Differentiation between left bundle branch block and left ventricular hypertrophy: implications for cardiac resynchronization therapy.

Recent clinical trials have demonstrated that cardiac resynchronization therapy (CRT) reduces heart failure hospitalizations and mortality in patients with complete left bundle branch block (LBBB), but potentially not those with right bundle branch block or nonspecific LV conduction delay, such as that due to LV hypertrophy (LVH). Furthermore, endocardial mapping and simulation studies have suggested that one-third of patients diagnosed with LBBB by conventional electrocardiographic criteria are misdiagnosed, and these patients likely have a combination of LVH, LV chamber dilatation and delaye…

Cardiac resynchronization therapy and obstructive sleep-related breathing disorder in patients with congestive heart failure.

OBJECTIVES: To assess the impact of cardiac resynchronization therapy (CRT) with or without atrial overdrive pacing, on sleep-related breathing disorder (SRBD). INTRODUCTION: CRT may have a positive influence on SRBD in patients who qualify for the therapy. Data are inconclusive in patients with obstructive SRBD. METHODS: Consenting patients eligible for CRT underwent a baseline polysomnography (PSG) 2 weeks after implantation during which pacing was withheld. Patients with an apnea hypopnea index (AHI) ≥15 but <50 were enrolled and randomized to atrial overdrive pacing (DDD) versus atrial s…

A prospective comparison of AV delay programming methods for hemodynamic optimization during cardiac resynchronization therapy.

INTRODUCTION: There are several methods for programming the optimal AV delay (AVD) during cardiac resynchronization therapy (CRT). These include Doppler echocardiographic measurements of mitral inflow or aortic outflow velocities, an arbitrarily fixed AVD, and calculations based on intracardiac electrogram (EGM) intervals. The present study was designed to compare the acute effects of AVD programming methods during CRT. METHODS AND RESULTS: We studied 28 patients at CRT implant with invasive measurements of LV dP/dt to determine the effect of AVD during atrial sensed (AS) and atrial paced (AP…

Echocardiographic measures of acute haemodynamic response after cardiac resynchronization therapy predict long-term clinical outcome.

AIMS: Although acute haemodynamic improvement in response to cardiac resynchronization therapy (CRT) is reflective of a favourable cardiac contractile response, there is limited information regarding not only its ability to predict long-term clinical outcome but also cardiac-substrate-specific differences in the prognostic value of this measure. METHODS AND RESULTS: Fifty-three heart failure patients (69 +/- 11 years) with low left ventricle ejection fraction (LVEF) (22 +/- 6%), wide QRS (169 +/- 31 ms), and indications for CRT were included. There were no significant differences in age, New …

An electrocardiogram-based algorithm to detect loss of left ventricular capture during cardiac resynchronization therapy.

BACKGROUND: Loss of left ventricular capture in patients with cardiac resynchronization devices may account for worsening heart failure and … ght ventricle only. CONCLUSION: Presence of biventricular capture--the prerequisite for successful cardiac resynchronization therapy--and loss of left ventricular capture can be accurately detected by an algorithm based on analysis of the R-S ratio on leads V1 and I of the surface electrocardiogram.

Long-term follow-up of cardiac resynchronization therapy in patients with end-stage heart failure.

UNLABELLED: Long-term follow-up of cardiac resynchronization therapy. INTRODUCTION: Cardiac resynchronization therapy (CRT) has been introduced to treat patients with end-stage heart failure, and results of this technique are promising. The aim of our study was to assess the sustained benefit of CRT in a large patient cohort with end-stage heart failure at long-term follow-up. In addition, the prognosis of responders and nonresponders was evaluated. METHODS AND RESULTS: 125 patients with end-stage heart failure, NYHA class III or IV, LVEF<35%, QRS duration>120 msec and left bundle branch blo…

Acute effects of cardiac resynchronization therapy on functional mitral regurgitation in advanced systolic heart failure.

OBJECTIVES: We studied the acute effects of cardiac resynchronization therapy (CRT) on functional mitral regurgitation in heart failure (HF) patients with left bundle branch block (LBBB). BACKGROUND: Both an decrease [corrected] in left ventricular (LV) closing force and mitral valve tethering have been implicated as mechanisms for functional mitral regurgitation (FMR) in dilated hearts. We hypothesized that an increase in LV closing force achieved by CRT could act to reduce FMR. METHODS: Twenty-four HF patients with LBBB and FMR were studied after implantation of a biventricular CRT system.…

[Cardiac resynchronization therapy by biventricular pacing. How many patients with left ventricular dysfunction are eligible?].

BACKGROUND AND OBJECTIVE: Cardiac resynchronization therapy by multisite biventricular pacing presents an additive therapeutic option in the treatment of severe congestive heart failure. The objective of the study was to evaluate how many patients with left ventricular dysfunction may potentially benefit from this therapy. METHODS: A total of 975 patients were screened for the prevalence of left ventricular dysfunction. Patients with a left ventricular ejection fraction (LVEF) <45 % were included into the investigation. Potential benefit of biventricular pacing was presumed in the presence of…

Usefulness of Electrocardiographic Left Atrial Abnormality to Predict Response to Cardiac Resynchronization Therapy in Patients With Mild Heart Failure and Left Bundle Branch Block (a Multicenter Automatic Defibrillator Implantation Trial with Cardiac Resynchronization Therapy Substudy).

Cardiac resynchronization therapy (CRT) has proven prognostic benefits in patients with heart failure (HF) with left bundle branch block (LBBB) QRS morphology. Electrocardiographic left atrial (LA) abnormality has been proposed as a noninvasive marker of atrial remodeling. We aimed to assess the impact of electrocardiographic LA abnormality for prognosis in patients with HF treated with CRT. Baseline resting 12-lead electrocardiograms recorded from 941 patients enrolled in the CRT arm of the Multicenter Automatic Defibrillator Implantation Trial with Cardiac Resynchronization Therapy was proce…

Lead-to-Port Reversal Mimicking Loss of Cardiac Resynchronization Therapy After CRT-D Generator Replacement.

BACKGROUND: Lead-to-port misconnections are a rare but clinically significant complication after cardiac device generator replacement and may mimic true lead failure or loss of capture, leading to diagnostic confusion and unnecessary interventions. CASE SUMMARY: A 72-year-old man with ischemic cardiomyopathy and a previously implanted cardiac resynchronization therapy defibrillator presented with worsening dyspnea following elective generator replacement performed at another center. Postprocedural electrocardiogram demonstrated loss of biventricular pacing. Chest radiography confirmed correct…

Transbaffle, Endocardial Systemic Right Ventricle Cardiac Resynchronization Therapy in D-Transposition of the Great Arteries.

BACKGROUND: Systemic right ventricular (SRV) cardiomyopathy commonly complicates d-transposition of the great arteries with previous atrial switch repair. Cardiac resynchronization therapy (CRT) is an effective therapy but may be difficult to achieve conventionally via coronary sinus (CS) lead implantation. CASE SUMMARY: A 58-year-old man with a history of d-transposition of the great arteries and prior Mustard procedure presented with progressive dyspnea and QRS duration >200 milliseconds. No suitable CS branch was identified; therefore, we delivered CRT with endocardial SRV lead implantatio…

Predicting Cardiac Resynchronization Therapy Response with Speckle-Tracking Echocardiography: A Systematic Review and Meta-Analysis.

Nearly 30% of patients with cardiac resynchronization therapy (CRT) are nonresponders. Speckle-tracking echocardiography (STE) parameters are able to evaluate electromechanical dyssynchrony that could improve outcomes. We aim to examine the association between various STE parameters with CRT response and clinical outcomes in heart failure patients. Searching was done on 3 medical databases. The STE parameters assessed are dyssynchrony index (time delay of peak strain between walls > 130 msec), myocardial work indices (global constructive and wasted work), and strain patterns. Meta-analysis wit…

Pressure-Volume Loop Optimization of Cardiac Resynchronization Therapy in a Left Ventricular Assist Device Patient.

BACKGROUND: Patients with a left ventricular assist device (LVAD) often have a pre-existing cardiac resynchronization therapy (CRT) device, although guidelines highlight uncertainty surrounding postimplant programming. CASE SUMMARY: A 60-year-old man with a HeartMate 3 (Abbott) had left bundle branch-optimized CRT. Right/left heart catheterization with biventricular pressure-volume (PV) loops was performed for device optimization. WHY BEYOND THE GUIDELINES: Guidelines suggest that both leaving CRT on (remodeling benefit) or turning off (improve battery) may be appropriate for patients with L…

The Prevalence and Trends of Contrast-Induced Nephropathy Among Hospitalized Patients Undergoing Cardiac Resynchronization Therapy.

INTRODUCTION: Contrast-induced nephropathy (CIN) is a significant complication following the use of iodinated contrast agents during cardiac procedures. Cardiac resynchronization therapy (CRT), increasingly used in heart failure management, requires contrast administration, putting patients at risk for CIN. However, the prevalence and outcomes of CIN in CRT recipients remain underexplored. METHODS: We conducted a retrospective cohort study using the Nationwide Readmissions Database (NRD) from 2016 to 2020. Adult patients undergoing CRT or CRT-D implantation were identified using International…

The effect of sodium-glucose co-transporter 2 inhibitors on outcomes after cardiac resynchronization therapy.

AIMS: The trials upon which recommendations for the use of cardiac resynchronization therapy (CRT) in heart failure used optimal medical therapy (OMT) before sodium-glucose co-transporter 2 inhibitors (SGLT2i). Moreover, the SGLT2i heart failure trials included only a small proportion of participants with CRT, and therefore, it remains uncertain whether SGLT2i should be considered part of OMT prior to CRT. METHODS AND RESULTS: We compared electrocardiogram (ECG) and echocardiographic responses to CRT as well as hospitalization and mortality rates in consecutive patients undergoing implantatio…

Cardiac resynchronization therapy in heart failure based on Strauss criteria for left bundle branch block.

AIMS: The left bundle branch block (LBBB) is a strong predictor of response to cardiac resynchronization therapy (CRT). However, a significant number of patients do not respond to the treatment. The study sought to evaluate the impact of the stricter Strauss criteria for left bundle branch block (St-LBBB) on CRT response, hospitalizations, ventricular arrhythmia (VA) events and mortality. METHODS: This study is a retrospective analysis of prospectively collected data on heart failure (HF) patients with LBBB admitted for CRT implantation. Patients were divided into two groups according to the …

Primary prevention implantable cardioverter defibrillator in cardiac resynchronization therapy recipients with advanced chronic kidney disease.

INTRODUCTION: The implantable cardioverter defibrillator (ICD) is effective for the prevention of sudden cardiac death (SCD) in patients with heart failure and a reduced ejection fraction (HFrEF). The benefit of the ICD in patients with advanced CKD, remains elusive. Moreover, the benefit of the ICD in patients with advanced chronic kidney disease (CKD) and HFrEF who are cardiac resynchronization therapy (CRT) recipients may be attenuated. HYPOTHESIS: We hypothesized that patients with CKD who are CRT recipients may derive less benefit from the ICD due to the competing risk of dying prior to …
