Selective instrumentation, reduction and repair in low-grade isthmic spondylolisthesis.

We treated 32 patients with low-grade (<30%) isthmic spondylolisthesis at L5-S1 with selective instrumentation, reduction of the slip, resection of the pseudoarthrosis adding autologous bone grafting and decompressing the root canal. The ages ranged from 18 to 54 years. After a mean of 3.4 (1-7) years, we found good radiological and clinical results in 27 patients. Five patients lost correction, of whom three were asymptomatic whereas two required fusion of L5-S1. We think this technique is a useful surgical option for select patients.

Reevaluation of routine gastrointestinal decompression after gastrectomy for gastric cancer.

BACKGROUND/AIMS: We reviewed postoperative courses of patients with gastric cancer who underwent gastrectomy to evaluate the need for routine postoperative gastrointestinal decompression. METHODOLOGY: Three hundred patients who underwent gastrectomy during 1998 and 1999 were enrolled in this study. A nasogastric tube was placed in all patients just after induction of the anesthesia. The patients were divided into two groups, 150 patients for each. In group 1, the nasogastric tube was maintained until the passage of flatus per rectum. In group 2, the nasogastric tube was removed immediately af

Snapping annular ligament of the elbow joint in the throwing arms of young brothers.

We examined young brothers with symptomatic snapping elbow in the throwing arm. Arthroscopic examination confirmed the mechanism of snapping, in which loose and protruded annular ligament-like tissue covered the volar half of the radial head in elbow extension and uncovered the radial head in deep elbow flexion. Arthroscopic resection of the annular ligament-like tissue was performed in one brother. Histologic examination of the removed tissue showed degenerated ligament tissue. Excision of loose annular ligament abolished snapping. Contralateral elbows of the brothers also showed similar asym

Hip spine syndrome: management of coexisting radiculopathy and arthritis of the lower extremity.

BACKGROUND CONTEXT: Significant lumbar spinal stenosis and lower extremity arthritis may coexist in the elderly. This combination of lumbar stenosis with radiculopathy and lower extremity arthritis may lead to diagnostic uncertainty. PURPOSE: To describe the findings of hip spine syndrome, a constellation of symptoms with extensive overlap of radiculopathy and lower extremity arthritis. CONCLUSIONS: Evaluation of the patient with lower extremity pain in consideration for total joint arthroplasty should include functional inquiry of the spinal nerves. Diagnostic tests and injections may allow

Instrumentation in spinal surgery.

As a result of rapid advances in the field of spinal instrumentation, the surgeon today has at his disposal many well-designed implants, such as pedicle screws, hooks, rods, plates, and interbody fusion devices. Many of these implants are modular and are used in combination, such as hooks and screws attached to rods, allowing the surgeon flexibility in selecting the appropriate implants to meet the particular needs of the patient. Spinal instrumentation restores or enhances the mechanical stability of the spine, corrects and maintains spinal alignment, and enhances spinal fusion. The disadvant

[Surgical treatment of lumbar spinal stenosis associated with unstable degenerative spondylolisthesis].

OBJECTIVES: To assess surgical treatment of lumbar spinal stenosis associated with unstable degenerative spondylolisthesis. METHODS: In 86 patients with lumbar spinal stenosis associated with unstable degenerative spondylolisthesis. (30 men and 56 women, aged from 30 to 77 years), 63 patients complained of lower back pain with both lower extremity pain, 10 patients pain in one leg, and 13 patients only lower back pain. Seventy-two of these patients complained of intermittent claudication, with a walking distance ranging from 10 to 1 000 m. Thirty-two patients had some changes in sensation, mo

[A case of Hirayama's disease successfully treated by anterior cervical decompression and fusion].

The authors report the case of a 16-year-old boy with Hirayama's disease(juvenile muscular atrophy of unilateral upper extremity). The present history began about 6 months previously, when he noticed slowly progressive weakness with atrophy of the left hand and forearm. Neurological examination on admission revealed diffuse distribution of muscular atrophy including the left hypothenar, thenar, forearm, and triceps muscles. However, EMG studies identified neurogenic changes in both upper extremities. There was no long tract sign of objective sensory impairment. Plain spinal radiograms showed a

String test measurement to assess the effect of spinal deformity correction on spinal canal length.

STUDY DESIGN: Long cassette coronal and lateral radiographs before and after surgical correction were analyzed and string test measurements made by three observers in 55 surgical cases (13 surgical types). OBJECTIVES: The purpose of the study was to assess the effect of various corrective maneuvers for spinal deformity on the spinal canal length. SUMMARY OF BACKGROUND DATA: When perioperative neurologic deficit occurs, the surgeon removes implants because they are displaced into the spinal canal or the canal has been lengthened. It is important to know the effect certain constructs have on c

[Chiari malformation (type I) associated with bilateral abducens nerve palsy: case report].

There are still many questions concerning the pathology and therapeutic strategy for Chiari malformation. We recently encountered a case of Chiari malformation associated with only bilateral abducens nerve palsy which was successfully treated by foramen magnum decompression. The pathology and mechanisms of Chiari malformation (type I) were evaluated in this case. The patient was a 12-year-old girl who had visited our institute 1 month previously complaining of diplopia. She had no particular past or family history and her general appearance was also normal. Bilateral abducens nerve palsy was t

Supraorbital neuralgia. On the clinical manifestations and a possible therapeutic approach.

The clinical manifestations of supraorbital neuralgia are apparently only incompletely known. The lack of awareness of this head pain may possibly be due to its rarity and problems with making the diagnosis. In the present work, the long-term result of minor, decompressive surgery of the supraorbital nerve in five patients is reported. The immediate improvement was good and, after a mean observation time of more than 6 years, an improvement of 50% to 100% was observed (mean, circa 85%). In the two patients with the longest postoperative observation time, approximately 8 years, pain has not rec

Sciatic neuropathy.

Injuries to the sciatic nerve cause neurologic deficits in the peroneal and tibial nerve distributions. Interestingly, most injuries result in more severe deficits to the peroneal division compared to the tibial division. Thus, it can sometimes be difficult to distinguish sciatic neuropathy from peroneal neuropathy. The long course of the sciatic nerve leaves it vulnerable to nerve injury from a variety of causes. Most sciatic neuropathies are acute in onset, such as from hip arthroplasty and hip fracture or dislocation, but some occur from prolonged compression, such as during coma. Entrapmen

[Surgical treatment of low back pain].

Sciatica in low back pain is usually caused by lumbar disc herniation, degenerative spinal disease or lumbar spinal stenosis. Important pathogenetic factors are mechanical compression of and inflammatory changes in nerve roots caused by nucleus pulposus tissue. Decompression of nerve roots and removal of nucleus pulposus tissue are the main surgical goals. Emphasis should be placed on clinical identification of the symptomatic nerve roots. Computerized tomography (CT) and magnetic resonance (MR) imaging are the most important diagnostic tools today. Plain X-ray may be required for correct iden

[Repeat operations for recurrent trigeminal neuralgia after microvascular decompression].

For the study of cause and treatment of recurrent trigeminal neuralgia, repeat operations were performed in 12 patients with recurrent trigeminal neuralgia after microvascular decompression. Among repeat operations, there was vascular loop recompression at trigeminal root in 10 patients, negative exploration in 2 patients. The pain relief was achieved after neurovascular redecompression or neurolysis in all patients. The patients had no recurrent pain during a follow-up from 6 months to 4 years. The causes of recurrent trigeminal neuralgia after microvascular decompression and the questions re

How I do it: endoscopic endonasal transclival resection of ventral foramen magnum meningioma.

BACKGROUND: Surgical management of ventral foramen magnum meningiomas (FMMs) is challenging due to intervening neurovascular structures. The endoscopic endonasal transclival approach provides a direct access, offering a potential route to achieve Simpson Grade 1 resection while minimizing brainstem retraction. METHOD: We detail the technical nuances of a binostril, four-handed EEA for a ventral FMM. We explain the key anatomical landmarks required for safe decompression, including clival drilling limits, the identification of pre-medullary neurovascular structures, and the execution of a mult

Hybrid Endoscopic/Fluoroscopic Gastrostomy Tube Placement: Case Report of a Novel Technique.

INTRODUCTION: We report the case of a 60-year-old female with a history of perforated gastric ulcer, abdominal abscesses, enterocutaneous fistula, and small bowel obstruction requiring durable gastric decompression prior to delayed fistula takedown. The patient had contraindications to or failed attempts at traditional gastrostomy approaches (surgical, endoscopic, interventional radiology). CASE REPORT: Gastrostomy was successfully performed via a novel technique combining interventional radiology and endoscopic placement. DISCUSSION: In certain patients with complex presentations, a hybrid

Acute myelopathy caused by granuloma formation secondary to gelatin sponge: A case report of a rare complication.

BACKGROUND: Gelatin-based hemostatic agents are widely used in neurosurgery due to their effectiveness and general biocompatibility. However, rare adverse events such as foreign body granuloma (FBG) formation can occur. CASE DESCRIPTION: Here, we report the case of a 52-year-old male who developed progressive neurological symptoms compatible with myelopathy following cervical decompression surgery in which Gelfoam® was applied. A FBG was found on imaging and confirmed following surgical excision. CONCLUSION: This case highlights the importance of including FBG in the differential diagnosis o

Cracking the Case: Post-pregnancy Bilateral Femoral Fragility, a Rare Clinical Challenge.

Pregnancy-associated musculoskeletal disorders are relatively rare but can present significant diagnostic and therapeutic challenges. We present a rare case of a 37-year-old woman with a twin pregnancy complicated by type 2 diabetes mellitus and obstetric cholestasis, who developed bilateral hip insufficiency fractures. Despite initial conservative management, her condition worsened, requiring surgical intervention in the form of core decompression and internal fixation. Postoperative recovery was successful, with significant pain relief and improved mobility. This case underscores the complex

Lymphatic Interventions in Congenital Heart Disease.

Lymphatic disorders in congenital heart disease can be broadly classified into chest compartment, abdominal compartment, or multicompartment disorders. Heavily T2-weighted noninvasive lymphatic imaging (for anatomy) and invasive dynamic contrast magnetic resonance lymphangiography (for flow) have become the main diagnostic modalities of choice to identify the cause of lymphatic disorders. Selective lymphatic duct embolization (SLDE) has largely replaced total thoracic duct embolization as the main lymphatic therapeutic procedure. Recurrence of symptoms needing repeat interventions is more comm
