week-old BALB/c mice. The mice were divided into seven groups: (i) untreated control; (ii) an

Radiosensitization effect of iridium (III) complex on lung cancer cells via mitochondria apoptosis pathway.

BACKGROUND: Lung cancer is the leading cause of cancer-related death in the worldwide. Although cisplatin and other platinum-based drugs are widely used as radiosensitizers in radiotherapy and considered the first-line treatment for advanced lung cancer, their clinical utility is often limited by drug resistance and severe cytotoxic side effects. In recent years, iridium-based complexes and other transition metal cation complexes with similar structural properties have garnered increasing research interest due to their potential anticancer properties. METHODS: Recently, we synthesized a novel

Are conventional and hypofractionated chemoradiotherapy comparable in glioblastoma patients?

BACKGROUND/OBJECTIVES: Hypofractionated radiation therapy (Hypo-RT) schedules may offer radiobiological, patient convenience, and healthcare resource advantages over standard fractionated radiation therapy (S-RT) for glioblastoma (GBM). Additionally, simulated integrated boost (SIB) Hypo-RT is proven to be an effective and safe treatment. We report on our experience regarding progression-free survival (PFS), overall survival (OS), and RT-related toxicities in GBM patients treated with Hypo-RT and S-RT. METHODS: Patients with IDH-wild-type GBM received either Hypo-RT (40.05-52.5 Gy/15 fraction

Hemodialysis-associated radioactive waste management in [131I]I and [177Lu]Lu radionuclide therapy.

BACKGROUND: Radionuclide therapy, such as with iodine-131 or lutetium-177-labeled substances, is used to treat thyroid cancer and neuroendocrine tumors. Due to the incorporated radioactivity, patients must remain hospitalized until radiation levels are deemed safe, according to national regulations. This is particularly problematic for patients who require hemodialysis (HD). While contamination of dialysis waste after radionuclide therapy is generally well recognized, a critical gap remains regarding its duration and extent. This poses risks for patient management and outpatient staff. PURPOS
