The cardiovascular perfusionist as a model for the successful technologist in high stress situations.

This study investigates the psychological profiles of highly stressed medical technologists. One hundred and four individuals representing a cross-section of the United States who function as operators of heart-lung machines during open heart surgery (perfusionists) were studied using both internal and external models based on the works of Eric Berne and Karen Horney. Daily exposure to life and death responsibilities combined with the constant pressures of maintaining current technical skills can make the profession selected for this study representative of high technology professions that req

Primary process thinking, primitive defensive operations and object relationships in borderline and neurotic patients.

In the present paper Kernberg's hypothesis concerning the connection between primary process thinking on the one hand and primitive defense mechanisms and modes of object relationships on the other hand were tested empirically in a sample of 30 hospitalized borderline and 30 hospitalized neurotic patients. The diagnoses of the patients were given according to the 'Diagnostic Interview for Borderlines' of Gunderson and Kolb, the functions mentioned above were assessed on the basis of the Holtzman Inkblot Technique applying scoring systems of Lerner and coworkers for primitive defense mechanisms

Naturalistic use of psychedelics does not modulate processing of self-related stimuli (but it might modulate attentional mechanisms): An event-related potentials study.

Classic psychedelics are able to profoundly alter the state of consciousness and lead to acute experiences of ego dissolution - the blurring of the distinction between representations of self and the external world. However, whether repeated use of psychedelics is associated with more prolonged and permanent modifications to the concept of self remains to be investigated. Therefore, we conducted a preregistered, cross-sectional study in which experienced psychedelics users (15 or more lifetime experiences with psychedelics; N = 56) were compared to nonusers (N = 57) in terms of neural reactivi…

Ruminative self-focus in daily life: associations with daily activities and depressive symptoms.

The present study examined the situations and conditions in which ruminative self-focus is less likely to occur in daily life. Previous researchers have described a mood-brightening effect of depression, where depressed individuals exhibit greater positive emotional reactivity to positive daily events than do nondepressed individuals. To better understand this paradoxical effect, we investigated the moderating role of depression in the relationship between daily activities and ruminative thinking. Forty-one Japanese undergraduates (9 women and 32 men) recorded their thought contents and the ty

Emergence of self and other in perception and action: an event-control approach.

The present paper analyzes the regularities referred to via the concept 'self.' This is important, for cognitive science traditionally models the self as a cognitive mediator between perceptual inputs and behavioral outputs. This leads to the assertion that the self causes action. Recent findings in social psychology indicate this is not the case and, as a consequence, certain cognitive scientists model the self as being epiphenomenal. In contrast, the present paper proposes an alternative approach (i.e., the event-control approach) that is based on recently discovered regularities between per

The diving bell meets the butterfly: identity lost and re-membered.

Jean Dominique Bauby, former editor of Elle, suffered a stroke to his brain stem that left him with locked-in syndrome. Subsequently, through blinking his left eye, he writes his memoir of this experience, The Diving Bell and the Butterfly. This paper explores the meaning of embodiment, especially as one's body bears upon one's personal identity. It explores the various challenges and threats to selfhood that result from Bauby's experience and recounts how Bauby rises to the challenge through his memory and imagination.

Refusal of autonomy: the use of words by a young adult in analysis.

Analytic work with a depressed twenty-three-year-old woman is presented showing how she used ruminative thoughts to maintain a fixed internal dyadic relationship in which neither self not object had autonomy. The resulting omnipotent/impotent impasse meant that she was unable to move forward in her development. In the analysis this relationship was externalized through her use of words, which were employed as actions designed to have a concrete effect on both her self and her analyst. For a long period in the analysis it was essential to understand and interpret the function of her words, rath

Personality, life stress and the course of eating disorders.

We tested whether personality disturbance and stressful life events can help explain why some patients with Anorexia nervosa or Bulimia nervosa remain ill for several years. In a follow-up of 37 patients, both factors were found to be significantly associated with a poorer outcome after a year, and preliminary data suggest this trend continues after two years. Further follow-up is necessary, but the data so far suggests that treatment goals and methods must be tailored to widely dissimilar needs. They also imply that therapeutic efforts to help patients cope with stressful events may be reward

Effects of four treatment methods on social phobic patients not suitable for insight-oriented psychotherapy.

Forty-two social phobic men and women, rated unsuitable for insight-oriented psychotherapy, received one of four randomly assigned types of treatment for 3 months. All patients received basal therapy (B) in the form of standardized information, self-exposure instructions, and anxiolytic medication. One group received this treatment only, with monthly appointments. The others, in addition, received either therapist-directed prolonged exposure in vivo, in some cases supplemented with exposure in imagination (PE), dynamically oriented supportive therapy (ST) or relaxation therapy (R). There was a

The significance of Kleinian contributions to psychoanalysis II. Freudian and Kleinian conceptions of early mental development.

This paper discusses and compares Freudian and Kleinian conceptions of the development of early mental life from the vertices of the genetic hypothesis (and reconstruction), the infantile neurosis, the conception of the evolution of the infant, the issue of reality, and the oedipus complex. In so doing, some of the newer findings of infant development are considered. We can now correlate Kleinian conceptions of early infantile reconstruction in light of these new developments; this paper seeks further to demonstrate that the classical notion of primary narcissism, which is laterally known as t

Freud, Sebald and the head of Medusa: The unconscious sense of guilt.

This paper explores Freud's concept of unconscious guilt (Freud, S. 1923. "The Ego and the Id". In The Standard Edition of the Complete Psychological Works of Sigmund Freud, Vol. 19. 1-66), as illuminated in Sebald's masterpiece, Austerlitz (Sebald, W. G. 2001. Austerlitz, Translated by Anthea Bell. London: Penguin Books). Both writers examine the human impulse to look away from suffering, personal and collective, and the guilt inherent both in avoidance and in bearing witness. Sebald, preoccupied with Germany's post-war "self-amnesia" (Sebald, W. G. 2003. On the Natural History of Destruction…

Involuntary Treatment for Child and Adolescent Anorexia Nervosa-A Narrative Review and Possible Advances to Move Away from Coercion.

BACKGROUND: Children and adolescents with psychiatric disorders frequently experience hospital treatment as coercive. In particular, for patients with severe anorexia nervosa (AN), clinical and ethical challenges often arise if they do not voluntarily agree to hospital admission, often due to the ego-syntonic nature of the disorder. In these cases, involuntary treatment (IVT) might be life-saving. However, coercion can cause patients to experience excruciating feelings of pressure and guilt and might have long-term consequences. METHODS: This narrative review aimed to summarize the current em…

The company you keep: Is socialising with higher-status people bad for mental health?

Socialising with higher-status individuals can be hypothesised to exert opposing influences on the mental health of the ego. On the one hand, socialising with higher-status alters might enable individuals to access valuable resources. On the other hand, status-discrepant friendships could be detrimental to mental health by engendering feelings of unfairness. We sought to examine the impact of status-discrepant social relationships on depressive symptoms in the 2012 Korean General Social Survey (KGSS), a nationally representative sample. We show that socialising with higher-status people is pos…

[Does customized weight curves improve prenatal diagnosis of fetuses small for gestational age in a high-risk population?].

OBJECTIVES: To assess the accuracy of customized growth charts for the ultrasound antenatal diagnostic of fetus small for gestational age in a high-risk population of preterm. METHODS: All premature infants born in a French university maternity center for a year and classified as small for gestational age at birth by using customized growth charts developed by Ego et al. were included in this retrospective study. At the ultrasound performed closest to the term, customized growth charts and population growth curves were compared for the antenatal diagnosis of a premature infants group classifi…

The psychoanalytic psychotherapy of a mentally retarded man.

In this paper, the author discusses the psychoanalytically informed treatment of a 25-year-old patient, who at the beginning of treatment was severely developmentally retarded. The patient showed repetitive behaviours (tapping his fingers, tying and retying his shoelaces); visual and auditory hallucinations, delusions, vague to absent ego boundaries; inability to do simple maths, tell the time, read or write; and angry vulgar outbursts accompanied by occasional throwing of objects. He was bewildering and unresponsive to various psychological and physical treatments that had been performed sinc…
