Percutaneous Interventions for Secondary Mitral Regurgitation.

Mitral regurgitation is frequently associated with ventricular dysfunction and carries a high mortality. Guideline-directed medical therapy, surgical mitral valve repair or replacement, and, in the setting of advanced heart failure, heart transplant and left ventricular assist devices have been the mainstay of treatment. However, rapid advancement in the field has resulted in approval of edge-to-edge mitral valve repair with the MitraClip, and there are several novel catheter-based percutaneous options in clinical trials. Percutaneous options, while promising, must be deployed in patients who

Temporary Mechanical Circulatory Support in Acute Heart Failure.

Cardiogenic shock (CS) is a challenging syndrome, associated with significant morbidity and mortality. Although pharmacological therapies are successful and can successfully control this acute cardiac illness, some patients remain refractory to drugs. Therefore, a more aggressive treatment strategy is needed. Temporary mechanical circulatory support (TCS) can be used to stabilise patients with decompensated heart failure. In the last two decades, the increased use of TCS has led to several kinds of devices becoming available. However, indications for TCS and device selection are part of a comp

New percutaneous interventions in heart failure.

Chronic heart failure (HF) is one of the main causes of mortality and morbidity worldwide and represents a growing challenge in interventional cardiology. Its prevalence is recently increasing due to the improvements in cardiac care with subsequent significant reduction in the HF acute mortality. HF may occur with either a reduced left ventricular ejection fraction (LVEF) (LVEF<40%, HFrEF) or a preserved EF (typically LVEF≥50%, HFpEF) indicative of diastolic dysfunction. Despite recent advances in drug therapies as well as in nonpharmacological strategies, including defibrillators, cardiac res

An Institutional Comparison of Patients Supported With Surgical vs Percutaneous Biventricular Assist Devices.

BACKGROUND: Surgical (sBiVADs) are well established in end-stage heart failure, whereas newer and less invasive percutaneous biventricular assist devices (pBiVADs) are understudied. These 2 approaches are reported in a large series of patients with pBiVADs. METHODS: All consecutive biventricular assist device (BiVAD)-supported patients at a single institution (Keck Hospital of USC, Los Angeles, CA) from 2014 to 2023 were retrospectively reviewed (n = 46). Patients with sBiVADs (n = 27) and pBiVADs (n = 19) (defined as Impella 5.5 with Impella RP [Abiomed; n = 4] or percutaneous oxygenated rig

Reversal of end-stage heart failure in juvenile hemochromatosis with iron chelation therapy: a case report.

BACKGROUND: Juvenile hemochromatosis is the most severe form of iron overloading phenotype. Although rare, it should be suspected in patients who present with hypogonadotropic hypogonadism, diabetes mellitus, or cardiomyopathy without a clear cause. CASE PRESENTATION: A young Serbian male presenting with end-stage heart failure was referred for extracorporeal membrane oxygenation. An endomyocardial biopsy revealed cytoplasmic iron deposits in myocytes. His condition was stabilized with biventricular assist devices and he was listed for heart transplantation. Iron chelation therapy was commenc

[Heart transplantation--indications and results].

Heart transplantation has become a standard procedure in the treatment for irreversible heart failure. Criteria for both recipients and donors have been extended. One year survival now reaches 81%. In the immediate postoperative course patients are endangered by infection. In the long-term course coronary artery disease of the transplanted heart becomes the most serious problem. Alteration of liver and kidney function due to chronic medication as well as malignancies and hypertension also occur. In case of chronic transplant failure retransplantation may be indicated. Acute cardiac failure bef

Challenges in advanced heart failure care in Japan: Bridging the gap in durable mechanical circulatory support utilization and heart transplantation.

In Japan, the incidence of heart failure has escalated to become a major cause of morbidity and mortality. Advanced therapeutic interventions, such as heart transplantation and durable left ventricular assist devices, have become a focus of interest in recent decades. However, the unique sociocultural landscape of Japan, coupled with a historically controversial background of organ transplantation, has impeded progress in these areas. This has resulted in a notable donor shortage and the absence of a robust framework for heart transplantation. This review aims to highlight the specific challen

Right ventricular failure: Current strategies and future development.

Right heart failure can be defined as a clinical syndrome consisting of signs and symptoms of heart failure resulting from right ventricular dysfunction. Function is normally altered due to three mechanisms: (1) pressure overload (2) volume overload, or (3) a decrease in contractility due to ischaemia, cardiomyopathy or arrythmias. Diagnosis is based upon a combination of clinical assessment plus echocardiographic, laboratory and haemodynamic parameters, and clinical risk assessment. Treatment includes medical management, mechanical assist devices and transplantation if recovery is not observe

Progress Toward a Portable Artificial Lung: A Systematic Review.

BACKGROUND: Chronic respiratory diseases are the third leading global cause of death. As lung transplantation is a definitive treatment for highly selected candidates, extracorporeal membrane oxygenation (ECMO) can be a life-sustaining therapy. Standard ECMO therapy immobilizes the patient, which increases morbidity and mortality. There is a need for a portable venovenous (VV)-ECMO device that allows hospital ambulation and ultimately home use. AIM: This systematic review aims to identify clinically utilized and preclinical portable respiratory assist devices and describe their efficiency, pr

Continuous-flow left ventricular assist device and the right ventricle.

Left ventricular assist devices (LVADs) have become accepted as treatment for heart failure as a result of improvements in diagnosing and treating left ventricular failure and limited donor availability. In the Pivotal Study of the HeartMate II in the bridge to transplantation population, the incidence of right ventricular failure without the implantation of a right ventricular assist device was 14%, with an additional 6% of the participants ill enough that they required implantation of a right ventricular assist device. This complication increases mortality, cost, and length of stay. This art

HeartMate 3 Left Ventricular Assist Device Implantation in a Pediatric Patient With Limb-Girdle Muscular Dystrophy.

BACKGROUND: The development of cardiac complications is common in patients with muscular dystrophy. However, advanced heart failure therapies such as implantation of durable ventricular assist devices and transplant are infrequently used in patients who develop cardiomyopathy, often because of comorbid impairments in mobility and respiratory function. CASE REPORT: A 16-year-old male with limb-girdle muscular dystrophy type R4 presented with chronic decompensated heart failure. Recent worsening of his respiratory function and mobility were confounded by severe heart failure. In addition to our

Long term CentriMag biventricular support using hybrid cannulation as a bridge to transplant in a pediatric patient.

Paracorporeal continuous-flow ventricular assist devices designed for short-term support can also potentially provide long-term circulatory support as bridges to transplantation in children. We describe the long-term use of the CentriMag biventricular assist device with multiple pump changes in a 9-year-old boy with idiopathic-dilated cardiomyopathy. The initially implanted Berlin Heart EXCOR pumps were replaced by CentriMag due to thromboembolic complications. The CentriMag pumps were exchanged 15 times due to clot and fibrin formation or when the pumps reached their expiration dates. Connect

From innovation to implementation: expanding global access to mechanical circulatory support and heart transplantation.

Mechanical circulatory support (MCS) and heart transplantation are established therapies for advanced heart failure and cardiogenic shock. Technological advances have improved outcomes, including randomized evidence supporting the use of temporary MCS, such as percutaneous ventricular assist devices, in selected patients with ST-elevation myocardial infarction-related cardiogenic shock, alongside substantial gains in survival with contemporary durable left ventricular assist devices (LVADs). Despite this progress, access remains highly inequitable. Although global transplant activity has incre

Use of an intrapericardial, continuous-flow, centrifugal pump in patients awaiting heart transplantation.

BACKGROUND: Contemporary ventricular assist device therapy results in a high rate of successful heart transplantation but is associated with bleeding, infections, and other complications. Further reductions in pump size, centrifugal design, and intrapericardial positioning may reduce complications and improve outcomes. METHODS AND RESULTS: We studied a small, intrapericardially positioned, continuous-flow centrifugal pump in patients requiring an implanted ventricular assist device as a bridge to heart transplantation. The course of investigational pump recipients was compared with that of pa

Current and future options for adult biventricular assistance: a review of literature.

In cardiogenic shock various short-term mechanical assistances may be employed, including an Extra Corporeal Membrane Oxygenator and other non-dischargeable devices. Once hemodynamic stabilization is achieved and the patient evolves towards a persisting biventricular dysfunction or an underlying long-standing end-stage disease is present, aside from Orthotopic Heart Transplantation, a limited number of long-term therapeutic options may be offered. So far, only the Syncardia Total Artificial Heart and the Berlin Heart EXCOR (which is not approved for adult use in the United States unlike in Eur
