Relative Resuscitation Capacity: A Practical Framework for Pediatric Intravenous Access.

BACKGROUND: Adequate intravenous (IV) access is a key component of anesthetic care, but this concept is often discussed in qualitative rather than quantitative terms, particularly within the pediatric subspecialty. AIMS: We propose a concept termed relative resuscitation capacity as a framework to standardize IV catheter sizes across patients of different sizes. METHODS: Manufacturer-reported IV catheter flow rates were used to calculate time to replace estimated blood volume (EBV). A short 16G catheter in an adult was defined to have a relative resuscitation capacity of 1, which serves as a

Carbon footprint of total intravenous anaesthesia vs. inhalational sevoflurane anaesthesia in adults: a modelling study.

INTRODUCTION: The carbon footprint of propofol is less than that of sevoflurane per minimum alveolar concentration hour equivalent of drug, but there are justifiable concerns about the amount of clinical waste generated during total intravenous anaesthesia (TIVA), especially in the context of shorter procedures. The aim of this study was to assess the carbon footprint of inhalational sevoflurane anaesthesia vs. TIVA with propofol and remifentanil, including all the consumables used, over the first 6 h of general anaesthesia in adult patients. METHODS: A hybrid approach was used to model 10 sc

Glucose measurements with accu check inform II versus hexokinase plasma method during surgery under general anesthesia, an observational cohort study.

PURPOSE: Limited research exists on translation of in-vitro glucose measurement interfering compounds to the in-vivo situation. We investigated whether Point-of-Care glucose measurements by Accu Chek Inform II (ACI II) were accurate to monitor glucose concentrations during surgery with general anesthesia by comparing with the reference laboratory hexokinase plasma glucose test. METHOD: Patients undergoing surgery with general anesthesia were included. Anesthesia was maintained with either Sevoflurane or Total intravenous anesthesia (TIVA). Prior to and after induction, blood glucose was measu

Intraoperative fetal heart rate monitoring during emergency neurosurgery in a parturient.

Malignant brain tumors during pregnancy are rare, and these patients seldom require immediate surgical intervention. A 27-year-old pregnant woman underwent emergency craniotomy. Anesthesia was induced with intravenous thiopental-fentanyl; it was maintained with isoflurane in oxygen and continuous intravenous remifentanil infusion. We used full stomach precautions but omitted succinylcholine for fear of increasing the intracranial pressure during induction of anesthesia. To detect fetal hypoxia and the effects of anesthesia on fetal hemodynamics, the fetal heart rate (FHR) was monitored using a

Pre-anesthetic cimetidine alteration of gastric fluid volume and pH.

Gastric volume and pH were studied immediately after induction of anesthesia and endotracheal intubation in 101 elective surgical patients. Of 44 patients not given cimetidine, 82% had a gastric pH less than 2.5 with a mean pH of 1.6; 45% of these patients had a gastric aspirate pH less than 2.5 associated with a volume exceeding 25 ml. In 57 patients premedicated with intravenous cimetidine at variable intervals (15 to 60 minutes) prior to induction of anesthesia, a significant time-dependent increase was noted in gastric pH (p less than 0.001) together with a decline in gastric volume (p les

Central blocks with levobupivacaina in children.

Regional anesthesia has become a routine practice in paediatric anesthesia and local anaesthetics are now widely used in infants and children. Although local anaesthetics are generally quite safe and effective, they may produce systemic toxic reactions affecting the heart and brain. Because postoperative analgesia is often the primary justification for regional anesthesia in infants and children, bupivacaine, a long-acting local anaesthetic, is the most commonly used local anaesthetic for paediatric regional anesthesia. Levobupiva-caine has been used in children by caudal injection, by lumbar

Anesthesia management using ROP-1680 AsisTIVA : a case report.

AsisTIVA is total intravenous anesthesia support software for syringe pump control. It remains the only automated program to have achieved both practical and commercial viability. AsisTIVA has been certified for use at Tokushima University, Tohoku University, and at its developer institution, Fukui University. We conducted our initial anesthesia management with AsisTIVA at Tokushima University. This initial experience was successful, with no adverse effects. AsisTIVA controls propofol and remifentanil concentrations to maintain a Bispectral index of 45 and adjusts rocuronium concentrations to

Total intravenous anaesthesia in emergency surgery: a narrative review.

INTRODUCTION: Although the use of total intravenous anaesthesia is well established in elective surgery, its use remains limited in emergency settings, particularly for rapid sequence induction and intubation. National audit data and a recent coroner's report have highlighted implementation difficulties, while comparative outcome evidence is lacking and no emergency-specific guidelines exist. METHODS: This narrative review examines the evidence base for total intravenous anaesthesia in emergency surgery, including pharmacological considerations; rapid sequence induction techniques; safety dat

Effect of fentanyl constant-rate infusions with or without medetomidine on the minimum infusion rate of propofol required to prevent motor movement in dogs.

Propofol is a potential injectable anesthetic agent used in total intravenous anesthesia. However, the sparing effect of fentanyl and medetomidine on the required propofol dose in dogs remains unclear. We aimed to investigate the effect of fentanyl constant-rate infusion (CRI) with or without medetomidine on the minimum infusion rate of propofol required to prevent motor movement (MIRNM) in dogs. Six healthy purpose-bred dogs were anesthetized on three occasions with propofol alone (loading dose [LD], 8 mg/kg to effect; initial infusion rate [IR], 0.70 mg/kg/min); propofol (LD, 6 mg/kg to effe

[Anesthesia using propofol for a patient with arthrogryposis multiplex congenita].

Arthrogryposis multiplex congenita (AMC) is a syndrome with multiple persistent limb contractures, often accompanied by associated anomalies. Pediatric patients with AMC frequently require operations, necessitating general endotracheal anesthesia. We report on the anesthesia used for a 14-year-old male with AMC, who required the surgical removal of soft tissue from both hip joints. We used propofol and vecuronium bromide to induce anesthesia, and maintained anesthesia with 40% nitrous oxide, oxygen, and propofol at 6 mg.kg-1.1 hr-1. Both induction and maintenance were smooth, and no hypertherm

Propofol anesthesia in a patient with Isaacs syndrome--report of a case and literature review.

Isaacs syndrome is an unusual lower motor neuron disease characterized by myokymia (muscle twitching), muscular stiffness, and decreased tendon reflexes. We reported a patient who was affected with this rare disease, with manifestation of involuntary muscular contractions and required general anesthesia for bilateral tonsillectomies. Understanding the presentation and characterization of this unusual disease may be helpful in making choice of anesthetics or anesthetic techniques. Its possible mechanisms of action and its specific considerations in anesthesia in the literature are reviewed and

New intravenous anaesthetics and neuromuscular blocking drugs. A review of their properties and clinical use.

The newer neuromuscular blocking drugs include vecuronium and atracurium. Vecuronium is a competitive neuromuscular blocking drug with a steroid nucleus. A dose of 0.1 mg/kg has an onset time of 2 minutes and provides surgical paralysis for 20 minutes. Recovery to 90% twitch height occurs in 40 to 50 minutes. Vecuronium has few adverse effects and its use is associated with cardiovascular stability. Atracurium is a competitive neuromuscular blocking drug which undergoes Hofmann degradation and ester hydrolysis in plasma. A dose of 0.6 mg/kg has an onset time of around 2 minutes and provides su

Methylphenidate actively induces emergence from general anesthesia.

BACKGROUND: Although accumulating evidence suggests that arousal pathways in the brain play important roles in emergence from general anesthesia, the roles of monoaminergic arousal circuits are unclear. In this study, the authors tested the hypothesis that methylphenidate (an inhibitor of dopamine and norepinephrine transporters) induces emergence from isoflurane general anesthesia. METHODS: Using adult rats, the authors tested the effect of intravenous methylphenidate on time to emergence from isoflurane general anesthesia. They then performed experiments to test separately for methylphenida

Intraoperative acute brain swelling when performing indirect anastomosis in a patient with moyamoya disease -A case report-.

A 6-year-old male patient who was suffering from a cold and a transient ischemic attack was scheduled to undergo encephalo-duro-arterio-synangiosis for treating his moyamoya disease. Acute brain edema occurred just after opening the dura mater. Head elevation, reduction of the head rotation and hyperventilation were done. The inhalational agents were discontinued and total intravenous anesthesia was started. The swelling was reduced after intravenously infusing mannitol. An abrupt return from hypocapnia to normocapnea during the induction of general anesthesia was thought to be the cause of th
