Gallbladder wall thickening secondary to focal lymphatic obstruction.

Gallbladder wall thickening (3 mm or greater) has been described as a sign of acute or chronic inflammatory disease of the gallbladder. However, gallbladder wall thickening is a nonspecific finding in contracted, postprandial gallbladders and in cases of hypoproteinemia, elevated portal venous pressure, congestive heart failure, multiple myeloma, and hepatitis. This report describes the case of a patient who had a markedly thickened gallbladder wall secondary to focal obstruction of gallbladder lymphatic drainage by malignant lymphoma in the portal lymph nodes.

Asymptomatic emphysematous cholecystitis.

Emphysematous cholecystitis (EC) is a life-threatening complication of acute cholecystitis. Its clinical manifestations are usually vague, but asymptomatic cases are very rare. We present such a case with an emphasis on sonographic (US) findings. In this case, detection of gas echoes in the gallbladder wall and the surrounding hepatic tissue led us to an appropriate treatment. US is now the technique of first choice for diagnosing gallbladder diseases, and knowledge of US findings encountered in this case can help prevent a hazardous delay in emergent treatment.

Duplicate gallbladder: A case report of a patient with cholecystitis after cholecystectomy.

INTRODUCTION: Duplicate gallbladder is a congenital anomaly with various anatomical presentations that can pose difficult diagnostic dilemmas. This case presents the consequence of recurrent cholecystitis after prior cholecystectomy due to delay in diagnosis of a duplicate gallbladder and insufficient treatment at first presentation. It also provides the opportunity to discuss the anatomical variations of duplicate gallbladders and their clinical implications. PRESENTATION OF CASE: We report on a 46-year-old woman who presented with symptoms of cholecystitis despite a history of cholecystecto

Mucin genes in gallstone disease.

Gallstone disease is a complex disorder that can be caused by environmental influences, common genetic factors and their interactions. Three major pathogenic abnormalities are considered to involve in gallstone formation: cholesterol supersaturation in bile, precipitation and nucleation of excess cholesterol, and gallbladder hypomotility, while, mucin takes part in the cholesterol nucleation process. Up to date, more than 20 mucin genes have been reported, 9 of them are identified at the mRNA and/or protein level in native gallbladder and its associated diseases. In the gallbladder, mucin is e

[A clinical evaluation of the indications and utility of percutaneous gallbladder drainage].

Percutaneous gallbladder drainage (PGD) was performed in 78 patients with diseases of the bile duct system and pancreas, including 48 patients with acute cholecystitis. The procedure was utilized for the treatment of acute inflammation and obstructive jaundice, and this study was initiated to evaluate its utility and the potential indications. Following PGD, the inflammatory changes improved, and the clinical symptoms subsided, especially in cases of acute cholecystitis, in which it appears to have an excellent therapeutic value. Compared to cases of acute cholecystitis treated without PGD and

A test for patency of the cystic duct in acute cholecystitis.

A procedure was devised to quickly and reliably determine the patency of the cystic duct in patients suspected of having acute cholecystitis. First the gallbladder was stimulated to empty by a cholecystokinin injection. Thirty minutes later a radiolabeled biliary marker, either 150 muCi 131-I rose bengal or 2 mCi 99-mTc dihydrothioctic acid, was injected, and the accumulation of radioactivity in the liver and gallbladder regions was monitored by external gamma emission imaging and recording devices. The images of diagnostic importance were obtained between 60 and 90 minutes after injection of

A Potential Pitfall in POCUS of the Gallbladder: Beware of the Duodenum.

It is estimated that 20 million people in the United States have gallbladder disease. Of the patients who present to the Emergency Department (ED) with abdominal pain, 3-10% have acute cholecystitis. Point-of-care ultrasound (POCUS) evaluation of the biliary system is a valuable tool to diagnose gallbladder disease and can greatly expedite the diagnostic evaluation of patients. One source of error in POCUS of the gallbladder is imaging nearby structures that can mimic the gallbladder, such as the duodenum.

Cholecystokinin and morphine pharmacological intervention during 99mTc-HIDA cholescintigraphy: a rational approach.

Pharmacological intervention with either cholecystokinin-8 (CCK-8) or morphine during 99mTc- hepatoiminodiacetic acid (HIDA) cholescintigraphy is required primarily for the assessment of the diseases affecting the gallbladder, the common bile duct, or the sphincter of Oddi. For imaging, the patient should be prepared by an overnight fast, or with 4 hours of minimum fast. Pre-emptying with CCK-8 is probably undesirable and should either be avoided or one should wait for at least 4 hours after CCK-8 to begin the 99mTc-HIDA study to achieve higher specificity of the test for acute cholecystitis.

Gallbladder sludge and microlithiasis disappearance in a cystic fibrosis patient 1 year after triple combination therapy initiation.

Gallstones, microlithiasis, gallbladder sludge, and micro-gallbladder are frequently reported in cystic fibrosis patients, and modulators could modify gallbladder disease, probably reducing biliary secretions viscosity.

Cholescintigraphy: a valuable diagnostic method in acute cholecystitis.

In 91 patients (36 men, 55 women) who were hospitalized with symptoms of acute cholecystitis, cholescintigraphy was performed with 99m-Tc-ethyl-IDA, in order to evaluate the method in the diagnosis of acute inflammatory disease of the gallbladder. Five groups of patients were recognized: (A) abnormal scintigraphy and verified gallbladder disease, (B) abnormal scintigraphy without verified gallbladder disease (false positive). (C) normal cholescintigraphy but cholelithiasis (false negative), (D) normal cholescintigraphy and no gallbladder disease, and (E) cholescintigraphy in icteric patients.

Carcinoma of the gallbladder.

Results of a retrospective review of 20 consecutive patients with histologically proved carcinoma of the gallbladder encountered during a 15 year period confirm the historically established poor prognosis of the disease. One patient in whom the disease was discovered incidentally and early in its course is alive nine years after diagnosis. The other 19 patients died within eight months of diagnosis, regardless of the extent of the disease or method of treatment. By identifying patients who are at an increased risk, by virtue of such factors as cholelithiasis and advanced age, and scrutinizing

Correlation between gallstones and fasting blood glucose to serum high-density lipoprotein cholesterol ratio among American adults.

BACKGROUND: Research indicates that the ratio of fasting blood glucose (FBG) to serum high-density lipoprotein cholesterol (HDL-C) (GHR) can accurately predict many diseases. Nevertheless, the relationship between GHR and the risk of gallbladder stones remains unclear. This study investigates the possible relationship between GHR and the incidence of gallbladder stones. METHODS: This research used information gathered from the National Health and Nutrition Examination Survey (NHANES) between March 2017 and March 2020. A calculation was made to determine the GHR by dividing the fasting blood g

Gallbladder Mucosa-Associated Lymphoid Tissue (MALT) Lymphoma With Gallstone Pancreatitis and Prostate Cancer.

Primary mucosa-associated lymphoid tissue (MALT) lymphoma of the gallbladder is an extremely rare disease. We describe a case of a 79-year-old man with chronic hepatitis C-related cirrhosis presenting with acute epigastric pain and laboratory findings consistent with gallstone pancreatitis. He underwent a cholecystectomy, and histopathology review of the surgical specimen revealed a primary MALT lymphoma of the gallbladder. Staging studies revealed no evidence of metastatic disease but incidentally identified a malignancy of the prostate. The patient's gallbladder MALT lymphoma was managed con

Primary gallbladder small lymphocytic lymphoma as a rare postcholecystectomy finding.

Introduction. Primary lymphoma of the gallbladder is an extremely rare entity with approximately 50 cases reported so far. In many of these cases the presenting symptoms were mimicking symptomatic gallstone disease and the diagnosis was made postoperatively, especially when the preoperative imaging results were far from suspicious for malignant disease. Patients and Methods. We report a case of primary lymphoma of the gallbladder in an 85-year-old man with gallstone disease, who was admitted for elective cholecystectomy 2 months after an episode of acute cholecystitis and pancreatitis. [Dynamics of hepatobiliary system involvement in occupational COLD formation].

Study of 462 patients with occupational pulmonary diseases revealed that hepatobiliary diseases are among concomitants of occupational COLD. Inflammatory changes in gallbladder and bile ducts with bile dyskinesia are first manifestations of such diseases developing early at chronic nonobstructive bronchitis stage. Shortly, hepatosteatosis adds to these disorders. Final stage of the pathologic process in some patients is bile stones or liver cirrhosis.

Diffuse lymphoplasmacytic chronic cholecystitis is highly specific for extrahepatic biliary tract disease but does not distinguish between primary and secondary sclerosing cholangiopathy.

Previous studies of gallbladder pathology in primary sclerosing cholangitis (PSC) have suggested that a distinctive histologic triad ("diffuse lymphoplasmacytic acalculous cholecystitis," composed of diffuse, mucosal-based, dense lymphoplasmacytic infiltrates) is commonly present in gallbladders of patients with PSC and is relatively specific for that disease. However, prior control populations have included only patients with cholecystitis/cholelithiasis and hepatitis, and have not evaluated patients with non-PSC-associated extrahepatic biliary tract disease. We recently observed cases of dif

Porcelain Gallbladder.

Porcelain gallbladder or calcified gallbladder is a rare entity and is considered as the end stage of chronic cholecystitis. This disease is rarely diagnosed preoperatively and usually mimics carcinoma gallbladder. Hereby, we present a rare and interesting case of porcelain gallbladder that was diagnosed preoperatively and managed by cholecystectomy. Horizontal duodenal papilla is associated with a special spectrum of pancreaticobiliary diseases: a retrospective magnetic resonance cholangiopancreatography-based study.

BACKGROUND: Horizontal duodenal papilla (HDP) is not an uncommon ectopic major papilla. The impact of HDP on the occurrence of pancreaticobiliary diseases remains unclear. Here, we explored the associations in patients who underwent magnetic resonance cholangiopancreatography (MRCP). METHODS: Consecutive patients who underwent MRCP at Xijing Hospital (Xi'an, China) between January 2020 and December 2021 were eligible. Patients were divided into HDP and regular papilla (RP) according to the position of the major papilla. The primary outcome was the proportion of congenital pancreaticobiliary d

Multicenter investigation of lifestyle-related diseases and visceral disorders in thalidomide embryopathy at around 50 years of age.

BACKGROUND: In utero exposure to thalidomide causes a wide range of birth defects, including phocomelia, hearing loss and visceral disorders, known as thalidomide embryopathy (TE). Fifty years after the first report of TE, we conducted the first cross-sectional multicenter study to investigate the development of lifestyle-related diseases and identify risk factors for visceral disorders in subjects with TE. METHODS: Seventy-six cases with TE (31 men, 45 women) underwent medical examinations between 2011 and 2014 to determine the types of TE-related anomalies (limbs, auditory organs, or viscer
