Predictors of Treatment Outcomes in Internet-Delivered Cognitive Behavioral Therapy Among Autistic Youth With Anxiety Disorders.

PURPOSE: Anxiety disorders are widespread and significantly impair the mental health and daily functioning of autistic children and adolescents. Internet-delivered cognitive behavioral therapy (iCBT) has emerged as a widespread, low-cost alternative to traditional CBT, helping to reduce demands on therapists without sacrificing efficacy. However, the predictors of treatment outcomes in iCBT for this population remain understudied. This study used data from a randomized clinical trial evaluating parent-led, therapist-supported iCBT among 57 autistic youth with anxiety disorders to examine wheth…

The effects of cognitive behavioral therapy-based digital therapeutic intervention on patients with alcohol use disorder.

INTRODUCTION: This study investigated the effectiveness of a digital therapeutic intervention for individuals with alcohol use problems. Digital interventions are increasingly considered viable alternatives or complements to traditional face-to-face treatments, especially in improving accessibility and adherence. METHODS: A total of 30 outpatients were recruited and randomly assigned to either a digital intervention group or a face-to-face cognitive behavioral therapy (CBT) control group. After excluding two dropouts, data from 28 participants were analyzed. The digital intervention group rec…

A systematic review of anti-suicidal effects of sedative-hypnotics and cognitive behavioral therapy for insomnia.

Suicide accounts for over 700,000 deaths per year globally and remains a public health priority. Evidence suggests that sleep-related interventions may be effective in reducing depressive symptom severity and suicidal thoughts in patients diagnosed with depression and comorbid insomnia. This study aims to systematically review the efficacy of sedative-hypnotics and/or cognitive behavioral therapy for insomnia (CBT-I) on measures of suicidality. In accordance with Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, PubMed, Medline, Cochrane Library, Embase, Sc…

Development and Application of a Psychoeducation-Focused Web-Based Cognitive Behavioral Therapy Program for Panic Disorder.

Web-based services are essential for making psychiatric treatments more accessible. Evidence-based treatments, including cognitive behavioral therapy for panic disorder, should be disseminated to a broader segment of society. In our study, we developed a web-based cognitive behavioral psychoeducation program for panic disorder and evaluated its short-term effects in a clinical sample. The 4-week program was administered to 31 patients diagnosed with panic disorder, and related outcomes were assessed using the Panic Disorder Severity Scale (PDSS), Anxiety Sensitivity Index-3 (ASI-3), Health Anx…

Behavioral experiments vs. verbal interventions in cognitive therapy for social anxiety disorder: A randomized controlled trial.

OBJECTIVE: The efficacy of cognitive therapy (CT) for social anxiety disorder (SAD) is well documented, although effect sizes (ES) vary significantly across studies. These variations may be attributable to differences in the active components of the treatments. A key component potentially influencing outcomes is the use of behavioral experiments (BEs), as opposed to purely verbal interventions (VIs) for changing social anxiety related negative beliefs. The present trial compared the effectiveness of CT incorporating behavioral experiments (CT-BE) with that of CT focusing more exclusively on ve

Secondary outcomes of enhanced cognitive behavioral therapy (eCBT) for children and adolescents with obsessive-compulsive disorder.

BACKGROUND: Obsessive-compulsive disorder (OCD) is a debilitating mental health condition usually presenting with a high degree of comorbid symptoms in the majority of cases. Although face-to-face cognitive-behavioral therapy (CBT) is considered the therapeutic golden standard for pediatric OCD, its accessibility, availability, and consistency in delivery are still limited. To address some of these challenges, an enhanced CBT (eCBT) package was created and introduced. This study explored eCBT's broad-based impact on OCD-related comorbid symptoms, functional impairment, quality of life and fami

A systematic review of the effects of rumination-focused cognitive behavioral therapy in reducing depressive symptoms.

There is still potential room for improving the effectiveness of standard Cognitive Behavioral Therapy (CBT) in preventing the onset of depression, achieving full remission, and preventing relapse or recurrence of depression. Standard CBT seems less effective in reducing depressive rumination, a key risk factor leading to the onset and persistence of depression. To improve treatment efficacy for depression, rumination-focused cognitive behavioral therapy (RFCBT) was developed, which was modified from CBT and specifically targeted to manage rumination. This systematic review aimed to assess the…

Rumination induction task in fMRI: Effects of rumination focused cognitive behavioral therapy and stability in youth.

BACKGROUND: Rumination is implicated in the onset and maintenance of major depressive disorder (MDD). Rumination-Focused Cognitive Behavioral Therapy (RF-CBT) effectively targets rumination and may change resting-state brain connectivity and change in activation during a rumination induction task (RIT) post-intervention predicts depressive symptoms two years later. We examined brain activation changes during an RIT in adolescents with remitted MDD following RF-CBT and evaluated RIT reliability (or stability) during treatment as usual (TAU). METHOD: Fifty-five adolescents ages 14-17 completed …

Generalization Between Perceptually Similar Stimuli Is Associated With Improvement in Social Anxiety Following Cognitive-Behavioral Therapy.

Cognitive-behavioral therapy (CBT) for anxiety disorders requires that people learn to inhibit their fear during exposure to stimuli that no longer pose a threat. We investigate whether individual differences in this inhibitory learning ability, measured prior to treatment, can predict responsiveness to CBT for social anxiety disorder. Participants (N = 128) were randomized to CBT or a wait-list control and completed tests of fear generalization and extinction prior to and following the intervention period. Contrary to expectations, individual differences in extinction, measured at pretreatmen

Long-term effects of integrated cognitive behavioral therapy for chronic pain: A qualitative and quantitative study.

Cognitive behavioral therapy (CBT) is known to improve chronic pain management. However, past studies revealed only small to moderate benefits in short-term results, and long-term follow-up studies are lacking. This study aimed to follow an integrated CBT program's effectiveness 1.5 years after its completion. This observational study was the follow-up on the data collected from our CBT sessions conducted under 3 different studies in 2018 to 2019. Seven assessment items (Numerical Rating Scale, Pain Catastrophizing Scale [PCS], Pain Disability Assessment Scale [PDAS], Patient Health Questionna…

An exploration of neural predictors of treatment compliance in cognitive-behavioral group therapy for hoarding disorder.

A persistent and influential barrier to effective cognitive-behavioral therapy (CBT) for patients with hoarding disorder (HD) is treatment retention and compliance. Recent research has suggested that HD patients have abnormal brain activity identified by functional magnetic resonance (fMRI) in regions often engaged for executive functioning (e.g., right superior frontal gyrus, anterior insula, and anterior cingulate), which raises questions about whether these abnormalities could relate to patients' ability to attend, understand, and engage in HD treatment. We examined data from 74 HD-diagnose

Altered fronto-amygdalar functional connectivity predicts response to cognitive behavioral therapy in pediatric obsessive-compulsive disorder.

BACKGROUND: Based on findings from adults with obsessive-compulsive disorder (OCD), this study examined alterations in resting-state functional connectivity (rs-fc) between the basolateral amygdala (BLA) and the ventromedial prefrontal cortex (vmPFC) in children and adolescents with OCD. We also assessed whether such BLA-vmPFC connectivity changed with or predicted response to exposure and response prevention (E/RP), the first-line treatment for pediatric OCD, given the involvement of these regions in fear processing, regulation, and extinction learning-a probable mechanism of action of E/RP.
