Clinicopathologic attributes of common geriatric dermatologic entities.

Certain dermatologic lesions may initially present or be more commonly ascribed to the elderly. These disorders encompass a diverse array of etiologically unrelated degenerative, autoimmune, idiopathic, and neoplastic conditions that may dramatically impact the quality of life and produce significant morbidity and mortality. As the population ages, a more complete understanding of the clinical and histopathologic features unique to the geriatric dermatologic patient is essential.

Lamotrigine use in geriatric patients with bipolar depression.

OBJECTIVE: To study the effectiveness of adding lamotrigine to the treatment of inpatient geriatric patients with bipolar disorder (BD) who were in the depressed phase and had been on lithium and valproate for at least 3 months. METHOD: Lamotrigine was started at 25 mg given at bedtime, with weekly incremental increases of 12.5 mg daily until a total dosage of either 75 mg or 100 mg was obtained. Improvement was measured by clinical interview and Hamilton Depression Rating Scale (HDRS) scores. Patients were reassessed at 6 weeks, and if their HDRS score had decreased by at least 50%, they wer

[Prevalence and etiology of anemia in an institutionalized geriatric population].

OBJECTIVE: Anemia is a common problem in the elderly, but its etiology in this age group is not well known, nor is its correlation with the mental and functional states. The objectives of the present study were to determine the prevalence of anemia in a large institutionalized geriatric population, to diagnose the etiology, and to analyze the correlation of anemia with the nutritional, mental and functional status. METHODS: The study was performed in a 970 bed multilevel geriatric complex, from which 481 residents were randomly chosen. Their mean age was 81.4 +/- 8.7 years, and 46 of these re

Geriatric syndromes and assessment in older cancer patients.

Older individuals are at risk for adverse events in all settings where cancer is treated. Common geriatric syndromes can complicate cancer therapy, and thus, increase patient morbidity and the costs of care. Furthermore, cancer treatment can worsen geriatric syndromes. It is often difficult to determine whether declining health is a result of cancer treatment or the patient's underlying disease. Baseline assessment of multiple factors may facilitate detection of a decline in the patient's health status, which may be remediable. Geriatric syndromes may substantially affect quality of life and a

Vertigo, tinnitus, and hearing loss in the geriatric patient.

OBJECTIVE: To document clinical changes after a course of chiropractic care in a geriatric patient with vertigo, tinnitus, and hearing loss. CLINICAL FEATURES: A 75-year-old woman with a longstanding history of vertigo, tinnitus, and hearing loss experienced an intensified progression of these symptoms 5 weeks before seeking chiropractic care. Radiographs revealed a C3 retrolisthesis with moderate degenerative changes C4-C7. Significant decreases in audiologic function were evident, and the RAND 36 Health Survey revealed subjective distress. INTERVENTION AND OUTCOME: The patient received upp

Geriatric intertrochanteric hip fractures: an economic analysis.

Hip fractures have a high economic and social cost to society. Rising healthcare costs, coupled with managed healthcare systems, have forced a close inspection of healthcare expenditures. To evaluate the impact of geriatric intertrochanteric hip fractures upon a hospital system, an economic cost-analysis was undertaken. An analysis was made of financial and hospital data of elderly patients who sustained an intertrochanteric hip fracture and subsequently underwent open reduction and internal fixation. Increasing patient age did not correlate with length of stay, overall hospital cost, or net h

The effect of sequential antidepressant treatment on geriatric depression.

This study evaluated the outcome of elderly depressed patients who were refractory to, or intolerant of, an initial trial of nortriptyline and who then underwent systematic treatment with other antidepressant therapies. 101 patients entered the study and 61% responded to nortriptyline, 64% of patients who did not improve with nortriptyline eventually responded to other antidepressant treatments. Response to second-line treatment (phenelzine) was significantly slower than response to the tricyclic antidepressant. Treatment nonresponders were more likely to have a longer episode length at index

Clozapine use in female geriatric patients with psychoses.

This study was undertaken to evaluate the clinical response of geriatric patients with psychoses to the atypical neuroleptic drug, clozapine. Records of patients over 60 years of age (n = 12, all female) who had received clozapine over a 30-month period from selected hospitals in Western Pennsylvania were reviewed. Among the six patients who were rapidly titrated (300 mg/day in 3 weeks), none are currently receiving clozapine, while four patients who received a slower titration and lower dosage (range, 25 mg/day to 300 mg/day; mean, 150 mg/day) remain clinically improved on stable doses of clo

In-office management of common geriatric foot problems.

Foot disorders can be a major source of pain, discomfort, and disability for older adults. Neuropathy, impaired leukocyte function, vascular insufficiency, and trauma predispose older diabetics to limb-threatening complications. Therefore, examine the diabetic patient's foot during each visit. Painful foot disorders and lower extremity joint impairment represent major causes of treatable gait disturbance. Treatment of hyperkeratotic lesions (corns, calluses) consists of aseptic debridement and weight dispersion. Interspace maceration, fungal infections, and ulceration must also be managed with

Electrocardiographic changes in institutionalized geriatric glaucoma patients.

A geriatric study was conducted on 213 institutionalized geriatric glaucoma patients (mean age 83.9 years) and 100 control patients (mean age 81.3 years). A 12-lead electrocardiogram (ECG) analyzed according to the Minnesota code was recorded for 212 glaucoma patients and 95 control patients. The most frequent finding (in 56% of the glaucoma patients and in 38% of the control patients, P less than 0.05) was a negative or isoelectric T-wave, suggestive of ischemic heart disease. ECG findings suggestive of coronary heart disease (Q/QS patterns, ST-segment depression, negative or isoelectric T-wa

Orthostatic hypotension: therapeutic alternatives for geriatric patients.

Symptomatic orthostatic hypotension is a serious problem in the elderly because it can precipitate falls and fractures, myocardial infarctions, and strokes. Several disorders may cause symptomatic orthostatic hypotension including age-related changes in physiology, disorders of the autonomic nervous system, drugs, and a decrease in circulating blood volume. Orthostatic hypotension is defined as a fall in systolic pressure of at least 20-30 mm Hg and a fall in diastolic pressure of at least 10-15 mm Hg upon rising, with symptoms of cerebral ischemia. Management includes a search for reversible

Oral manifestations of drug therapies in the geriatric patient.

Oral manifestations of drug therapies in the geriatric patient must be considered in a multifactorial approach. A logical pattern of individual "normality" must be perceived, and variation from this pattern, with the aid of both physical examination and adequate drug history, will enable the clinician to interpret protean manifestations of untoward actions of chemotherapeutic agents in and about the oral cavity.

Eye disease in a geriatric nursing home population.

All 225 tenants (181 female and 44 male patients) of a nursing home in New York City underwent eye examinations at least once within a 1-year period to determine the prevalence of senile cataract, age-related macular degeneration (AMD), open-angle glaucoma, and diabetic retinopathy. The average age was 85.4 years (range, 60-108 years). With aphakia and pseudophakia included in the diagnosis of "cataract," the respective prevalences were found to be 81, 37, 11, and 2.1%. There was a statistically significant increase in the prevalence of cataracts and AMD in those patients 85 years of age or ol

Anal function in geriatric patients with faecal incontinence.

The association of faecal incontinence with constipation and confusion in the elderly is well recognised but the anal function of faecally incontinent geriatric patients is poorly understood. Anal studies were therefore performed on 99 geriatric patients (49 with faecal incontinence, 19 continent patients with faecal impaction and 31 geriatric control patients with normal bowel habit) and 57 younger healthy control patients. An age related reduction in anal squeeze pressure but not resting pressure was identified. A reduction in anal resting pressure was detected in the faecally incontinent ge

Normal pressure hydrocephalus in geriatric medicine: a challenge.

Normal pressure hydrocephalus (NPH) in elderly patients is reviewed. Since the clinical signs that characterize NPH--incontinence, difficulty walking, and dementia--occur frequently in the elderly in association with a wide variety of disorders, attention is paid to the differential diagnosis of NPH on the basis of both clinical findings and laboratory tests. Success rates for treatment of NPH using surgical shunting are discussed, as are mortality rates and complication rates for this type of surgery in the elderly. Factors that may be of value in predicting which NPH patients are most likely

Depression in a geriatric patient: alleviation by insertion of a pacemaker.

A geriatric patient had a number of depressive episodes over a 2 1/2-year period, each requiring hospital admission; no episode completely responded to antidepressant therapy. On his last admission an elective organic work-up revealed a sick sinus syndrome, for which he subsequently received a pacemaker, alleviating all of his symptoms.

Identifying a common--and benign--geriatric skin lesion.

Typical sebaceous hyperplasia is recognized by the yellowish color, lobulated configuration, and central umbilication. Other skin lesions that may be similar in appearance include basal cell epithelioma, molluscum contagiosum, and xanthoma. Chemical cauterization with phenol or trichloroacetic acid is worth trying if the sebaceous hyperplasia is in the middle or upper dermis. When the hyperplasia extends into the subcutaneous tissue, however, these topical agents are less effective.
