Conventional radiography of the hilum and mediastinum in bronchogenic carcinoma.

Knowledge of normal and abnormal hilar and mediastinal anatomy as depicted by chest radiography is important for the practicing radiologist. This article is an illustrated review of some of the alterations of normal anatomy that can indicate signs of metastatic spread of bronchogenic carcinoma. Thorough analysis of conventional radiographs with recognition of normal variants and detection of abnormal alterations will expedite or obviate certain therapeutic options in patients with bronchogenic carcinoma.

A new technique for the repair of pediatric hydroceles.

The major difficulties encountered in the surgical correction of pediatric communicating hydroceles are separating the hernial sac from the spermatic vessels and vas, and identifying the anatomical location of the internal inguinal ring. The passage of a pediatric Foley catheter through the hernial sac via a high scrotal incision greatly expedites this surgery. Herein, we describe a new surgical technique and discuss its merits.

The role of the obstetrician-gynecologist in the management of breast lesions.

This paper is based on the author's frequent experience in the diagnosis of breast masses in his everyday practice as an obstetriciangynecologist. It appears that gynecologists, by the very nature of their practices, are in an excellent position to head the case-finding expedition for breast lesions. Furthermore, the obstetrician-gynecologist can use acquired surgical skills to great advantage in the management of benign lesions and assist surgical associates in the management of the malignant lesions.

Francisco Oller y Ferrer (1758-1831). The surgeon who performed the first smallpox vaccination in a Hispanic American territory.

Balmis arrived on February 9, 1804, in Puerto Rico, the beginning of the American stage of the Philanthropic Expedition of the vaccine that was to begin the campaign to spread the vaccine throughout Hispanic America, in the Philippines, in Macao and Canton and on the island of St. Helena. With this, the surgeon and the expeditionaries under his orders would go down in the annals for heading one of the most transcendental sanitary enterprises carried out in the history of mankind. However, in our historical research we reiterate that the arrival of the vaccine in Hispanic America did not occur

A cruise ship emergency medical evacuation triggered by handheld ultrasound findings and directed by tele-ultrasound.

Cruise ships travel far from shoreside medical care and present a unique austere medical environment. For the cruise ship physician, decisions regarding emergency medical evacuation can be challenging. In the event that a passenger or crew member becomes seriously ill or is injured, the use of point-of-care ultrasound may assist in clarifying the diagnosis and stratifying the risk of a delayed care, and at times expedite an emergent medical evacuation. In this report we present the first case reported in the literaturę of an emergency medical evacuation from a cruise ship triggered by handheld

Operative versus conservative management for 'fetal distress' in labour.

BACKGROUND: Suspected fetal distress usually results in expedited delivery of a baby (often operatively). The potential harm to a mother and baby from operative delivery may not always be justified especially when fetal distress may be misdiagnosed. Even with a correct diagnosis it is not clear whether an operative or conservative approach is better. OBJECTIVES: The objective of this review was to assess the effects of operative management for fetal distress on maternal and perinatal morbidity. SEARCH METHODS: We searched the Cochrane Pregnancy and Childbirth Group's Trials Register (15 Febr

Ventricular Septal Rupture in Cardiac Sarcoidosis Diagnosed by Point-of-Care Ultrasound.

A 60-year-old female with cardiac sarcoidosis and a history of ventricular aneurysm presented with acute palpitations and diaphoresis. Bedside point-of-care ultrasound (POCUS) identified a ventricular septal rupture (VSR) with left-to-right shunting, suggesting acute rupture of the aneurysm. This case highlights the critical role of POCUS in a rare and life-threatening complication of cardiac sarcoidosis, expediting management and improving patient outcomes.

Heart Transplantation in a Patient With Liddle Syndrome.

Liddle syndrome is characterized by hypertension, hypokalemia, and metabolic alkalosis. This paper presents the case of a 29-year-old man with Liddle syndrome complicated by stage D heart failure and stage 4 chronic kidney disease. Despite appropriate management, this patient developed refractory cardiogenic shock, ultimately requiring mechanical circulatory support with venoarterial extracorporeal membrane oxygenation and expedited heart transplant listing.

Abdominal Pain in a Young Female Adolescent: Point-of-Care Ultrasound Added Value.

An adolescent female patient presented to the emergency department with diffuse, severe lower abdominal pain and vomiting. The initial suspected diagnosis was appendicitis. Point-of-care ultrasound did not visualize the appendix but demonstrated a suspected left ovarian torsion, which was confirmed by radiology-performed ultrasound. The clinical presentation, in combination with images obtained via point-of-care ultrasound, helped to expedite gynecology consultation and immediate surgery.

Colocolic intussusceptions in children: a pictorial essay and review of the literature.

Colocolic intussusception is a variation of intussusception that is rarely encountered in pediatric patients and as such can prove to be a diagnostic challenge for both the emergency clinician and radiologist. Knowledge of the presentation and imaging findings in these cases can expedite diagnosis and guide the patient to appropriate treatment. The demographics, clinical symptoms, imaging findings, and eventual surgical and pathologic outcomes of 8 children with colocolic intussusception will be presented in this pictorial essay, with a review of the available literature.

Transesophageal bronchoscopic ultrasound-guided fine-needle aspiration (EUS-B-FNA)-Pushing the boundaries in the diagnosis.

Isolated mediastinal adenopathy is a diagnostic challenge in the paediatric population, often requiring invasive surgical procedures for diagnosis. We describe a novel minimally invasive modality in a 20 month toddler-transesophageal bronchoscopic ultrasound-guided fine-needle aspiration (EUS-B-FNA). This is the youngest reported use of this modality, highlighting feasibility, technical issues, safety, and rapid diagnosis leading to expedited treatment.

Pediatric acute hemorrhagic leukoencephalitis: report of a surviving patient and review.

Acute hemorrhagic leukoencephalitis (AHLE) is a rare, fulminant CNS demyelinating condition usually diagnosed at autopsy. We report the clinical, laboratory, radiographic, and pathologic features of the first nonfatal case of pediatric AHLE confirmed by brain biopsy. Pathologic diagnosis of this condition may be critical to exclude more-common processes and to expedite the decision to administer high-dose corticosteroid therapy, which is potentially lifesaving.

The many faces of pulmonary aspiration.

The clinical and radiologic manifestations of aspiration are truly protean. The diagnosis is straightforward in patients with known risk factors and new radiographic findings in dependent portions of lung. However, diagnosis is difficult in patients without known risk factors or who present with less specific radiologic findings. Careful history-taking skills and knowledge of the diverse radiologic appearances of aspiration are thus required to facilitate diagnosis and expedite treatment.

[Neuroectodermal dysplasia in 2 Uzbek families].

The authors present the clinical and genealogical description of 6 patients with neuroectodermal dysplasia diagnosed by a medical expedition party in Khankin district of the Khorezm region. Unique combination of clinical signs (total alopecia, microcephalia, oligophrenia, hyperhydrosis and hypogenitalism) helped differentiating the syndrome from other well-known hereditary neuroectodermal dysplasias. The nature of the syndrome segregation in the families suggested its autosomal-recessive mode of inheritance.

Nonobstetric surgery in pregnancy.

Diagnostic and judgmental uncertainty that results in operative delay, leading to a more severe degree of illness and more complex surgery, is the major factor effecting both maternal and fetal morbidity and mortality in pregnant surgical patients. The acute abdomen is responsible for most errors in diagnosis and therapy. An understanding of anatomic, physiologic, and laboratory changes occurring in pregnancy and a timely interdisciplinary approach will expedite management and optimize outcome.

Cerebellar hemorrhage and CT scanning.

We have reviewed the clinical syndrome of acute cerebellar hemorrhage and have presented five cases of cerebellar hemorrhage of various types in which CT scanning was a significant aid in management. Recognizing the clinical syndrome, establishing the diagnosis rapidly by CT scanning, and avoiding potentially lethal diagnostic procedures, are all of crucial importance in expediting surgical treatment and improving the morbidity and mortality associated with this entity.

Fatal accidents among elite mountaineers: A historical perspective from the European Alps.

The lifetime risk of a fatal mountain accident among elite European alpine mountaineers and its time trends are determined by studying a fixed cohort of 390 elite mountaineers listed in the Encyclopaedia of the Alps (Hiebler, 1977). At publication of the encyclopaedia, 158 individuals were still living and were followed up until the end of 2008. The crude lifetime risk of a fatal accident for elite mountaineers is 0.203 [95% confidence interval (CI), 0.165 to 0.246). The difference in mortality between male (0.207; 95% CI: 0.168 to 0.251) and female mountaineers (0.118; 95% CI: 0.033 to 0.343)
