Eligibility for Liver Transplantation in Patients with Intrahepatic Cholangiocarcinoma.

PURPOSE: Intrahepatic cholangiocarcinoma (iCCA) has been a contraindication for liver transplantation due to frequent recurrence and poor survival. We sought to determine the true proportion of transplantation-eligible iCCA patients and their outcomes without transplantation. METHODS: Data from patients evaluated for iCCA at two academic medical centers between 2008 and 2018 were analyzed retrospectively. Overall survival was determined for patients categorized as eligible for liver transplantation based on six criteria: unresectability, no extrahepatic disease, stable disease after chemother

Clinical management of cancer after kidney transplantation.

Advances in immunosuppressive therapy have markedly improved short-term graft survival post-kidney transplantation (KT); however, these advances have also profoundly altered long-term disease patterns in transplant recipients. Cancer remains a major barrier limiting improvements in long-term survival. Kidney transplant recipients are at increased risk of malignancy owing to impaired graft function and immunosuppressive medications. To improve cancer-related outcomes, comprehensive strategies encompassing prevention, early detection through screening, and appropriate treatment are required. Pri

The Diversity of Transplant Glomerulitis and its Relationship to Alloantibody.

INTRODUCTION: Although transplant glomerulitis often signifies antibody-mediated rejection (AMR), the pathophysiology of complement degradation split-product 4d (C4d) staining of peritubular capillaries (C4dptc) and donor-specific antibodies (DSAs) negative cases is unclear. METHODS: We compared Banff 1997 defined glomerulitis (n = 271, from 4300 biopsies) using serological and pathological dichotomization, against rejection without glomerulitis (g0REJ, n = 612) and normal controls (NIL, n = 1442). RESULTS: Epidemiological risk factors were female recipients, living donation, early T-cell-me

Management of Surgical Complications in Pediatric Kidney Transplantation.

Introduction: Graft and patient survival after kidney transplantation in children has increased in the past decade; however, post-transplant surgical complications occur in up to 15.4% of recipients and pose a significant threat to graft survival. Due to anatomic discrepancies in children, kidney transplantation in this population is nuanced and requires meticulous planning. This narrative review summarizes the most common postoperative surgical complications following kidney transplantation in children. Methods: PubMed and Google Scholar were queried for full-text articles that reported pedia

The APOL1 gene and kidney transplantation: a review article.

INTRODUCTION: Chronic kidney disease (CKD) is a global public health problem and kidney transplantation is the treatment of choice for patients with stage 5 (G5) CKD. Kidney transplant survival at the end of the first year exceeds 95%, and at 5 years, it approaches 90%. However, the decline in the estimated glomerular filtration rate (eGFR) over the years remains a challenge. The rate of eGFR decline is attributed to several factors and has recently been attributed to the presence of APOL1 gene variants. DISCUSSION: The risk variants G1 and G2 of the APOL1 gene are related to CKD. Evidence su

The Role of Natural Killer Cells in Kidney Transplantation.

Natural killer (NK) cells are increasingly recognized as central effectors in kidney allograft injury, extending beyond their traditional role in innate immunity. This narrative review summarizes current evidence linking NK cells to microvascular inflammation, with a focus on both antibody-dependent and antibody-independent rejection. In antibody-mediated rejection, NK cells may be activated through Fc gamma receptor IIIa (CD16a) engagement with endothelium-bound donor-specific antibodies, leading to antibody-dependent cellular cytotoxicity and proinflammatory cytokine release. In parallel, em

The Role of Microsurgery in Living Donor Liver Transplantation.

Liver transplantation (LT) is an accepted curative therapy to save select patients with end-stage liver failure, congenital liver diseases affecting the liver or bile duct, and hepatocellular carcinoma. Living donor liver transplantation is an indispensable and alternative choice to manage the scarcity of deceased liver grafts and to provide a quick way to access a healthy graft. Successful hepatic artery reconstruction for liver transplantation is crucial for outcomes. Difficult hepatic artery reconstructions (HAR) are not uncommon in LT, especially in living donor liver transplantation (LDLT

Immunopathogenesis of antibody-mediated rejection in liver transplantation.

Antibody-mediated rejection (AMR) after liver transplantation (LT) is increasingly recognized as a cause of graft dysfunction, chronic injury, and graft loss, but remains difficult to diagnose because serologic, histologic, and clinical findings are often incomplete or overlapping. This review focuses specifically on liver AMR and distinguishes liver-specific evidence from mechanisms inferred from experimental models or other solid-organ transplants. We summarize how donor-specific antibodies (DSA), complement activation, Fc receptor-mediated effector mechanisms, endothelial activation, and mi…

Pain syndromes in transplantation: the role of the gut microbiome.

PURPOSE OF REVIEW: The purpose of this review is to discuss common pain phenotypes in transplantation, summarize known gut microbiome features associated with chronic pain, and to map known gut microbiome features in transplantation with diagnosis-independent pain features. RECENT FINDINGS: Persistent pain is common across solid organ and hematopoietic stem cell transplantation, arises from diverse mechanisms, and often extends well beyond the perioperative period. Growing evidence supports the gut microbiome as a biologically plausible modulator of chronic pain across disease states, includi

Severe Postoperative Bleeding after Heart Transplantation.

BACKGROUND: Postoperative bleeding is frequent in cardiac surgery, but its incidence, risk factors, and consequences remain largely unknown after heart transplantation. The main objective of this study was to describe the incidence of severe bleeding complications after adult heart transplantation. METHODS: The authors conducted an observational study including all adult patients who received a heart transplant between 2015 and 2022 in two French referral centers. The primary endpoint was the incidence of severe bleeding complications defined by a Universal Definition of Perioperative Bleedin

Outcomes of lung transplantation surgery performed at night.

OBJECTIVE: The logistics and planning involved in lung transplantation (LuTx) are complex. Recent advancements in LuTx surgery have introduced the possibility of deferring surgery to daytime hours and might reduce the burden on operating personnel and improve patient outcomes; however, this might interfere with the daytime program and could be labor-intensive and costly. Therefore, this study sought to investigate the effect of time of surgery on LuTx outcomes. METHODS: Adult patients undergoing LuTx between 2010 and 2022 were included in this single-center retrospective cohort study. Patient

Therapeutic Potential of Mitochondrial Transplantation with Focus on DBD.

Diabetic Bladder Disease (DBD), a common urological complication of diabetes mellitus, severely compromises the quality of life of affected patients. Mitochondria, the primary energy-producing organelles in cells, are closely correlated with the pathogenesis and progression of DBD. As an emerging therapeutic modality, mitochondrial transplantation exhibits substantial potential for the management of DBD. This paper presents a comprehensive review of mitochondrial transplantation, with a focus on its fundamental theories, application conditions, safety profiles, and mitochondrial sources. Subse

Managing antibody-mediated rejection in heart transplant.

INTRODUCTION: Antibody-mediated rejection (AMR) is a major obstacle after heart transplantation. Although its pathophysiology is now clearer than a decade ago, clinical management is still fragmented and poorly evidence-based. AREAS COVERED: Recent work reframes the classic paradigm of donor-specific antibodies (DSA) dependent complement activation as only one axis of injury. Converging data place natural killer (NK) cells at the center of vascular damage: they execute DSA-driven antibody-dependent cellular cytotoxicity (ADCC) via Fcγ receptors and, through 'missing-self' recognition, trigger

Are Renal Cysts a Barrier to Donation and Kidney Transplant?

OBJECTIVES: We evaluated the long-term outcomes of kidney transplants performed using grafts that contain simple renal cysts, to determine whether these cysts constitute a barrier to kidney donation or transplant. MATERIALS AND METHODS: All kidney transplants performed by our team from February 2012 to May 2025 were retrospectively reviewed. Cases in which the graft contained at least 1 simple renal cyst were included. Demographic characteristics, cyst size, perioperative complications, postoperative renal function, and cyst size during follow-up ultrasonography were recorded. Postoperative s…

Liver transplantation: embracing the future with machine perfusion.

Despite the annual increase in liver transplants, a persistent disparity between suitable donors and recipients continues to challenge the transplant community. The introduction of machine perfusion emerged as a novel approach to organ preservation. This technique, which utilizes extended criteria donor grafts, is expected to reduce rejection rates and expand the donor pool. Its principle, like that of any ex vivo machine perfusion, is based on the continuous supply of nutrients and oxygen to maintain cellular metabolism while interrupting anaerobic processes. This helps prevent conditions suc…

Targeting ferroptosis halts graft deterioration in transplantation.

Ischemia-reperfusion injury accelerates graft deterioration and limits the use of marginal donor organs. Recently, Veeckmans et al. showed that pharmacological ferroptosis inhibition preserves liver and lung graft function across porcine and human perfusion platforms, positioning lipid peroxidation-driven cell death as an actionable target in machine perfusion-era transplantation.

[Toe-to-hand transplantation for thumb and finger reconstruction].

Reconstruction of missing thumbs or fingers remains one of the most demanding challenges in hand surgery. Over the past century, toe-to-hand transplantation has evolved from early experimental pedicled transfers into a highly refined microsurgical procedure, now widely regarded as the "gold standard" for restoring hand function. This article reviews the historical and technical development of toe transplantation-from Nicoladoni's pioneering concepts in the late 19th century, through Dr. YANG Dongyue's landmark second-toe transfer performed in 1966, to contemporary Chinese contributions such as

Immunosuppressive microparticles for allogeneic transplantation in nerve repair.

Peripheral nerve damage affects millions of people each year, possibly resulting in long-term functional deficits and chronic pain. Severe nerve injury is largely treated with autologous grafting, which produces donor site morbidity and has limited supply. Although cell therapies and living allografts are promising alternatives, their clinical translation is hampered in part due to their immunogenicity. Local delivery of immunosuppressants via controlled-release microparticles could protect both allografts and cellular constructs, while potentially avoiding the detrimental effects of systemica
