Sleep apnea in patients with left ventricular dysfunction: beneficial effects of nasal CPAP.

Five patients with congestive heart failure (CHF) and 1 with left ventricular dysfunction but without CHF were found to have sleep apnea. Central sleep apnea (CSA) related to Cheyne-Stokes respiration was seen in 4 cases while obstructive sleep apnea (OSA) was seen in 2. All patients had symptoms of sleep apnea. Nasal continuous positive airway pressure (NCPAP) was effective in reversing CSA and OSA in all patients with improvement in sleep structure and alleviation of symptoms of sleep apnea. In addition, all experienced a reduction in cardiac dyspnea. This was associated with a 5% or greater

Physiological and geometrical effects in the upper airways with and without mandibular advance device for sleep apnea treatment.

Sleep apnea is a sleep disorder that occurs when the breathing of a person is interrupted during the sleep. This interruption occurs because of the patient has narrowed airways and the upper airways muscles relax, closes in and blocks the airway. Therefore, any forces or reaction originated by the air flow dynamics over the relaxed upper airways muscles could make to close the upper airways, and consequently the air could not flow into your lungs, provoking sleep apnea. Fully describing the dynamic behavior of the airflow in this area is a severe challenge for the physicians. In this paper we

Sleep disordered breathing in the elderly: comparison of women and men.

The prevalence of obstructive sleep apnea syndrome in patients up to the age of 60 is known to be two times higher in men then in women. Hormonal changes during menopause might underlie changes in this relationship in the elderly. This study was designed to detect differences in the type and frequency of sleep-disordered breathing between women and men over the age of 65 years, having the same body mass index. The study was conducted using a matched-pair approach consisting of a sample population of 40 pairs of patients over the age of 65. All patients met the following exclusion criteria: age

Sleep disorders in patients with severe nasal obstruction due to septal deviation.

The role of nasal obstruction in etiology of the sleep apnea disease is controversial in literature. Forty-six subjects (3 women, 43 men) with severe nasal obstruction due to septal deviation were evaluated. The examination included case history, clinical ORL examination by ENT specialist, rhinomanometry and whole night sleep recording (SCSB, oximetry, EKG). Mean age of the patients was 40.0 years (from 17 to 68 years) and body mass index 26.0 kg/m2 (from 20.3 to 34.6 kg/m2). The chief complaint of the patients was a severe nasal obstruction. Thirty-one patients (67%) had also heavy disturbing

Gastroesophageal reflux and apnea in prematurely born infants during wakefulness and sleep.

The hypothesis that acid gastroesophageal reflux may be responsible for the persistence of apnea was tested on 20 prematurely born infants, at a median conceptional age of 38.7 weeks. Gastroesophageal reflux was identified using distal esophageal pH monitoring. Apneas of durations greater than 10 seconds were identified and classified as either central or obstructive and mixed, using recordings of respiration. Wakefulness, active sleep, and quiet sleep were identified using electroencephalography and by assessing eye movements. Of 134 episodes of acid gastroesophageal reflux in the 20 subjects

Evaluation of sleep-disordered breathing. Is polysomnography necessary?

To determine whether polysomnography is necessary to assess the presence and severity of sleep-disordered breathing, bedside observations by physicians were compared with the results of polysomnography in 37 patients with clinically suspected obstructive sleep apnea. Physician observations correlated with objective findings from polysomnography in detecting the presence of obstructive apnea (p less than 0.01), and had a high specificity and positive predictive value. The 20 patients correctly identified by clinical observation had a longer duration of apneic episodes (p = 0.02), increased seve

The Pharyngeal Resistance Index-A Promising Diagnostic Tool in OSA.

This study investigates the newly developed pharyngeal resistance index derived from computational fluid dynamics analysis of the upper airway in patients with obstructive sleep apnea. The aim was to evaluate this index as a diagnostic and severity assessment tool by comparing it with the apnea-hypopnea index before and after maxillomandibular advancement surgery. A retrospective single-center study was conducted at the Department of Oral and Maxillofacial Surgery, Charité-Universitätsmedizin Berlin, including 16 patients with obstructive sleep apnea who underwent maxillomandibular advancement

Post-Concussion Syndrome and Sleep Apnea: A Retrospective Study.

BACKGROUND: Concussion symptoms typically resolve within 7-10 days, but 10%-25% of patients do not fully recover. They can develop post-concussion syndrome (PCS), which includes sleep abnormalities such as obstructive sleep apnea. It is unclear how specific sleep problems manifest in PCS and how it relates to cognition and symptomology. METHODS: A retrospective chart review was conducted on PCS patients seen at the University Health Network (UHN) Concussion Clinic and sent for sleep study. Neuropsychology tests, concussion features, PCS symptoms, and demographics were abstracted from clinical

Prevalence of unspecified sleep apnea and the use of continuous positive airway pressure in hospitalized patients, 2004 National Hospital Discharge Survey.

Diagnosed obstructive sleep apnea affects 2-4% of middle aged Americans and represents a substantial health care burden. Despite its prevalence, little is known about the demographic characteristics or clinical management of sleep apnea patients hospitalized for other comorbidities and surgeries. The aim of this study was to provide a broad characterization of the epidemiology of sleep apnea in hospitalized patients in the United States and to describe the trends in the management of their sleep apnea during their hospitalizations. Using the 2004 National Hospital Discharge Survey (NHDS), a na

Sleep apnea and obesity.

The dramatic increase in the worldwide prevalence of obesity has paralleled the increase in the prevalence of obstructive sleep apnea (OSA). Even with heightened awareness by the lay and medical communities, OSA is still markedly under-diagnosed, as evidenced by the persistent presentation of late-stage cardiovascular complications in obese individuals newly diagnosed with sleep apnea. The clinical sequela of untreated and poorly-treated sleep apnea include conditions that are considered components of the metabolic syndrome for which central obesity is one of the major case-defining features.

Sleep Disorders in Children with Rett Syndrome.

Rett syndrome (RTT) is an X-linked neurodevelopmental disorder marked by neurological regression, autonomic dysfunction, seizures, and significant sleep and breathing abnormalities. About 80% of affected individuals, especially young children, experience sleep disturbances such as insomnia, sleep-disordered breathing, nocturnal vocalizations, bruxism, and seizures. Breathing irregularities during sleep-like apnea, alternating hyperventilation, and hypoventilation-are common, with both obstructive and central sleep apnea identified through polysomnography. This review focuses on the prevalent s

Metabolomics, sleepiness, and sleep duration in sleep apnea.

PURPOSE: Although the mechanism is unclear, daytime sleepiness, a common sequela of obstructive sleep apnea (OSA), has been found to be correlated with a adverse cardiovascular outcomes. Reviewing metabolomics mechanisms of sleep disturbances and cardiovascular disease may help to explain this correlation. METHODS: This review examines the current literature on the relationships between sleepiness, sleep duration, and metabolites in sleep apnea. RESULTS: Although there is a lack of comprehensive literature in this emerging area, existing studies point to a variety of metabolites in different

[Sleep disorders and epilepsy].

It has been reported that patients with epilepsy often have insomnia and/or daytime sleepiness; the symptomatologic features differ in seizure types. Not only the administration of anti-epileptics, but also inappropriate sleep hygiene cause daytime sleepiness. In subjective assessment of sleepiness, we need to pay attention if it can correctly assess or not. The prevalence of obstructive sleep apnea in patients with epilepsy is approximately 10-30%. Sleep apnea deteriorates the seizure control because of worsen sleep condition by sleep apnea, especially in elderly patients. Some researchers re

Tirzepatide-Related Acute Liver Injury.

UNLABELLED: Obstructive sleep apnea (OSA) is identified by instances of either full or partial collapse of the airway during sleep, leading to reduced oxygen levels or awakening from sleep. This disruption causes interrupted and insufficient sleep, impacting cardiovascular well-being, mental health, and overall quality of life. Pediatric OSA is more challenging to diagnose and a single apnoea is considered to be significant in this age group. A hospital based prospective study with 100 children between the ages of 4 and 12 years with sleep disordered breathing. Evaluated for the severity of ob

Astrocytoma in an infant with prolonged apnea.

A 4-week-old infant experienced prolonged central sleep apnea requiring resuscitation. At 6 months of age he developed episodic obstructive apnea (diagnosed as laryngospasm by direct laryngoscopy) with an abnormal EEG and a normal computed tomography (CT) scan, and at 14 months of age he developed hemiparesis due to a gemistocytic astrocytoma grade III-IV. Following removal of the mass, he has had occasional seizure activity, but no further episodes of obstructive apnea.

The use of combined thermal/pressure polyvinylidene fluoride film airflow sensor in polysomnography.

BACKGROUND: The technologies recommended by the American Academy of Sleep Medicine (AASM) to monitor airflow in polysomnography (PSG) include the simultaneous monitoring of two physical variables: air temperature (for thermal airflow) and air pressure (for nasal pressure). To comply with airflow monitoring standards in the sleep lab setting thus often requires the patient to wear two sensors under the nose during testing. We hypothesized that a single combined thermal/pressure sensor using polyvinylidene fluoride (PVDF) film responsive to both airflow temperature and pressure would be effectiv
