Four hours after surgery for aortic valve stenosis and tricuspid valve regurgitation, an unknown foreign body was present on the routine chest X-ray. We performed re-sternotomy in order to retrieve this foreign body. The foreign body was easy to move on fluoroscopy but we could not extract it. We concluded that the foreign body was in a subdiaphragmatic location. As a consequence, we performed gastroscopy. A white, frothy mass (similar to an undissolved effervescent tablet) within an ulcerated lesion was seen and partially extracted.

An older woman presented to the hospital with abdominal pain and subsequently had three episodes of melaena, requiring blood transfusions. She was known to suffer with severe depression and was on high-dose fluoxetine. Gastroscopy and colonoscopy failed to reveal an underlying cause for gastrointestinal (GI) bleeding. Possibility of high-dose selective serotonin reuptake inhibitors causing GI bleed was raised. Fluoxetine was discontinued and the patient was commenced on mirtazapine. The patient had no further GI bleeding and had significant improvement in haemoglobin.

We report a case of a 65-year-old woman who presented with recurrent episodes of severe, postprandial abdominal pain followed by projectile vomiting. Gastroscopy revealed a large polyp in the prepyloric region. During peristalsis, the polyp was repeatedly 'passing' through the pylorus into the duodenal bulb, hence obstructing the lumen. The polyp was eventually removed in a piecemeal fashion. Histopathologic examination revealed an inflammatory fibroid polyp (known also as Vanek's tumour). Chronic atrophic gastritis and a sessile polyp were diagnosed during gastroscopy of a patient with splenomegaly and portal hypertension. The polyp was removed endoscopically. Histologically, it proved to be an endocrine tumour, which was identified as gastrinoma by immunohistochemistry. Report on 7 cases of massive hemorrhage from hiatal hernias. In all cases diagnosis was made before operation by oesophago-gastroscopy and/or by selective angiography. Early operation is the only successful therapy. The procedure depends on the kind of lesion and the type of hernia. In sliding hernia a Nissen fundoplication, in the paraesophageal type a fundoplication combined with gastropexy is performed. In cases of hyperacidity a combination with vagotomy and pylorplasty is necessary. Ulcers should be excised or resected by a Billroth-I-type gastrectomy. All seven patients could be cured.

RATIONALE: Gastric carcinoma of fundic gland type is a new type of gastric cancer with a low incidence rate. The mechanism of gastric carcinoma of the fundic gland differs significantly from that of common adenocarcinoma. While it typically presents as a benign growth, there are cases where it can progress to malignancy.
