Unexpected loss of contact allergy to aluminium induced by vaccine.

BACKGROUND: In studies in Gothenburg, Sweden, in the 1990s of an aluminium hydroxide-adsorbed pertussis toxoid vaccine, 745 of ~76 000 vaccinated children developed long-lasting itchy subcutaneous nodules at the vaccination site. Of 495 children with itchy nodules patch tested for aluminium allergy, 376 (76%) were positive. OBJECTIVES: To study the prognosis of the vaccine-induced aluminium allergy. PATIENTS AND METHODS: Two hundred and forty-one children with demonstrated aluminium allergy in the previous study were patch tested again 5-9 years after the initial test, with the same procedur

Biologics in IgE-mediated food allergy: A systematic review and meta-analysis of interventional studies.

BACKGROUND AND AIMS: IgE-mediated food allergy (FA) is a major healthcare problem, affecting millions of children and adults worldwide. FA management usually involves elimination diets; however, there is increasing interest in alternative strategies that enable individualized optimal approaches. Yet, there is little consensus on the optimal strategies for managing FA. This review aimed to evaluate the effectiveness and safety of biologics, including omalizumab (OMA), as monotherapy or in combination with oral immunotherapy (OIT), for FA management. METHODS: A systematic review (SR) and meta-a

Kidney exchange strategies: new aspects and applications with a focus on deceased donor-initiated chains.

Kidney paired donation (KPD) is a valuable way to overcome immunological incompatibility in the context of living donation, and several strategies have been implemented to boost its development. In this article, we reviewed the current state of the art in this field, with a particular focus on advanced KPD strategies, including the most recent idea of initiating living donor (LD) transplantation chains with a deceased donor (DD) kidney, first applied successfully in 2018. Since then, Italy has been running a national programme in which a chain-initiating kidney is selected from a DD pool and a

Human leukocyte antigens and alloimmunization in heart transplantation: an open debate.

Considerable advances in heart transplantation outcome have been achieved through the improvement of donor-recipient selection, better organ preservation, lower rates of perioperative mortality and the use of innovative immunosuppressive protocols. Nevertheless, long-term survival is still influenced by late complications. We support the introduction of HLA matching as an additional criterion in the heart allocation. Indeed, allosensitization is an important factor affecting heart transplantation and the presence of anti-HLA antibodies causes an increased risk of antibody-mediated rejection an

Precision Medicine in Hymenoptera Venom Allergy: Diagnostics, Biomarkers, and Therapy of Different Endotypes and Phenotypes.

Allergic reactions to stings of Hymenoptera species may be severe and are potentially fatal deviations of the immunological response observed in healthy individuals. However, venom-specific immunotherapy (VIT) is an immunomodulatory approach able to cure venom allergy in the majority of affected patients. An appropriate therapeutic intervention and the efficacy of VIT not only depend on a conclusive diagnosis, but might also be influenced by the patient-specific manifestation of the disease. As with other diseases, it should be borne in mind that there are different endotypes and phenotypes of

Measurement of serum levels of eosinophil cationic protein to monitor patients with seasonal respiratory allergy induced by Parietaria pollen (treated and untreated with specific immunotherapy).

This trial studied the behavior of a marker of eosinophilic inflammation, eosinophil cationic protein (ECP), in the peripheral blood of two groups of subjects with seasonal allergic respiratory symptoms (rhinitis and mild bronchial asthma) induced by pollen allergens of Parietaria judaica (P.j.) (one group treated and another untreated with specific immunotherapy [SIT]), to determine what contribution these serial measurements might provide, in comparison with various other tools now available for pollinosis monitoring. In a previously randomized order, we selected 25 patients with monosensiti

An Hourly Dose-Escalation Desensitization Protocol for Aspirin-Exacerbated Respiratory Disease.

BACKGROUND: Aspirin desensitization followed by maintenance therapy effectively improves symptom control in patients with aspirin exacerbated respiratory disease (AERD). The majority of current desensitization protocols use 3-hour dosing intervals and often require 2 to 3 days to complete. OBJECTIVE: We evaluated hourly dose escalations in a subset of patients with chronic rhinosinusitis, nasal polyps, and asthma who historically reacted to aspirin within 1 hour or were avoiding aspirin with the goal of developing a safe and efficient desensitization protocol. METHODS: Fifty-seven aspirin de

Evaluation and management of hypersensitivity reactions to chemotherapy agents.

Hypersensitivity reactions to chemotherapy drugs pose significant difficulties in management, especially when no suitable alternative is available or acceptable and delay in continuation of treatment may be life-threatening. Such reactions may be IgE- or non-IgE-mediated and have varied manifestations. Timely recognition and treatment of life-threatening hypersensitivity reactions are essential. Identification of patients at high risk of developing hypersensitivity reactions allows risk stratification to guide clinical decision-making. Skin testing for carboplatin hypersensitivity has good pre

Tolerability of a new fast updosed immunologically enhanced subcutaneous immunotherapy formulation with an optimized allergen to adjuvant ratio under routine practice conditions: a noninterventional observational study.

BACKGROUND AND OBJECTIVES: A fast updosed immunologically enhanced subcutaneous immunotherapy (SCIT) formulation with an optimized allergen to aluminium hydroxide ratio was first introduced in September 2009 in Germany. A large randomized controlled trial showed that the formulation had considerable immunologic effects and good tolerability. In this open-label, uncontrolled, noninterventional study, tolerability was investigated during routine application. PATIENTS AND METHODS: Patients with allergic rhinoconjunctivitis and/or asthma were treated with pollen and mite allergens using a 5-injec

Role of Treg in immune regulation of allergic diseases.

Allergy is a Th2-mediated disease that involves the formation of specific IgE antibodies against innocuous environmental substances. The prevalence of allergic diseases has dramatically increased over the past decades, affecting up to 30% of the population in industrialized countries. The understanding of mechanisms underlying allergic diseases as well as those operating in non-allergic healthy responses and allergen-specific immunotherapy has experienced exciting advances over the past 15 years. Studies in healthy non-atopic individuals and several clinical trials of allergen-specific immunot

Immune mechanisms affecting bovine leucocytes during suppression of tuberculin skin sensitivity in cattle.

Complete desensitization to tuberculin skin-testing (anergy) was produced in cattle by repeated intravenous injections of living BCG organisms into animals sensitized by a prior subcutaneous dose of BCG. Two levels of desensitization were produced; complete desensitization following 10 i.v. doses and partial desensitization following 5 i.v. doses of 100 mg BCG. Intradermal tuberculin testing at the end of the experiment stimulated the appearance of reactive blood lymphocytes in sensitized cattle as measured by in vitro 3H-thymidine uptake. The cattle which were partially desensitized showed th

Early-life peanut oral immunotherapy associated with long-term ingestion and immunologic changes.

BACKGROUND: The IMPACT trial (NCT01867671) demonstrated strong desensitization and the potential for remission with peanut oral immunotherapy (pnOIT) in 1- to 3-year-olds. Data on long-term outcomes of early intervention oral immunotherapy (OIT) are limited. OBJECTIVE: IMPACT-PLuS sought to assess the long-term efficacy, safety, and mechanistic changes related to early-life pnOIT. METHODS: Participants randomized in IMPACT (n = 146) were recruited. The primary outcome was long-term efficacy, defined as ongoing peanut consumption. Secondary outcomes included safety, peanut serology, and skin

Beyond desensitization: the SCFA-Treg-gut microbiota axis as a mechanistic framework for sustained unresponsiveness in food allergy.

Food allergy is an immune-mediated condition associated with acute reactions, risk of anaphylaxis, and a substantial day-to-day management burden. Oral immunotherapy (OIT) can increase the amount of allergen tolerated during treatment, but this desensitized state commonly depends on continued exposure. Sustained unresponsiveness (SU), also termed remission in some trials, is a stricter operational endpoint in which clinical non-reactivity is retained after a predefined period of treatment withdrawal and is confirmed by oral food challenge. The biological processes that determine whether desens

Safety and efficacy of three office-based egg oral immunotherapy protocols.

BACKGROUND: Egg allergy is one of the most common IgE-mediated food allergies. There is no treatment approved by the US Food and Drug Administration that allows the liberalization of egg into the diet. Studies have shown that oral immunotherapy (OIT) provides protection against accidental ingestion and may also allow patients to eat egg freely, but vary in terms of protocols, efficacy, and adverse effects. OBJECTIVE: To assess the safety and efficacy of three different office-based egg OIT protocols. METHODS: Describe three egg OIT protocols, specifically low-dose native egg, high-dose nativ

Biological Effects of F(ab')2 Fragments Generated by Imlifidase From Anti-HLA IgG Antibodies From Transplant Patients.

Imlifidase cleaves intact IgG molecules, generating F(ab')2 fragments that still bind to the target antigen but are devoid of the Fc-dependent effector functions, notably complement activation. Imlifidase (Idefirix) has been conditionally approved for desensitisation to enable kidney transplantation in highly sensitised adult patients. We hypothesised that the F(ab')2 fragments can prevent the attachment of the full IgG at the time of anti-HLA antibodies rebound when imlifidase action fades away, lowering the deleterious effect of the antibody burden on the transplant. This competition was exp

Characterization of Human Group 9 Innate Lymphoid Cells in Response to Allergen Immunotherapy in Patients With Allergic Rhinitis.

BACKGROUND: Innate lymphoid cells (ILCs) are classically divided into three groups: ILC1, ILC2, and ILC3 to reflect their functional analogy to Th1, Th2, and Th17. There is also an IL-9 single-producing T cell subset, namely the Th9 cell, which plays a dominant role in the onset of allergic diseases compared with traditional Th2 cells. Although a corresponding cell subset of ILCs to different Th cell subsets exists, so far the counterpart of Th9 cells in ILCs has not been reported. OBJECTIVE: In this study, we aimed to report the existence of group 9 innate lymphoid cells (ILC9s) and characte

Risk Factors for Severe Sting Reactions and Side Effects During Venom Immunotherapy.

Understanding the risk factors leading to severe systemic sting reactions (SSRs) is crucial for initiating venom immunotherapy (VIT) and for educating affected individuals and their families. Some of these risk factors are well established, some are no longer considered risk factors, and some remain controversial. Well-established risk factors for severe SSRs include clonal mast cell disease, high baseline serum tryptase, and advanced age. The absence of skin symptoms and the rapid onset of symptoms are indicators of severe SSRs. Recent publications indicate that antihypertensive treatment and
